It can occur through reduced production or increased loss of red blood cells.<\/p>\n<p>Three essential elements must be present to produce red blood cells: iron, vitamin B12, and folic acid. The most common cause of anemia is iron deficiency, which affects more than 2 billion people worldwide.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFQue-es-la-anemia-por-deficiencia-de-hierro\"><\/span>What is iron deficiency anemia?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>The estimate <span class=\"term\" data-term-id=\"589\">predominance<\/span> of iron deficiency worldwide is twice that of iron deficiency anemia. Iron deficiency anemia occurs when there is not enough iron to create red blood cells <\/p>\n<div id=\"square-placement-country-holder\" class=\"country-dependent advert\">\n<div class=\"box-placement square-placement\">\n<div class=\"advert__frame\">\n<div id=\"dermnet-dermnet-mobbox\">\n<\/div><\/div><\/div><\/div>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFQuien-tiene-deficiencia-de-hierro\"><\/span>Who is deficient in iron?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>The main groups at risk for iron deficiency and iron deficiency anemia are preschool children, adolescents, pregnant women, and young people, which are times of increased <span class=\"term\" data-term-id=\"1262\">physiological<\/span> Need for iron.  <\/p>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFQue-causa-la-deficiencia-de-hierro\"><\/span>What causes iron deficiency?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>In people living in developing countries, iron deficiency tends to be due to insufficient dietary iron intake or blood loss from intestinal worm colonization. In high-income countries, iron deficiency can result from a vegetarian diet, <span class=\"term\" data-term-id=\"319\">chronic<\/span> blood loss or malabsorption.<\/p>\n<ul>\n<li>Diet-related iron deficiency\n<ul>\n<li>\n<p>Malnutrition: poverty, premature babies (milk is a poor source of iron), young children who are fussy<\/li>\n<li>Strict vegetarian and vegan diets<\/li>\n<li>Cereal-based diets: decreases the bioavailability of iron, since the phytates in grains reduce iron absorption.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<ul>\n<li>Loss of blood\n<ul>\n<li>Heavy menstruation (periods)<\/li>\n<li>Gastrointestinal bleeding - from peptide <span class=\"term\" data-term-id=\"290\">ulcer<\/span>, polyps or <span class=\"term\" data-term-id=\"30\">Cancer<\/span>, can occur over a long period<\/li>\n<li>Excessive blood donation. <\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<ul>\n<li>Gastrointestinal iron deficiency\n<ul>\n<li>Malabsorption<\/li>\n<li>Crohn&#039;s disease<\/li>\n<li>Helicobacter <span class=\"term\" data-term-id=\"145\">infection<\/span> or <span class=\"term\" data-term-id=\"809\">atrophic<\/span> gastritis, which can also lead to vitamin B12 deficiency<\/li>\n<li>Intestinal parasitic infections, such as hookworm or tapeworm<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<ul>\n<li>Medication-related iron deficiency\n<ul>\n<li>Aspirin and nonsteroidal anti-steroids<span class=\"term\" data-term-id=\"490\">inflammatory<\/span> drugs - cause gastritis<\/li>\n<li>Proton pump inhibitors - may affect iron absorption<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<ul>\n<li>Other conditions\n<ul>\n<li>The pregnancy<\/li>\n<li>Bleeding disorders, such as von Willebrand disease<\/li>\n<li>Final stage <span class=\"term\" data-term-id=\"896\">renal<\/span> failure: a combination of blood loss from <span class=\"term\" data-term-id=\"1791\">dialysis<\/span> and low levels of erythropoietin (a hormone that stimulates the production of red blood cells)<\/li>\n<li>Congestive heart failure, possibly due to subclinical <span class=\"term\" data-term-id=\"148\">inflammation<\/span> and impaired iron absorption<\/li>\n<li>Myelodysplasia: disease of the bone marrow that can present with anemia.<\/li>\n<li>\n<span class=\"term\" data-term-id=\"1162\">Intravascular<\/span> <span class=\"term\" data-term-id=\"1523\">hemolysis<\/span> (rare) as in paroxysmal nocturnal hemoglobinuria<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFCuales-son-las-caracteristicas-clinicas-de-la-deficiencia-de-hierro\"><\/span>What are the clinical features of iron deficiency?