The main clinical types correlate with a specific histopathological pattern.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"Histologia-de-lepra\"><\/span>\n<span class=\"term\" data-term-id=\"128\">Histology<\/span> of leprosy<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Lepromatous leprosy: below normal <span class=\"term\" data-term-id=\"85\">epidermis<\/span> and <span class=\"term\" data-term-id=\"114\">grenz zone<\/span>, there are sheets or groups of <span class=\"term\" data-term-id=\"457\">macrophages<\/span> (Figure 1). Macrophages are abundant pink to pale in color. <span class=\"term\" data-term-id=\"838\">cytoplasm<\/span> which corresponds to numerous intracellular (parasitized) <span class=\"term\" data-term-id=\"883\">organisms<\/span> (Figure 2). The Associated <span class=\"term\" data-term-id=\"541\">infiltrate<\/span> is generally scarce and mainly <span class=\"term\" data-term-id=\"422\">lymphocytic<\/span>. In older lesions, macrophages may have bubbling cytoplasm (Figure 2). Unlike tuberculoid leprosy, there are no well-formed <span class=\"term\" data-term-id=\"1718\">granulomas<\/span> or <span class=\"term\" data-term-id=\"543\">infiltration<\/span> of <span class=\"term\" data-term-id=\"952\">nerves<\/span>.<\/p>\n<p>Tuberculoid leprosy: there is a tuberculoid or sarcoid <span class=\"term\" data-term-id=\"1076\">granulomatous<\/span> reaction throughout <span class=\"term\" data-term-id=\"61\">dermis<\/span> consisting of some Langhans <span class=\"term\" data-term-id=\"739\">giant cells<\/span>, macrophages in tight groups, and few companions <span class=\"term\" data-term-id=\"421\">lymphocytes<\/span> (figure 3). The infiltrate can infiltrate widely <span class=\"term\" data-term-id=\"1374\">peripheral<\/span> nerves (figure 4, arrow), vessels, overlying epidermis and arrectores pilorum.<\/p>\n<p>Indeterminate leprosy: the infiltrate is strongly lymphocytic with a smaller number of <span class=\"term\" data-term-id=\"1667\">epithelioid<\/span> <span class=\"term\" data-term-id=\"858\">histiocytes<\/span> in clusters The infiltrate often involves peripheral nerves. The organisms are usually very difficult to find.<\/p>\n<p><span class=\"term\" data-term-id=\"90\">Erythema<\/span> leprous nodosum: shows a dense infiltrate that extends towards the <span class=\"term\" data-term-id=\"1282\">subcutis<\/span>, particularly around the vasculature. Lepromatous leprosy lesions will show numerous organisms that are easy to identify. Cases of tuberculoid, indeterminate, and erythema nodosum leprosum can be difficult as the organisms may be rare. <span class=\"term\" data-term-id=\"941\">Polymerase chain reaction<\/span> (<span class=\"term\" data-term-id=\"942\">PCR<\/span>) studies may be useful in confirming the diagnosis of leprosy.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"Diagnostico-diferencial-de-lepra-patologia\"><\/span>\n<span class=\"term\" data-term-id=\"813\">Differential diagnosis<\/span> of leprosy <span class=\"term\" data-term-id=\"223\">pathology<\/span><br \/>\n<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Granulomatous disorders: Tuberculoid leprosy may resemble sarcoidosis or other granulomatous disorders. Special studies and clinical correlation can help in difficult cases.<\/p>\n<p><span class=\"term\" data-term-id=\"859\">Histiocytic<\/span> reactions: the foamy histiocytes, laden with organisms in lepromatous leprosy, can mimic the histiocytic reactions seen with <span class=\"term\" data-term-id=\"370\">statement<\/span> aluminum chloride (a common antiperspirant and hemostatic agent) or <span class=\"term\" data-term-id=\"1345\">orthopedic<\/span> hardware.<\/p>\n<p>Tuberculosis and other mycobacterial infections: Other mycobacterial infections often cause tissue <span class=\"term\" data-term-id=\"403\">necrosis<\/span> and are associated with suppuration. <em>M. avium<\/em> complex can closely mimic lepromatous leprosy. Special studies can help in the differential.<\/p>\n<p><span class=\"term\" data-term-id=\"976\">Lymphoma<\/span> &#8211; <span class=\"term\" data-term-id=\"631\">T cell<\/span> Lymphomas may be neurotropic and associated with scattered histiocytes, producing a pattern similar to indeterminate leprosy.
