class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Crusted scabies, or Norwegian scabies as it was formally known, is a highly contagious hyperinfestation with <em>Sarcoptes scabiei var hominis<\/em>. It is a presentation of scabies in <span class=\"term\" data-term-id=\"1696\">immunocompromised<\/span> patients It can also result from a misdiagnosis such as psoriasis or <span class=\"term\" data-term-id=\"378\">seborrheic<\/span> <span class=\"term\" data-term-id=\"53\">dermatitis<\/span> and treatment of these disorders.<\/p>\n<p>People with crusted scabies are estimated to have up to 4,000 <span class=\"term\" data-term-id=\"1312\">Mites<\/span> per gram of skin. Patients are often infected with more than 1 million mites. Compare this to most scabies patients, who are infected with around 10-20 mites.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFA-quien-le-da-sarna-costrosa\"><\/span>Who gets scabby scabies?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Scabies is transmitted from another individual who is infested by scabies mites. Crusted scabies can arise in people who do not achieve a normal immune response to mites.  <\/p>\n<p>Worldwide, crusted scabies is more <span class=\"term\" data-term-id=\"1676\">predominant<\/span> in remote Aboriginal communities in northern Australia.<\/p>\n<p>Risk factors for crusted scabies include:<\/p>\n<ul>\n<li>Increase in age<\/li>\n<li>Dementia<\/li>\n<li>Down <span class=\"term\" data-term-id=\"350\">syndrome<\/span><\/li>\n<li>Human immunodeficiency virus <span class=\"term\" data-term-id=\"145\">infection<\/span> (<span class=\"term\" data-term-id=\"127\">HIV<\/span>)<\/li>\n<li>Leprosy<\/li>\n<li><span class=\"term\" data-term-id=\"976\">Lymphoma<\/span><\/li>\n<li><span class=\"term\" data-term-id=\"380\">Systemic<\/span> lupus erythematosus (SLE)<\/li>\n<li>Long-term use of corticosteroids or immunosuppressants<\/li>\n<li>Institutional housing \u2013 prisons and nursing homes (institutional scabies)<\/li>\n<\/ul>\n<div id=\"square-placement-country-holder\" class=\"country-dependent advert\">\n<div class=\"box-placement square-placement\">\n<div class=\"advert__frame\">\n<div id=\"dermnet-dermnet-mobbox\">\n<\/div><\/div><\/div><\/div>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFQue-causa-la-sarna-con-costra\"><\/span>What causes crusted scabies?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>The exact reason why crusted scabies arises is unknown. It is associated with elevation <span class=\"term\" data-term-id=\"448\">eosinophils<\/span>, IgE and IgG. the <span class=\"term\" data-term-id=\"61\">dermis<\/span> is infiltrated with <span class=\"term\" data-term-id=\"421\">lymphocytes<\/span> and eosinophils. There is a higher proportion of CD8 + lymphocytes compared to normal.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFCuales-son-las-caracteristicas-clinicas-de-la-sarna-con-costra\"><\/span>What are the clinical characteristics of crusted scabies? <span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Crusted scabies begins as ill-defined red spots that later become thick <span class=\"term\" data-term-id=\"385\">scaly<\/span> <span class=\"term\" data-term-id=\"368\">plates<\/span> between the fingers, under the <span class=\"term\" data-term-id=\"193\">nail<\/span>, or diffusely on palms and soles. Other common areas include elbows and knees. This may be due to the immune compromised nature of the individual. The following may contribute to reducing scratching:<\/p>\n<ul>\n<li>Physical limitation<\/li>\n<li>Neuropsychiatric disorders.<\/li>\n<li>Osteoarticular deformities<\/li>\n<li>Muscular <span class=\"term\" data-term-id=\"18\">atrophy<\/span>\n<\/li>\n<li>waste of <span class=\"term\" data-term-id=\"47\">cutaneous<\/span> sensation<\/li>\n<\/ul>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFCuales-son-las-complicaciones-de-la-sarna-con-costra\"><\/span>What are the complications of crusted scabies?