It happens <span class=\"term\" data-term-id=\"1351\">adjacent<\/span> to an amalgam of dental filling.<\/p>\n<div id=\"square-placement-country-holder\" class=\"country-dependent advert\">\n<div class=\"box-placement square-placement\">\n<div class=\"advert__frame\">\n<div id=\"dermnet-dermnet-mobbox\">\n<\/div><\/div><\/div><\/div>\n<h2><span class=\"ez-toc-section\" id=\"Caracteristicas-clinicas\"><\/span>Clinical features<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Clinically and <span class=\"term\" data-term-id=\"1669\">histologically<\/span>, is indistinguishable from <span class=\"term\" data-term-id=\"862\">idiopathic<\/span> oral lichen planus.<\/p>\n<p>The clinical clues that may suggest this diagnosis are:<\/p>\n<ol>\n<li>very close to a dental amalgam filling<\/li>\n<li>\n<span class=\"term\" data-term-id=\"324\">located<\/span> <span class=\"term\" data-term-id=\"164\">injury<\/span>\n<\/li>\n<li>\n<span class=\"term\" data-term-id=\"16\">asymmetric<\/span>.<\/li>\n<\/ol>\n<p>The buccal <span class=\"term\" data-term-id=\"189\">mucous membrane<\/span> (inside the cheek) is the most common affected site, with the edge of the tongue being the next most common.<\/p>\n<p>Oral lichenoid lesions have been classified into three groups according to the relationship with the amalgam filling:<\/p>\n<ul>\n<li>Group I: the lesion is limited to the area in direct contact with the amalgam filling<\/li>\n<li>Group II: lesion exceeds contact area<\/li>\n<li>Group III: the lesion does not show contact with an amalgam filling.<\/li>\n<\/ul>\n<p>As with idiopathic oral lichen planus, there are four clinical patterns: <span class=\"term\" data-term-id=\"1400\">lattice<\/span>, <span class=\"term\" data-term-id=\"537\">erosive<\/span>, <span class=\"term\" data-term-id=\"809\">atrophic<\/span> and <span class=\"term\" data-term-id=\"235\">license plate<\/span>-like, with the most commonly seen reticular pattern.<\/p>\n<p>The injury can be <span class=\"term\" data-term-id=\"731\">asymptomatic<\/span> or show symptoms such as pain or tenderness when eating hot or spicy foods.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"Como-se-hace-el-diagnostico\"><\/span>How do you make the diagnostic<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>The diagnosis will be suspected clinically due to the close relationship between the lesion and a dental amalgam filling. However, idiopathic lichen planus can develop in sites of <span class=\"term\" data-term-id=\"1365\">trauma<\/span> like the rubbing of rough dental restorations. Signs of lichen planus elsewhere, including in the mouth, skin, <span class=\"term\" data-term-id=\"193\">nail<\/span> and the scalp should be sought.<\/p>\n<p>A <span class=\"term\" data-term-id=\"24\">biopsy<\/span> of the injury will show the same <span class=\"term\" data-term-id=\"474\">histological<\/span> features such as idiopathic oral lichen planus. <span class=\"term\" data-term-id=\"142\">Immunofluorescence<\/span> can be done to exclude <span class=\"term\" data-term-id=\"691\">autoimmune<\/span> diseases of the mouth, and again it is the same as for idiopathic oral lichen planus.<\/p>\n<p><span class=\"term\" data-term-id=\"224\">Patch<\/span> Tests with amalgam and inorganic mercury salts will be positive more frequently in patients with a lichenoid amalgam reaction than in those with idiopathic oral lichen planus. However, a negative result does not exclude the diagnosis. Therefore, the role of this test in making the diagnosis is unclear.<\/p>\n<p>The only diagnostic test is the response to replacing the amalgam filler with an alternative filler material. However, the effect of removing the amalgam cannot be predicted by biopsy. <span class=\"term\" data-term-id=\"128\">histology<\/span> or patch test results.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"Tratamiento\"><\/span>Treatment<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>If the injury is symptomatic, it is recommended to replace the amalgam with an alternative filler material. In European studies, more than 90% of patients showed marked improvement or disappearance of the lesion after replacement of the padding, especially for Group I and tongue lesions. Complete removal was more likely to occur in Group I patients with a positive reaction to the amalgam patch test.<\/p>\n<p>However, if the injury is not causing symptoms or if the patient has lichen planus of the skin, an annual checkup and the use of alternative fillings for future dental work is suggested.<\/p>\n<p>In addition, the following treatments may be helpful:<\/p>\n<ol>\n<li>powerful <span class=\"term\" data-term-id=\"1330\">current<\/span> steroids like clobetasol propionate<\/li>\n<li>other topical treatments like tacrolimus\n<\/li>\n<li>oral medications including oral steroids such as predniso (lo) ne.<\/li>\n<\/ol>\n<p>No progression to oral <span class=\"term\" data-term-id=\"30\">Cancer<\/span> An oral lichenoid lesion adjacent to the amalgam has been reported to date.<\/p>\n<div id=\"square-placement-country-holder\" class=\"country-dependent advert\">\n<div class=\"box-placement square-placement\">\n<div class=\"advert__frame\">\n<div id=\"dermnet-dermnet-mobbox\">\n<\/div><\/div><\/div><\/div>\n<h2><span class=\"ez-toc-section\" id=\"Mecanismo-propuesto\"><\/span>Proposed Mechanism<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Lichenoid amalgam reaction appears to be delayed (type IV) cell-mediated contact <span class=\"term\" data-term-id=\"943\">hypersensitivity<\/span> either mercury or another component of amalgam.<\/p>\n<\/section>","protected":false},"excerpt":{"rendered":"<p>Que es un liquenoide reacci\u00f3n de amalgama?
