It is the first one <span class=\"term\" data-term-id=\"466\">monoclonal<\/span> <span class=\"term\" data-term-id=\"425\">antibody<\/span> available for hereditary angioedema.<\/p>\n<p>Lanadelumab was approved by the United States Food and Drug Administration (FDA) in August 2018, in Canada in September 2018 and by the European Commission in November 2018. Regulatory applications for the marketing of lanadelumab are also being considered in Australia. Lanadelumab is not currently available in New Zealand.<\/p>\n<p>Lanadelumab is marketed under the brand name Takhzyro \u2122 by Dyax Corp (a subsidiary of Shire) based in Burlington, MA, USA.<\/p>\n<div id=\"square-placement-country-holder\" class=\"country-dependent advert\">\n<div class=\"box-placement square-placement\">\n<div class=\"advert__frame\">\n<div id=\"dermnet-dermnet-mobbox\">\n<\/div><\/div><\/div><\/div>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFComo-funciona-lanadelumab\"><\/span>How does lanadelumab work?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Lanadelumab is completely human <span class=\"term\" data-term-id=\"427\">immunoglobulin<\/span> G1, kappa light chain monoclonal antibody that provides <span class=\"term\" data-term-id=\"1021\">inhibition<\/span> of <span class=\"term\" data-term-id=\"1513\">plasma<\/span> kallikrein, a critical regulator of hereditary angioedema attacks.<\/p>\n<ul>\n<li>The activity of plasma kallikrein is regulated by the activity of C1-INH.<\/li>\n<li>C1-INH deficiency or dysfunction leads to uncontrolled increases in plasma kallikrein activity.<\/li>\n<li>Plasma kallikrein is a <span class=\"term\" data-term-id=\"689\">protease<\/span> that works by dividing the height <span class=\"term\" data-term-id=\"1851\">molecular<\/span> kininogen weight (a <span class=\"term\" data-term-id=\"604\">coagulation<\/span> protein) to generate the<span class=\"term\" data-term-id=\"490\">inflammatory<\/span> <span class=\"term\" data-term-id=\"1732\">peptide<\/span> bradykinin Bradykinin is a potent vasodilator and increases <span class=\"term\" data-term-id=\"753\">vascular<\/span> <span class=\"term\" data-term-id=\"1444\">permeability<\/span>, which is the cause of the swelling and pain associated with hereditary angioedema.<\/li>\n<li>Lanadelumab clogs the <span class=\"term\" data-term-id=\"1743\">proteolytic<\/span> active site of plasma kallikrein, preventing the division of kininogen into bradykinin.<\/li>\n<li>Lanadelumab therefore decreases plasma kallikrein and bradykinin activity in patients with hereditary angioedema.<\/li>\n<\/ul>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFCual-es-la-indicacion-para-lanadelumab\"><\/span>What is the indication for lanadelumab?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Lanadelumab is indicated to prevent recurrent attacks of hereditary angioedema in patients 12 years of age and older.<\/p>\n<ul>\n<li>It is not intended for the treatment of <span class=\"term\" data-term-id=\"349\">acute<\/span> attacks of hereditary angioedema.<\/li>\n<li>Treatment should be started under the supervision of a physician experienced in treating patients with hereditary angioedema.<\/li>\n<\/ul>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFComo-se-administra-lanadelumab\"><\/span>How is lanadelumab administered?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Lanadelumab can be self-administered by the patient or administered by a caregiver after receiving training on <span class=\"term\" data-term-id=\"900\">subcutaneous<\/span> injection technique by a healthcare professional.<\/p>\n<ul>\n<li>The recommended starting dose is 300 mg lanadelumab by subcutaneous injection every 2 weeks.<\/li>\n<li>Injection should be restricted to the recommended injection sites: the abdomen, thighs, and upper outer arms, with the injection site rotating as necessary.<\/li>\n<li>In patients who do not have attacks of swelling during treatment, the dose can be reduced to 300 mg every 4 weeks.<\/li>\n<li>If a dose of lanadelumab is missed, the missed dose should be administered as soon as possible, ensuring at least 10 days between doses.<\/li>\n<\/ul>\n<div id=\"square-placement-country-holder\" class=\"country-dependent advert\">\n<div class=\"box-placement In the event of a severe hypersensitivity reaction, administration of lanadelumab should be stopped immediately and appropriate treatment initiated.<\/p>\n<h3><span class=\"ez-toc-section\" id=\"General\"><\/span>general<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>Lanadelumab is not intended for the treatment of acute attacks of hereditary angioedema. Hepatic impairment is not expected to affect exposure to lanadelumab. Monoclonal <span class=\"term\" data-term-id=\"426\">antibodies<\/span> like lanadelumab it can cross the placenta during the third trimester of pregnancy with potentially harmful effects on the fetus. Studies of lanadelumab in pregnant women <span class=\"term\" data-term-id=\"1746\">cynomolgus monkeys<\/span>, treated with 33 times the maximum recommended dose for humans, revealed no evidence of harm to the developing fetus.