It can also be spread to other people. Symptoms of an active infection can include:<\/p>\n<ul>\n<li>\n<span class=\"term\" data-term-id=\"1176\">Fever<\/span>, night sweats<\/li>\n<li>Cough<\/li>\n<li>Chest pain<\/li>\n<li>Expectorate <span class=\"term\" data-term-id=\"1874\">sputum<\/span> I blood<\/li>\n<li>Weakness, fatigue<\/li>\n<li>Weight loss, loss of appetite.<\/li>\n<\/ul>\n<p>Active TB often affects the lungs, but it can affect any part of the body, including the skin (<span class=\"term\" data-term-id=\"47\">cutaneous<\/span> TUBERCULOSIS).<\/p>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFQue-es-la-deteccion-de-tuberculosis\"><\/span>What is TB screening?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>TB screening refers to the process of examining and evaluating a patient who does not have any symptoms of active TB. An evaluation is done to identify and treat <span class=\"term\" data-term-id=\"1040\">latent<\/span> TB so that it can be effectively treated before it becomes an active infection.<\/p>\n<div id=\"square-placement-country-holder\" class=\"country-dependent advert\">\n<div class=\"box-placement square-placement\">\n<div class=\"advert__frame\">\n<div id=\"dermnet-dermnet-mobbox\">\n<\/div><\/div><\/div><\/div>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFQue-es-la-tuberculosis-latente\"><\/span>What is latent tuberculosis?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Latent TB occurs when patients have been alive <em>M. tuberculosis<\/em> <span class=\"term\" data-term-id=\"935\">bacteria<\/span> without knowing that they are infected because their immune system has blocked the bacteria. In healthy people, the tuberculosis bacteria can remain alive in the body without causing illness for many years. Latent TB does not infect other people.<\/p>\n<p>If the immune system breaks down, for example if normal <span class=\"term\" data-term-id=\"433\">cytokine<\/span> TNF\u03b1 is eliminated by certain treatments, latent TB can turn into active TB.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFQuien-esta-en-riesgo-de-tuberculosis\"><\/span>Who is at risk for tuberculosis?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Active or latent TB infection is not common in New Zealand and other developed countries, but it continues to be reported in certain population groups. Risk factors for TB infection include:<\/p>\n<ul>\n<li>Close contact with a TB patient<\/li>\n<li>Living in or visiting a country or community where TB is common<\/li>\n<li>Living in a crowded community, which includes an institution such as a nursing home, a long-stay hospital, a prison<\/li>\n<li>Working in hospitals and healthcare settings.<\/li>\n<\/ul>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFCuando-se-debe-realizar-el-examen-de-tuberculosis\"><\/span>When should the tuberculosis test be done?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>TB screening should be done in people at risk for TB and in people at risk of converting latent, undetected TB to active TB. For example:<\/p>\n<ul>\n<li>Treatment with corticosteroids, especially biologics. <span class=\"term\" data-term-id=\"289\">Tumor<\/span> <span class=\"term\" data-term-id=\"403\">Necrosis<\/span> Inhibitors of factor alpha (TNF\u03b1) and immunosuppressive agents such as azathioprine, cyclosporine, mycophenolate or <span class=\"term\" data-term-id=\"1619\">chemotherapy<\/span>\n<\/li>\n<li>Disease, particularly the human immunodeficiency virus (<span class=\"term\" data-term-id=\"127\">HIV<\/span>) infection, <span class=\"term\" data-term-id=\"613\">leukemia<\/span>, <span class=\"term\" data-term-id=\"976\">lymphoma<\/span>, diabetes, alcoholism and <span class=\"term\" data-term-id=\"896\">renal<\/span> failure<\/li>\n<li>Weakness (old age, very young, and long-term or heavy smokers).<\/li>\n<\/ul>\n<div id=\"square-placement-country-holder\" class=\"country-dependent advert\">\n<div class=\"box-placement square-placement\">\n<div class=\"advert__frame\">\n<div id=\"dermnet-dermnet-mobbox\">\n<\/div><\/div><\/div><\/div>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFQue-pruebas-de-deteccion-de-tuberculosis-se-realizan\"><\/span>What TB tests are done?