It should not be used to treat melanoma in patients with a normal or wild type <em>BRAF<\/em> gene.<\/p>\n<p>In 2015, the United States Food and Drug Administration (FDA) approved cobimetinib (Cotellic \u2122, Genentech Inc. California, USA) in combination with vemurafenib (a specific BRAF kinase inhibitor) for the treatment of melanoma .<\/p>\n<p>In the same year, the Committee for Medicinal Products for Human Use of the European Medicines Agency issued a positive opinion for the marketing authorization of cobimetinib in the European Union.<\/p>\n<p>Cobimetinib has since received approval for marketing in New Zealand through Medsafe in 2017 for the treatment of melanoma patients.<\/p>\n<p>The FDA approval was based on the results of the Phase III CoBRIM study, which showed that cobimetinib, when used in combination with vemurafenib, reduced the risk of disease worsening or death by about half in melanoma patients.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"Evidencia-clave-de-ensayos-clinicos-para-cobimetinib\"><\/span>Key evidence from clinical trials for cobimetinib<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<h3><span class=\"ez-toc-section\" id=\"El-estudio-CoBRIM\"><\/span>The CoBRIM study<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>CoBRIM was an international, randomized, <span class=\"term\" data-term-id=\"1750\">double-blind<\/span>, <span class=\"term\" data-term-id=\"1103\">placebo<\/span>controlled phase III that evaluated the safety and <span class=\"term\" data-term-id=\"914\">effectiveness<\/span> cobimetinib 60 mg once daily plus vemurafenib 960 mg twice daily compared to vemurafenib 960 mg twice daily plus placebo. A total of 495 patients participated in the study, all of whom had not been previously treated. <em>BRAF<\/em> V600 <span class=\"term\" data-term-id=\"870\">mutation<\/span>-positive, unresectable, locally advanced or <span class=\"term\" data-term-id=\"1000\">metastatic<\/span> melanoma.<\/p>\n<p>The presence of <em>BRAF<\/em> The V600 mutation was detected using the cobas<sup>\u00ae<\/sup>; 407 4800 <em>BRAF<\/em> V600 mutation test.<\/p>\n<ul>\n<li>All patients received vemurafenib 960 mg orally twice daily on days 1\u201328.<\/li>\n<li>Patients were randomized to receive oral cobimetinib 60 mg or placebo equivalent once daily on days 1 to 21 of each 28-day cycle. <span class=\"term\" data-term-id=\"1748\">Randomization<\/span> It was stratified by geographic region (North America vs. Europe vs. Australia \/ New Zealand \/ others) and disease stage (unresectable stage IIIc, M1a or M1b vs. stage M1c).<\/li>\n<li>Treatment continued until disease progression, unacceptable. <span class=\"term\" data-term-id=\"1160\">toxicity<\/span>or withdrawal of consent.<\/li>\n<li>Patients randomized to placebo were not offered cobimetinib at the time of disease progression.<\/li>\n<li>The main efficacy outcome was investigator-assessed progression-free survival (PFS) according to RECIST v1.1.<\/li>\n<li>Secondary end points included PFS by an independent review committee, objective response rate, overall survival, and duration of response.<\/li>\n<\/ul>\n<p>The efficacy results are summarized in Table 1.<\/p>\n<section class=\"textBlock imageLinkBlockTitle\">\n<h5 class=\"colour--primary\"><span class=\"ez-toc-section\" id=\"Resultados-de-eficacia-del-estudio-CoBRIM\"><\/span>CoBRIM Study Efficacy Results<span class=\"ez-toc-section-end\"><\/span><\/h5>\n<\/section>\n<section class=\"imageLinkBlock\">\n<div class=\"flex\">\n<div class=\"imageLinkBlock__item__image\">\n<p>                        <img alt='table-7__protectwyjqcm90zwn0il0_focusfillwzi5ncwymjisinkildqzxq-8519590-4006995' src='https:\/\/doctorhoogstra.com\/wp-content\/uploads\/2020\/04\/Table-7__ProtectWyJQcm90ZWN0Il0_FocusFillWzI5NCwyMjIsInkiLDQzXQ-8519590.png'>\n<\/div>\n<p class=\"p-small\">CoBRIM Study Efficacy Results<\/p>\n<\/p><\/div>\n<\/section>\n<div class=\"clearfix imageLinkBlock\"><\/div>\n<h3><span class=\"ez-toc-section\" id=\"Eventos-adversos-%E2%80%93-experiencia-en-ensayos-clinicos-estudio-CoBRIM\"><\/span>\n<span class=\"term\" data-term-id=\"909\">Adverse events<\/span> - experience in clinical trials (CoBRIM study)<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>The most common (\u2265 20%) <span class=\"term\" data-term-id=\"1333\">Adverse reactions<\/span> with cobimetinib were:<\/p>\n<ul>\n<li>Diarrhea<\/li>\n<li>\n<span class=\"term\" data-term-id=\"706\">Photosensitivity<\/span> reaction<\/li>\n<li>Nausea<\/li>\n<li>Pyrexia<\/li>\n<li>Vomiting<\/li>\n<\/ul>\n<p>Because clinical trials are conducted under highly variable conditions, the <span class=\"term\" data-term-id=\"1794\">adverse reaction<\/span> rates seen in clinical trials may not reflect rates seen in practice<span>. <\/span><span>In the CoBRIM study, 15% of patients receiving cobimetinib experienced an adverse reaction that resulted in permanent discontinuation of the drug.