It is characterized by <span class=\"term\" data-term-id=\"421\">lymphocytes<\/span> around <span class=\"term\" data-term-id=\"953\">blood vessels<\/span>That is, they are angiocentric.<\/p>\n<p>According to the definition of the World Health Organization, lymphomatoid granulomatosis is defined as angiocentric and angiodestructive. <span class=\"term\" data-term-id=\"1402\">lymphoproliferative<\/span> disease involving <span class=\"term\" data-term-id=\"1790\">extranodal<\/span> sites, made up of <span class=\"term\" data-term-id=\"430\">B cells<\/span> positive for Epstein-Barr virus (EBV) and mixed with reagent <span class=\"term\" data-term-id=\"417\">T cells<\/span>. It also often occurs in association with an underlying immunodeficiency state. It is a form of non-Hodgkin lymphoma. See Epstein-Barr virus associated lymphoproliferative disorders.<\/p>\n<p>Lymphomatoid granulomatosis is twice as common in men as it is in women and occurs between the ages of 30 and 50. The lungs are generally involved with the skin, and the central nervous system is also commonly affected. Other organs may be involved, including the kidneys, liver, spleen, <span class=\"term\" data-term-id=\"1775\">lymph nodes<\/span>, eyes and gastrointestinal tract.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFCuales-son-los-sintomas\"><\/span>What are the symptoms?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>The most common presenting symptoms are cough, <span class=\"term\" data-term-id=\"1176\">fever<\/span> and skin lesions. Affected patients can generally feel bad, with weight loss, <span class=\"term\" data-term-id=\"402\">neurological <\/span>abnormalities, dyspnea, and chest pain.<\/p>\n<div id=\"square-placement-country-holder\" class=\"country-dependent advert\">\n<div class=\"box-placement square-placement\">\n<div class=\"advert__frame\">\n<div id=\"dermnet-dermnet-mobbox\">\n<\/div><\/div><\/div><\/div>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFCuales-son-los-hallazgos-del-examen-de-piel\"><\/span>What are the findings of the skin exam?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Skin lesions can be in the form of:<\/p>\n<ul>\n<li>Small red bumps (<span class=\"term\" data-term-id=\"366\">papules<\/span>)<\/li>\n<li>Lumps under the skin (<span class=\"term\" data-term-id=\"367\">nodules<\/span>)<\/li>\n<li><span class=\"term\" data-term-id=\"1279\">Ulcers<\/span><\/li>\n<li>Thickened patches<\/li>\n<li>Angry <span class=\"term\" data-term-id=\"122\">hair<\/span> <span class=\"term\" data-term-id=\"1301\">follicles<\/span> (folliculitis)<\/li>\n<\/ul>\n<p>Usually there are no symptoms of skin lesions, but they can be tender or itchy. The results of other laboratory tests tend to be nonspecific.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFCuales-son-las-caracteristicas-histopatologico-%C2%BFrecomendaciones\"><\/span>What are the characteristics <span class=\"term\" data-term-id=\"1419\">histopathological<\/span> recommendations?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Biopsy findings in lymphomatoid granulomatosis include a classic triad of features.<\/p>\n<ol>\n<li>Mixed <span class=\"term\" data-term-id=\"1266\">lymphoid<\/span> <span class=\"term\" data-term-id=\"541\">infiltrate<\/span>\n<\/li>\n<li>Lymphocytes infiltrated through the walls of arteries and veins<\/li>\n<li>\n<span class=\"term\" data-term-id=\"489\">Focal<\/span> areas of <span class=\"term\" data-term-id=\"403\">necrosis<\/span> (cell death).<\/li>\n<\/ol>\n<p>Special stains (<span class=\"term\" data-term-id=\"1421\">immunohistochemistry<\/span>) show a large number of reactive T cells with a variable number of <span class=\"term\" data-term-id=\"176\">evil one<\/span> B cells <span class=\"term\" data-term-id=\"474\">histological<\/span> the rating is 1-3 (from lowest to highest). Grade 1 only has a small number of EBV-positive B cells. Grade 2 has an increasing number of large EBV-positive B cells. Grade 3 is more like conventional forms of <span class=\"term\" data-term-id=\"1769\">diffuse<\/span> large B-cell lymphoma.<\/p>\n<div id=\"square-placement-country-holder\" class=\"country-dependent advert\">\n<div class=\"box-placement square-placement\">\n<div class=\"advert__frame\">\n<div id=\"dermnet-dermnet-mobbox\">\n<\/div><\/div><\/div><\/div>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFCuales-son-los-estados-de-inmunodeficiencia-asociados\"><\/span>What are the associated immunodeficiency states?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Lymphomatoid granulomatosis occurs against the background of a malfunctioning immune system, for example:<\/p>\n<ul>\n<li>Organ <span class=\"term\" data-term-id=\"1793\">transplant<\/span>\n<\/li>\n<li>Wiskott-Aldrich <span class=\"term\" data-term-id=\"350\">syndrome<\/span><\/li>\n<li>Human immunodeficiency virus (<span class=\"term\" data-term-id=\"127\">HIV<\/span>) <span class=\"term\" data-term-id=\"145\">infection<\/span><\/li>\n<li>X-linked lymphoproliferative syndrome<\/li>\n<li>Common variable immunodeficiency syndrome<\/li>\n<li>Other reduced immune surveillance disorders.<\/li>\n<\/ul>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFCuales-son-las-opciones-de-tratamiento\"><\/span>What are the treatment options?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>The ideal treatment for lymphomatoid granulomatosis is unknown. Treatments that have been reported as successful in at least some patients include:<\/p>\n<ul>\n<li>\n<span class=\"term\" data-term-id=\"380\">Systemic<\/span> steroids plus cyclophosphamide\n<\/li>\n<li>Interferon alfa 2b<\/li>\n<li>Combination <span class=\"term\" data-term-id=\"1619\">chemotherapy<\/span> with CHOP (cyclophosphamide, doxorubicin, vincristine and prednisone)<\/li>\n<li>Anti-CD20 <span class=\"term\" data-term-id=\"466\">monoclonal<\/span> <span class=\"term\" data-term-id=\"426\">antibodies<\/span> (rituximab)\n<\/li>\n<li>Monoclonal anti-CD20 + CHOP antibodies<\/li>\n<li>Autologous hematopoietic stem cell transplant after high doses of chemotherapy.<\/li>\n<\/ul>\n<h2><span class=\"ez-toc-section\" id=\"Cual-es-el-pronostico-con-granulomatosis-linfomatoide\"><\/span>Which is the <span class=\"term\" data-term-id=\"241\">forecast<\/span> with lymphomatoid granulomatosis?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Spontaneous recovery occurs in approximately 20% of patients with low-grade disease (grade 1). Others have <span class=\"term\" data-term-id=\"1759\">progressive<\/span> disease. The reported 5-year mortality rates have been between 38% and 88%. Median survival is between 14 and 72 months.