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>After taking a history and general exam, the main TB tests are:<\/p>\n<ul>\n<li>Chest x-ray<\/li>\n<li>Tuberculin skin test<\/li>\n<li>TB blood test (in adults).<\/li>\n<\/ul>\n<p>TB skin tests and blood tests are usually positive in patients with latent TB; they demonstrate previous infection and a good level of immunity to it. The tests are also positive in patients with active TB.<\/p>\n<p>Test results may require expert interpretation. Other tests may also be required, depending on the symptoms.<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Radiografia-de-pecho\"><\/span>Chest x-ray<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>Latent tuberculosis may be suspected on chest radiograph if any of the following is \/ is present:<\/p>\n<ul>\n<li>\n<span class=\"term\" data-term-id=\"369\">Fibrotic<\/span> zone<\/li>\n<li>Apical scars<\/li>\n<li>Blunt costophrenic angle<\/li>\n<li><span class=\"term\" data-term-id=\"1044\">Calcification<\/span><\/li>\n<li>\n<span class=\"term\" data-term-id=\"1538\">Pleural<\/span> thickening.<\/li>\n<\/ul>\n<p>Patients with active TB may have cavitation or other signs on the x-ray. A patient with latent TB may have a normal chest X-ray.<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Prueba-cutanea-de-tuberculina\"><\/span>Tuberculin skin test<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>The tuberculin skin test or Mantoux test contains a purified protein derivative (PPD) of the tuberculosis bacteria, which is why it is also called a PPD test. The test material does not contain live bacteria and cannot cause TB. The test is performed by specially trained laboratory technicians and other healthcare workers.<\/p>\n<ul>\n<li>The test is usually done on the inner aspect of a forearm.<\/li>\n<li>The selected area is first cleaned with an alcohol swab.<\/li>\n<li>0.1 ml of tuberculin (F TU) is injected intradermally, elevating a blister 6 to 10 mm in diameter.<\/li>\n<\/ul>\n<p>The test site is inspected 2-3 days later. The diameter of the thickening of the hard skin is recorded in millimeters. The test is classified as negative (not <span class=\"term\" data-term-id=\"143\">hardening<\/span> or less than 5 mm induration) or abnormal (more than 5 mm induration). There are several options, including QuantiFERON Gold \u2122. An advantage of IGRA over skin testing is that it is not positive after BCG vaccination. One downside is the difficulty and expense of performing the test accurately.<\/p>\n<p>Test results can be positive (infection likely), negative (unlikely), indeterminate, or equivocal (uncertain). Borderline reactions are seen more frequently in patients taking immunosuppressants, <span class=\"term\" data-term-id=\"380\">systemic<\/span> steroids and antimalarial drugs in patients who do not have tuberculosis. The test should be repeated at a later date, as a precautionary measure.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"Mi-prueba-es-negativa-%C2%BFPuedo-estar-seguro-de-que-no-tengo-TB\"><\/span>My test is negative. Can I be sure I don&#039;t have TB?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Even if initially negative, TB tests should be repeated from time to time in patients at risk of becoming infected with TB and in those at risk of latent TB turning into active TB.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFQue-sucede-si-la-prueba-es-positiva\"><\/span>What happens if the test is positive?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>If the test is positive, the result should be evaluated by a doctor. Other tests and follow-up may be appropriate.<\/p>\n<p>TB treatment is recommended in patients with active TB and in those at risk of conversion of latent TB to active TB, for example, if they have a <span class=\"term\" data-term-id=\"319\">chronic<\/span> immunodeficiency disease or treatment with an immunosuppressive drug.<\/p>\n<p>The usual treatment is isoniazid for 6 months, rifampicin for 3 months, or a combination.