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>The signs and symptoms of an iron deficiency depend on whether the patient is <span class=\"term\" data-term-id=\"1084\">anemic<\/span>, and if so, how fast does anemia develop. Responds quickly to iron replacement.<\/li>\n<li>Beeturia: excretion of red urine with the consumption of beets. In people with normal iron levels, ferric ions discolor betalaine (red <span class=\"term\" data-term-id=\"231\">pigment<\/span> in beets). In iron deficient states, there are inadequate amounts of iron to discolor this pigment.<\/li>\n<li>Restless legs <span class=\"term\" data-term-id=\"350\">syndrome<\/span> - marked leg discomfort that occurs at rest and is relieved by movement.<\/li>\n<\/ul>\n<p>Iron deficiency can also <span class=\"term\" data-term-id=\"1495\">predispose<\/span> to <span class=\"term\" data-term-id=\"937\">bacterial<\/span> and fungal infections like impetigo, boils and yeast infections.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFQue-pruebas-se-deben-hacer\"><\/span>What tests should be done?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<h3><span class=\"ez-toc-section\" id=\"Cuenta-llena-de-sangre\"><\/span>full blood count<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>A complete or complete blood count (BCF, CBC) is essential to detect anemia. Iron deficiency can be present when blood count rates are normal.<\/p>\n<p>If anemia is due to iron deficiency, the cells are smaller and contain less <span class=\"term\" data-term-id=\"1205\">hemoglobin<\/span> resulting in a reduced red blood cell count or <span class=\"term\" data-term-id=\"1294\">hematocrit<\/span>, mean corpuscular volume (MCV) and mean cellular hemoglobin concentration (MCH). The reticulocytic hemoglobin (Ret-Hb) content, which tends to be low in iron deficiency anemia, can be used to control the response to iron replacement. Red blood cells <span class=\"term\" data-term-id=\"69\">distribution<\/span> The width (RDW) may reveal a mixed deficiency of iron and vitamin B12 as this results in variable sized red blood cells.<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Ferritina\"><\/span><span class=\"term\" data-term-id=\"1579\">Ferritin<\/span><span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>Ferritin is a measure of iron stores and is the most sensitive and specific test for iron deficiency. Low ferritin levels below 15 \u03bcg \/ ml are diagnostic of iron deficiency. Levels above 40 \u03bcg \/ ml in a healthy person are considered optimal.<\/p>\n<p>Normal or high levels of ferritin do not exclude iron deficiency, because ferritin acts as a <span class=\"term\" data-term-id=\"349\">acute<\/span> Phase reagent. Levels are higher in the presence of chronic inflammation (eg, Rheumatoid <span class=\"term\" data-term-id=\"1578\">arthritis<\/span>) when <span class=\"term\" data-term-id=\"692\">erythrocytes<\/span> sedimentation rate (<span class=\"term\" data-term-id=\"1114\">ESR<\/span>) or <span class=\"term\" data-term-id=\"946\">C-reactive protein<\/span> (<span class=\"term\" data-term-id=\"947\">CRP<\/span>) are elevated. Against the background of inflammation, significantly higher cut-off values are used for ferritin (eg, 100 \u00b5g \/ ml) and are more predictive of iron deficiency. Ferritin is also higher in patients with chronic kidney disease and heart failure.<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Otras-pruebas-de-hierro\"><\/span>Other iron tests<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>In iron deficiency:<\/p>\n<ul>\n<li>\n<span class=\"term\" data-term-id=\"1430\">Serum<\/span> iron is reduced - be aware that serum iron can be highly variable, fluctuate throughout the day, and serum iron is not helpful in evaluating iron stores<\/li>\n<li>Iron binding capacity is increased - a measure of iron&#039;s ability to bind with transferrin (an iron transporter)<\/li>\n<li>Transferrin saturation is reduced <\/li>\n<li>Soluble transferrin <span class=\"term\" data-term-id=\"782\">receiver<\/span> (sTfR) is reduced: this reflects the total body reserves, except if there is a disease of the bone marrow. sTfR is an expensive test. It is useful to discriminate iron deficiency in difficult cases, for example, in patients with chronic kidney failure or chronic inflammation such as rheumatoid arthritis. It is not modified in anemia of chronic diseases.<\/li>\n<\/ul>\n<p>Retest iron status after three months of iron supplementation.<\/p>\n<p>Older patients sometimes have unexplained iron deficiency anemia.