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Complications of crusted scabies include:<\/p>\n<ul>\n<li>\n<span class=\"term\" data-term-id=\"147\">Infestation<\/span> from numerous contacts, including family and staff members<\/li>\n<li>Secondary <span class=\"term\" data-term-id=\"937\">bacterial<\/span> infection\n<\/li>\n<\/ul>\n<div id=\"square-placement-country-holder\" class=\"country-dependent advert\">\n<div class=\"box-placement square-placement\">\n<div class=\"advert__frame\">\n<div id=\"dermnet-dermnet-mobbox\">\n<\/div><\/div><\/div><\/div>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFComo-se-diagnostica-la-sarna-con-costra\"><\/span>How is crusted scabies diagnosed?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Scabies is easily diagnosed clinically and confirmed by identification of mites or eggs dermoscopically or <span class=\"term\" data-term-id=\"868\">microscopic<\/span> burrow examination. Confocal reflectance <span class=\"term\" data-term-id=\"867\">microscopy<\/span> It has also been used to examine burrows, mites, eggs and <span class=\"term\" data-term-id=\"1311\">Pinch<\/span> faeces. Fur <span class=\"term\" data-term-id=\"24\">biopsy<\/span> is rarely necessary and will show characteristic scabies <span class=\"term\" data-term-id=\"223\">pathology<\/span>.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFCual-es-el-tratamiento-para-la-sarna-con-costra\"><\/span>What is the treatment for crusted scabies?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>A <span class=\"term\" data-term-id=\"54\">dermatologist<\/span> or an infectious disease physician must be involved in the care of the patient and their contacts. Treatment of crusted scabies should be done in a hospital or residential setting, with the patient in a private room. Healthcare personnel should take contact precautions and wear full personal protective equipment, including shoes and <span class=\"term\" data-term-id=\"122\">hair<\/span> covers.<\/p>\n<p><span style=\"line-height: 1.4;\">Treatment consists of oral <\/span><span style=\"line-height: 1.4;\">ivermectin<\/span><span style=\"line-height: 1.4;\">  and <span class=\"term\" data-term-id=\"1330\">current<\/span> <\/span><span style=\"line-height: 1.4;\">insecticides<\/span><span style=\"line-height: 1.4;\">. The following classification system designed at the Royal Darwin Hospital can be used to determine management.<\/span> <\/p>\n<p>A: <span class=\"term\" data-term-id=\"69\">Distribution<\/span><\/p>\n<ol>\n<li>Wrists, web spaces, feet or <10% del \u00e1rea total de superficie corporal (TBSA)<\/li>\n<li>Above forearms, legs, buttocks, trunk (TBSA 10\u201330%)   <\/li>\n<li>Above the scalp more or &gt; 30% TBSA<\/li>\n<\/ol>\n<p>YEAH. <span class=\"term\" data-term-id=\"966\">Crust<\/span>\/shedding<\/p>\n<ol>\n<li>Light bark (<5 mm de profundidad <span class =\"term\" data-term-id=\"45\">Cortex<\/span>), minimal skin shedding<\/li>\n<li>Moderate crusts (5\u201310 mm), moderate peeling<\/li>\n<li>Severe (&gt; 10 mm), profuse skin shedding<\/li>\n<\/ol>\n<p>C. Past episodes<\/p>\n<ol>\n<li>I never had it before<\/li>\n<li>1\u20133 previous hospitalizations for crusted or scabies <span class=\"term\" data-term-id=\"742\">depigmentation<\/span> elbows, knees<\/li>\n<li> \u2265 4 prior hospitalizations for crusted scabies or depigmentation as above plus legs\/back or residual skin thickening\/<span style=\"text-decoration: underline;\"><span class=\"term\" data-term-id=\"398\">ichthyosis<\/span><\/span>\n<\/li>\n<\/ol>\n<p>D. skin condition<\/p>\n<ol>\n<li>No cracks or <span class=\"term\" data-term-id=\"1626\">pyoderma<\/span>\/impetigo\n<\/li>\n<li>Multiple <span class=\"term\" data-term-id=\"1016\">pustules<\/span> and\/or superficial sores and cracks<\/li>\n<li>Deep cracks in the skin with bleeding, <span class=\"term\" data-term-id=\"309\">extended<\/span> <span class=\"term\" data-term-id=\"1182\">purulent<\/span> <span class=\"term\" data-term-id=\"488\">exudates<\/span>\n<\/li>\n<\/ol>\n<p>Grade 1: total score 4\u20136<br \/>Grade 2: total score 7\u20139<br \/>Grade 3: total score 10\u201312<\/p>\n<p>The dose of ivermectin is 200 \u03bcg\/kg for adults and children over 14 kg, rounded to the nearest 3 mg and taken after a high-fat meal to aid absorption.