IgG represents the 75% of <span class=\"term\" data-term-id=\"426\">antibodies<\/span> circulating in the blood, which are an essential part of the immune response secondary to <span class=\"term\" data-term-id=\"145\">infection<\/span> and <span class=\"term\" data-term-id=\"1922\">toxins<\/span>. IgG is made by <span class=\"term\" data-term-id=\"1513\">plasma<\/span> cells, a specific type of B <span class=\"term\" data-term-id=\"420\">lymphocyte<\/span>.<\/p>\n<p>There are four IgG subclasses. The IgG4 subclass is the least common of these, representing approximately 4% of IgG in <span class=\"term\" data-term-id=\"1430\">serum<\/span>. IgG4 has a unique structure. Its specific biological role is uncertain. However, IgG4 is known to play a role in protection against type 1 <span class=\"term\" data-term-id=\"943\">hypersensitivity<\/span> reactions, for example to bee venom, and in the <span class=\"term\" data-term-id=\"527\">Pathogenesis<\/span> of <span class=\"term\" data-term-id=\"691\">autoimmune<\/span> blistering diseases such as pemphigus vulgaris, <span class=\"term\" data-term-id=\"331\">helminth<\/span> infections and <span class=\"term\" data-term-id=\"1742\">malignancy<\/span> even <span class=\"term\" data-term-id=\"1670\">melanoma<\/span>.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFQue-es-la-enfermedad-relacionada-con-IgG4\"><\/span>What is IgG4 related disease?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>IgG4-related disease is a recently described rare disease <span class=\"term\" data-term-id=\"350\">syndrome<\/span> consisting of many disease entities previously thought to be unrelated. IL-10 cytokines (<span class=\"term\" data-term-id=\"915\">interleukin<\/span> 10) and TGF-\u03b2 (transforming <span class=\"term\" data-term-id=\"523\">growth factor<\/span> Beta) are known to support IgG4 production and are at elevated levels in IgG4-related disease.<\/li>\n<\/ul>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFQuien-contrae-la-enfermedad-relacionada-con-IgG4\"><\/span>Who gets IgG4-related disease?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p><span>Most of the patients described with IgG4-related disease are middle-aged or older men.<\/span><\/p>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFCuales-son-las-caracteristicas-clinicas-de-la-enfermedad-relacionada-con-IgG4\"><\/span>What are the clinical features of IgG4-related disease?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>IgG4-related disease occurs in several ways.<\/p>\n<ul>\n<li>Most people with IgG4-related disease are constitutionally well. <\/li>\n<li>IgG4-related disease can be diagnosed as an incidental finding when someone undergoes a routine <span class=\"term\" data-term-id=\"1005\">radiological<\/span> or surgical research.<\/li>\n<li>60-90% of people with IgG4-related disease have multiple organ involvement.<\/li>\n<li>IgG4-related disease often occurs as swelling or within an organ (such as skin, orbit, and lung); This is known as an inflammatory pseudotumor.<\/li>\n<li>IgG-related autoimmune pancreatitis is more likely than other forms of IgG-related disease to be associated with the disease elsewhere, such as the hilum <span class=\"term\" data-term-id=\"503\">lymphadenopathy<\/span>, bile duct injuries, <span class=\"term\" data-term-id=\"1434\">lacrimal<\/span> and compromise of the salivary glands, <span class=\"term\" data-term-id=\"1641\">hypothyroidism<\/span>and retroperitoneal fibrosis.<\/li>\n<li>Lacrimal, parotid or submandibular salivary disease related to IgG4, <span class=\"term\" data-term-id=\"1051\">pneumonitis<\/span>and kidney disease are associated with pancreatitis in <20% de los casos.