<\/span><\/p>\n<p>The most common adverse reactions that led to permanent drug discontinuation were:<\/p>\n<ul>\n<li>Liver laboratory abnormalities, defined as increased <span class=\"term\" data-term-id=\"987\">aspartate aminotransferase<\/span> (2.4%), increased gamma glutamyltransferase (1.6%) and increased <span class=\"term\" data-term-id=\"990\">alanine aminotransferase<\/span> (<span class=\"term\" data-term-id=\"991\">ALT<\/span>; 1,6%)<\/li>\n<li>\n<span class=\"term\" data-term-id=\"255\">Eruption<\/span> (1,6%)<\/li>\n<li>Pyrexia (1.2%)<\/li>\n<li>Retinal detachment (2%).<\/li>\n<\/ul>\n<p>Among the 247 patients who received cobimetinib, adverse reactions resulted in dose interruption or reductions in 55%. Healthcare professionals in New Zealand are advised to report any suspected adverse reactions to the New Zealand Center for Pharmacovigilance.<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Sobredosis\"><\/span>Overdose<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>There is no experience with cobimetinib overdose in human clinical trials. In the event of a suspected overdose, cobimetinib should be discontinued and supportive care instituted. There is no specific antidote for overdose with cobimetinib.<\/p>\n<p>The New Zealand National Poisons Center should be contacted on 0800 764 766 for advice on managing an overdose.<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Cobimetinib-%E2%80%93-potencial-futuro\"><\/span>Cobimetinib - future potential<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<ul>\n<li>New treatments for metastatic melanoma work through different mechanisms to enhance immune response and blockage <span class=\"term\" data-term-id=\"956\">cellular<\/span> <span class=\"term\" data-term-id=\"893\">proliferation<\/span>.<\/li>\n<li>Like trametinib, cobimetinib is a <span class=\"term\" data-term-id=\"614\">MEK inhibitor<\/span>. MEK1 and MEK2 are tyrosine kinases that interact with BRAF and lead to uncontrolled growth of melanoma cells.<\/li>\n<li>Clinical trials have shown that the addition of cobimetinib to vemurafenib results in improved survival in melanoma patients harboring the BRAF V600 mutation.<\/li>\n<li>Overall survival in patients treated with nivolumab and pembrolizumab appears to be <span class=\"term\" data-term-id=\"277\">higher<\/span> compared to that achieved with cobimetinib and vemurafenib.<\/li>\n<li>However, overall survival in patients treated with a combination of vemurafenib and cobimetinib appears to be similar to other available treatment strategies for the treatment of unresectable metastases. <span class=\"term\" data-term-id=\"176\">evil one<\/span> melanoma, such as combinations of nivolumab and ipilimumab and dabrafenib with trametinib.<\/li>\n<li>For PFS, dabrafenib in combination with trametinib and vemurafenib in combination with cobimetinib appeared to have a higher probability of good performance than other available treatment strategies.<\/li>\n<li>For serious adverse events, pembrolizumab and nivolumab appeared to have a higher probability of fewer serious adverse events than cobimetinib and vemurafenib.<\/li>\n<li>However, neither of these interventions is cost-effective at maximum pharmacy retail prices, and the budgetary impacts on clinical practice can be substantial.<\/li>\n<li>The fast <span class=\"term\" data-term-id=\"289\">tumor<\/span> response to BRAF \/ MEK <span class=\"term\" data-term-id=\"1021\">inhibition<\/span> It is often short-lived as tumors develop resistance to combination therapy.<\/li>\n<li>New trials are beginning to investigate the addition of a third targeted agent or immunotherapy to increase the durability of response to treatment.<\/li>\n<\/ul>\n<section class=\"textBlock\">\n<div class=\"grey-box\"><em><span style=\"font-size: 0.9em\">New Zealand approved data sheets are the official source of information for these prescription drugs, including approved uses and risk information.