<\/p>\n<ul>\n<li>Grade 1: 3 doses \u2013 days 0, 1.7<\/li>\n<li>Grade 2: 5 doses \u2013 days 0, 1, 7, 8, 14<\/li>\n<li>Grade 3: 7 doses \u2013 days 0, 1, 7, 8, 14, 21, 28<\/li>\n<\/ul>\n<p>A topical scabicide is applied every other day for the first week, and then twice a week until healed.<\/p>\n<ul>\n<li>Permethrin 5% <span class=\"term\" data-term-id=\"1121\">cream<\/span> or benzyl benzoate emulsion at 25% for patients &gt; 6 months of age<\/li>\n<li>Crotamiton 10% for babies under 6 months.<\/li>\n<li>Sometimes additional 5% tea tree oil or sulfur <span class=\"term\" data-term-id=\"1122\">ointment<\/span>  <\/li>\n<\/ul>\n<p><span class=\"term\" data-term-id=\"1572\">Keratolytic<\/span> The creams are used on scaly plaques on alternate days to the scabicide. First, soften the crusts in warm water. Then apply any of:<\/p>\n<ul>\n<li>\n<p>Salicylic acid 5% to 10% in sorbolene cream<\/li>\n<li>\n<p>Lactic acid 5% plus urea 10% in sorbolene cream.<\/li>\n<\/ul>\n<p>The treating dermatologist or infectious disease physician should carefully re-examine the patient to decide when to discontinue treatment.<\/p>\n<p>A combination regimen for the treatment of crusted scabies developed by the US Centers for Disease Control and Prevention. <span class=\"term\" data-term-id=\"1854\">Control<\/span> (CDC) recommends:<\/p>\n<ul>\n<li>5% topical permethrin or 5% topical benzoyl benzoate applied daily for seven days, then twice a week until cured<\/li>\n<\/ul>\n<p><span>AND<\/span><\/p>\n<ul>\n<li>Oral ivermectin (200 <span class=\"nowrap\">mcg\/kg\/dose)<\/span> administered on days 1, 2, 8, 9 and 15 (add days 22, 29 if infestation is severe).<\/li>\n<\/ul>\n<h2><span class=\"ez-toc-section\" id=\"Tratamiento-de-familiares\"><\/span>Treatment of family members<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Family members should also be screened for crusted scabies and treated accordingly. Regardless of the test results, all family members should be treated with a topical scabicide and should repeat the treatment in one week.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"El-entorno\"><\/span>The environment<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Mites can survive up to 72 hours away from a human <span class=\"term\" data-term-id=\"1313\">host<\/span>. Living areas should be thoroughly cleaned, as crusted plaques usually fall off. Bedding and mattresses should also be cleaned.<\/p>\n<ul>\n<li>Washing with water (with or without detergent) removes most mites. Water temperature has no effect on the number of dead mites.<\/li>\n<li>If items cannot be washed, they should be kept sealed in plastic bags for at least 4 days. <\/li>\n<li>Sunlight is also effective in killing mites.<\/li>\n<\/ul>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFCual-es-el-resultado-de-la-sarna-con-costra\"><\/span>What is the outcome of crusted scabies?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Treatment will cure crusted scabies. However, patients with crusted scabies, due to their immune status and living conditions, are at risk of reinfestation. Education and regular monitoring are essential.<\/p>\n<p>Mortality is significantly higher in elderly patients with crusted scabies compared to patients with regular scabies. Disease-related mortality is attributed to <span class=\"term\" data-term-id=\"263\"><span class=\"term\" data-term-id=\"1165\">septicemia<\/span><\/span>.<\/p>\n<\/section>","protected":false},"excerpt":{"rendered":"<p>What is crusted scabies?