<\/li>\n<li>Patients with IgG4-related disease may have symptoms of <span class=\"term\" data-term-id=\"1136\">allergy<\/span> like asthma<\/li>\n<li>Up to 40-80% of patients with IgG4-related disease have lymphadenopathy.<\/li>\n<li>\n<span class=\"term\" data-term-id=\"47\">Cutaneous<\/span> Manifestations of IgG4-related disease include <span class=\"term\" data-term-id=\"967\">erythematous<\/span> <span class=\"term\" data-term-id=\"368\">plates<\/span>, <span class=\"term\" data-term-id=\"366\">papules<\/span>and <span class=\"term\" data-term-id=\"900\">subcutaneous<\/span> nodules, often itchy, and usually on the face or forearm. The diagnostic criteria have not been fully developed.<\/p>\n<p>Investigation into suspected IgG4-related disease requires a combination of clinical, <span class=\"term\" data-term-id=\"1552\">endoscopic<\/span>radiological and <span class=\"term\" data-term-id=\"620\">serological<\/span> Tests for organ involvement and terminal organ damage (eg, hormonal abnormalities).<\/p>\n<p>Tissue diagnosis requires a <span class=\"term\" data-term-id=\"24\">biopsy<\/span> of affected organic tissues, including skin biopsy. See IgG4-related skin disease <span class=\"term\" data-term-id=\"223\">pathology<\/span>. Features include:<\/p>\n<ul>\n<li>Lymphoplasmacytic <span class=\"term\" data-term-id=\"541\">infiltrate<\/span>\n<\/li>\n<li>Stormy fibrosis pattern<\/li>\n<li>Plasma cells positive for IgG4 (the number of positive cells to confirm the diagnosis depends on the tissue type; most&gt; 30-50 per high-power field, some, such as kidney, 10 per high-power field).<\/li>\n<\/ul>\n<p>Blood tests are not diagnostic, but can show:<\/p>\n<ul>\n<li>\n<span class=\"term\" data-term-id=\"1374\">Peripheral<\/span> <span class=\"term\" data-term-id=\"83\">eosinophilia<\/span>\n<\/li>\n<li>IgG4 raised serum.<\/li>\n<\/ul>\n<p>There are specific criteria for the diagnosis of some specific tissue disorders, such as autoimmune pancreatitis.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFComo-se-trata-la-enfermedad-relacionada-con-IgG4\"><\/span>How is IgG4-related disease treated?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>IgG4-related disease is usually treated with <span class=\"term\" data-term-id=\"380\">systemic<\/span> steroids, often prednisone 40 mg per day for 2 to 4 weeks followed by a gradual decrease in dose.<\/p>\n<p>In patients who cannot withdraw from prednisone, a steroid sparing agent such as azathioprine or mycophenolate can be used. Rituximab, a <span class=\"term\" data-term-id=\"429\">Cell B<\/span>exhausting <span class=\"term\" data-term-id=\"466\">monoclonal<\/span> <span class=\"term\" data-term-id=\"425\">antibody<\/span>, has been used with some success.<\/p>\n<p>Specific organ replacement therapy may be required:<\/p>\n<ul>\n<li>Thyroxine for thyroiditis (thyroid disease) causing hypothyroidism<\/li>\n<li>Pancreatic <span class=\"term\" data-term-id=\"1525\">enzyme<\/span> replacement for pancreatic insufficiency<\/li>\n<li>\n<span class=\"term\" data-term-id=\"1510\">Insulin<\/span> for diabetes mellitus<\/li>\n<li>Hormone replacement for hypopituitarism: hydrocortisone, thyroxine, growth hormone, desmopressin, and <span class=\"term\" data-term-id=\"1167\">sex<\/span> hormones<span class=\"term\" data-term-id=\"567\">testosterone<\/span> for men; <span class=\"term\" data-term-id=\"1844\">estrogen<\/span> and <span class=\"term\" data-term-id=\"1841\">progesterone<\/span> for women).<\/li>\n<\/ul>\n<h2><span class=\"ez-toc-section\" id=\"Cual-es-el-pronostico-de-la-enfermedad-relacionada-con-IgG4\"><\/span>Which is the <span class=\"term\" data-term-id=\"241\">forecast<\/span> of IgG4-related disease?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>The prognosis of IgG4-related disease is variable.
