Unlike other autoimmune diseases that cause blisters and ulcers in the mouth, it does not respond well to corticosteroids.<\/p>\n<p>Stomatitis means mouth pain.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFQuien-contrae-estomatitis-ulcerosa-cronica\"><\/span>Who gets chronic ulcerative stomatitis?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Chronic ulcerative stomatitis is a rare disease with less than 100 cases reported so far. It appears to almost exclusively affect white women in late middle age with an average age of onset of 60 years. There are rare reports of chronic ulcerative stomatitis affecting younger men or women. Often, symptoms have been present for many years before the diagnosis is made. It is associated with <span class=\"term\" data-term-id=\"426\">antibodies<\/span> against <span class=\"term\" data-term-id=\"917\">keratinocytes<\/span> <span class=\"term\" data-term-id=\"729\">nuclei<\/span> stratified <span class=\"term\" data-term-id=\"270\">scaly<\/span> <span class=\"term\" data-term-id=\"87\">epithelium<\/span>. I specify it <span class=\"term\" data-term-id=\"410\">antigen<\/span> has been identified as DeltaNp63alpha.<\/p>\n<div id=\"square-placement-country-holder\" class=\"country-dependent advert\">\n<div class=\"box-placement square-placement\">\n<div class=\"advert__frame\">\n<div id=\"dermnet-dermnet-mobbox\">\n<\/div><\/div><\/div><\/div>\n<h2><span class=\"ez-toc-section\" id=\"Caracteristicas-clinicas-de-la-estomatitis-ulcerosa-cronica\"><\/span>Clinical characteristics of chronic ulcerative stomatitis.<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Chronic ulcerative stomatitis presents as painful <span class=\"term\" data-term-id=\"536\">erosions<\/span> (shallow ulcers) in the mouth that may look like <span class=\"term\" data-term-id=\"537\">erosive<\/span> lichen planus May mimic other autoimmune conditions that cause oral ulcers or erosions, including pemphigus vulgaris and <span class=\"term\" data-term-id=\"36\">scar<\/span> pemphigoid, or resembles desquamative gingivitis. Ulcers can occur on the gums, inside the cheeks, or on the tongue. The condition lasts for many years, sometimes coming and going. Very rarely, similar lesions on the genitals or conjunctivals have also been reported. <span class=\"term\" data-term-id=\"189\">mucous membrane<\/span>. Skin lesions resembling lichen planus have been reported in some patients. It is still unclear whether chronic ulcerative stomatitis coexists with lichen planus or is a variant of it. The patient is otherwise fine.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFComo-se-diagnostica-la-estomatitis-ulcerosa-cronica\"><\/span>How is chronic ulcerative stomatitis diagnosed?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Chronic ulcerative stomatitis is diagnosed by combining clinical and pathological features.<\/p>\n<p><span class=\"term\" data-term-id=\"1018\">Mucous membrane<\/span> <span class=\"term\" data-term-id=\"1111\">biopsies<\/span> of the mouth show nonspecific pathological features, most often resembling lichen planus. However direct <span class=\"term\" data-term-id=\"142\">immunofluorescence<\/span> (DIF) shows a speckled pattern of IgG in the lower third of the epithelium (<span class=\"term\" data-term-id=\"831\">basal<\/span> and parabasal cell layers).<\/p>\n<p>Indirect immunofluorescence (IIF) in a blood sample detects an IgG and\/or IgA <span class=\"term\" data-term-id=\"351\">autoantibody<\/span> against keratinocyte nuclei in skin and guinea pig <span class=\"term\" data-term-id=\"1549\">esophageal<\/span> mucosa but not in HEp2 cells or kidney. This specific <span class=\"term\" data-term-id=\"425\">antibody<\/span> It is called stratified epithelium-specific (SES) antinuclear antibody (ANA). The antigen recognized by these <span class=\"term\" data-term-id=\"352\">autoantibodies<\/span> It&#039;s a 70kDa <span class=\"term\" data-term-id=\"847\">epithelial<\/span> <span class=\"term\" data-term-id=\"1612\">nuclear<\/span> protein and has been identified as DeltaNp63alpha. An ELISA assay has recently been developed to detect specific autoantibodies. The SES-ANA titer is usually high and persists despite the clinical symptoms. <span class=\"term\" data-term-id=\"1260\">remission<\/span> with treatment Although the titer is often lower with treatment, it does not disappear completely. The titer does not appear to correlate with disease activity. However, the combination of IgG and IgA autoantibodies can indicate serious disease.<\/p>\n<p>The diagnostic criteria have been proposed by Chorzelski et al (1998):<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Criterios-principales\"><\/span>Main criteria<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<ul>\n<li>erosive or exfoliative lesions in the mouth<\/li>\n<li>characteristic indirect and direct immunofluorescence (IIF and DIF)<\/li>\n<\/ul>\n<h3><span class=\"ez-toc-section\" id=\"Criterios-menores\"><\/span>Minor criteria<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<ul>\n<li>chronic course with <span class=\"term\" data-term-id=\"1501\">relapses<\/span>\n<\/li>\n<li>woman in the older age group<\/li>\n<li>response to hydroxychloroquine alone or combined with small doses of cortisone.<\/li>\n<\/ul>\n<h2><span class=\"ez-toc-section\" id=\"Tratamiento-de-estomatitis-ulcerosa-cronica\"><\/span>Treatment of chronic ulcerative stomatitis<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Chronic ulcerative stomatitis usually does not respond to <span class=\"term\" data-term-id=\"1330\">current<\/span> steroids or oral steroid treatment. However, there is usually an excellent initial response to oral hydroxychloroquine, 200-400 mg\/day. This can result in long-lasting remission. However, in the long term, hydroxychloroquine may not prevent relapses and low doses <span class=\"term\" data-term-id=\"380\">systemic<\/span> Additionally, corticosteroids and\/or dapsone may be required.<\/p>\n<\/section>","protected":false},"excerpt":{"rendered":"<p>Que es cr\u00f3nico ulcerativo estomatitis?