<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Madres-lactantes\"><\/span>Nursing mothers<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>It is not known whether lanadelumab or its <span class=\"term\" data-term-id=\"717\">metabolites<\/span> are excreted in human milk. Because many drugs are excreted in human milk, mothers should stop breastfeeding before using lanadelumab. Based on their characteristics, pharmacokinetic interactions with co-administered drugs are not expected.<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Interaccion-con-el-ensayo-aPPT\"><\/span>Interaction with the aPPT assay<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<ul>\n<li>Lanadelumab may increase activated partial thromboplastin time (aPTT) due to an interaction with the aPTT assay.<\/li>\n<li>\n<span class=\"term\" data-term-id=\"477\">Reagents<\/span> used in the laboratory test aPTT initiates intrinsic coagulation through the activation of plasma kallikrein in the contact activation system.<\/li>\n<li>Inhibition of plasma kallikrein by lanadelumab may increase aPTT in this assay.<\/li>\n<\/ul>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFCuales-son-los-posibles-efectos-secundarios-de-lanadelumab\"><\/span>What are the possible side effects of lanadelumab?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>The most common side effects in patients receiving lanadelumab in clinical trials are described below.<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Efectos-secundarios-con-mas-de-un-10-incidencia\"><\/span>Side effects with more than one 10% <span class=\"term\" data-term-id=\"578\">incidence<\/span><br \/>\n<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>At a dose of lanadelumab 300 mg every 2 weeks, the following <span class=\"term\" data-term-id=\"1333\">Adverse reactions<\/span> where seen:<\/p>\n<ul>\n<li>Injection site reactions, including pain <span class=\"term\" data-term-id=\"90\">erythema<\/span>and bruising (56%)<\/li>\n<li>Upper respiratory tract <span class=\"term\" data-term-id=\"145\">infection<\/span> (44%)<\/li>\n<li>Headache (33%)<\/li>\n<li>\n<span class=\"term\" data-term-id=\"939\">Myalgia<\/span> (11%).<\/li>\n<\/ul>\n<p>At a dose of lanadelumab 300 mg every 4 weeks, the following adverse reactions were observed:<\/p>\n<ul>\n<li>Injection site reactions (45%)<\/li>\n<li>Upper respiratory tract infection (31%)<\/li>\n<li>Headache (21%).<\/li>\n<\/ul>\n<h3><span class=\"ez-toc-section\" id=\"Efectos-secundarios-de-1-a-10-de-incidencia\"><\/span>Side effects from 1 to 10% incidence<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>Adverse reactions of incidence 1 to 10% observed with lanadelumab include:<\/p>\n<ul>\n<li>Alanine transferase increased (<span class=\"term\" data-term-id=\"991\">ALT<\/span>) and aspartate transferase (<span class=\"term\" data-term-id=\"988\">AST<\/span>) liver <span class=\"term\" data-term-id=\"1526\">enzymes<\/span> (2%)<\/li>\n<li>\n<p>Hypersensitivity (1%): symptoms of a serious illness. <span class=\"term\" data-term-id=\"1335\">allergic reaction<\/span> include <span class=\"term\" data-term-id=\"1646\">urticaria<\/span>, itching \/ swelling (especially of the face, tongue and throat), severe dizziness, and trouble breathing.<\/li>\n<\/ul>\n<p>At a dose of 300 mg lanadelumab every 2 weeks, the following adverse reactions were observed:<\/p>\n<ul>\n<li>\n<span class=\"term\" data-term-id=\"255\">Eruption<\/span> (4%)<\/li>\n<li>Dizziness (4%)<\/li>\n<li>Diarrhea (4%).<\/li>\n<\/ul>\n<p>At a dose of lanadelumab 300 mg every 4 weeks, the following adverse reactions were observed:<\/p>\n<ul>\n<li>Rash (10%)<\/li>\n<li>Dizziness (10%).<\/li>\n<\/ul>\n<h2><span class=\"ez-toc-section\" id=\"Inmunogenicidad\"><\/span>Immunogenicity<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Lanadelumab treatment has been associated with the development of anti-drug antibodies in 10 of 84 patients (11.9%). All antibody titers were low. The anti-drug antibody response was transient in the 20% of the subjects positive for anti-drug antibodies; two of 84 patients treated with lanadelumab (2.4%) tested positive for neutralizing antibodies.<\/p>\n<p>The development of anti-drug antibodies, including neutralizing antibodies against lanadelumab, did not appear to adversely affect the pharmacokinetics and <span class=\"term\" data-term-id=\"1735\">pharmacodynamics<\/span> profiles or clinical response.<\/p>\n<section class=\"textBlock\">\n<div class=\"grey-box\"><em><span style=\"font-size: 0.9em\">New Zealand approved data sheets are the official source of information for these prescription drugs, including approved uses and risk information.
