associated with cryopyrin <span class=\"term\" data-term-id=\"350\">syndrome<\/span> or syndromes (CAPS), also known as cryopyrinopathies, are <span class=\"term\" data-term-id=\"558\">genetic<\/span> <span class=\"term\" data-term-id=\"1251\">autoinflammatory<\/span> syndromes defined by \u201cgain of function\u201d <span class=\"term\" data-term-id=\"871\">mutations<\/span> affecting the protein cryopyrin.<\/p>\n<p>Three distinct clinical syndromes are recognized within CAPS.<\/p>\n<ul>\n<li>\n<span class=\"term\" data-term-id=\"849\">Family<\/span> Cold autoinflammatory syndrome (FCAS)<\/li>\n<li>\n<p>Muckle-Wells syndrome (MWS)<\/li>\n<li><span class=\"term\" data-term-id=\"1195\">Neonatal<\/span>multi-system boot <span class=\"term\" data-term-id=\"490\">inflammatory<\/span> disease\/<span class=\"term\" data-term-id=\"319\">chronic<\/span> child neurological <span class=\"term\" data-term-id=\"47\">cutaneous<\/span> and joint syndrome (NOMID \/ CINCA)<\/li>\n<\/ul>\n<p>The mildest form is FCAS, and the most severe form, NOMID\/CINCA, is often fatal. Clinical overlap is now recognized, suggesting that these are a disease continuum.<\/p>\n<p>Some cases of Schnitzler syndrome may also fall into this category.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFQuien-contrae-sindromes-periodicos-asociados-a-la-criopirina-y-por-que\"><\/span>Who gets cryopyrin-associated periodic syndromes and why?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>The periodic syndromes associated with cryopyrin are all <span class=\"term\" data-term-id=\"828\">autosomal<\/span> dominant genetic conditions, meaning that only one copy of the abnormal <span class=\"term\" data-term-id=\"774\">gene<\/span> It is required to develop the clinical syndrome. FCAS and Muckle-Wells syndrome are usually familial, inherited from an affected parent. NOMID\/CINCA appears as spontaneous <span class=\"term\" data-term-id=\"870\">mutation<\/span>, as it causes severe disabilities and is often fatal before adulthood.<\/p>\n<p>Patients with overlapping features are increasingly reported to:<\/p>\n<ul>\n<li>between FCAS and MWD<\/li>\n<li>between MWD and mild NOMID\/CINCA<\/li>\n<li>but not between FCAS or MWS and NOMID\/CINCA serious.<\/li>\n<\/ul>\n<p>Examples include sensitivity to cold, frequent headaches, or <span class=\"term\" data-term-id=\"731\">asymptomatic<\/span> papilledema (swelling at the back of the eye) in MWD or deafness in FCAS. Even within a family, members can be classified as having different syndromes based on clinical characteristics.<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Molecular-biologia-y-genetica\"><\/span>\n<span class=\"term\" data-term-id=\"1851\">Molecular<\/span> biology and genetics<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>All three conditions are caused by mutations in the same gene, <em>NLRP3<\/em>, Located in <span class=\"term\" data-term-id=\"1271\">chromosome<\/span> 1 (1q44). Approximately 100 different mutations, mostly missense mutations in exon 3 of this gene, have been identified in patients with cryopyrin-associated periodic syndromes. Missense mutations in exons 4 and 6 have rarely been identified. Certain mutations result in a specific clinical syndrome, for example, Y570C, Y570F, F309S, F523L are only reported in severe NOMID\/CINCA. Other mutations (e.g., R260W, V198M, V262G, D303N) have been identified in more than one clinical picture, even within a family. This would suggest that there may be additional influences, genetic or environmental, that affect clinical presentation. Asymptomatic <span class=\"term\" data-term-id=\"1906\">carriers<\/span>, mainly in affected families, have also been detected.<\/p>\n<p>the <em>NLRP3<\/em> The gene is particularly expressed in white blood cells (especially <span class=\"term\" data-term-id=\"196\"><span class=\"term\" data-term-id=\"442\">neutrophils<\/span><\/span>) and chondrocytes (cartilage cells). It encodes cryopyrin, a protein involved in the formation of inflammasomes. Inflammalamomas are protein complexes found inside cells. They are important in the innate immune system. Disease-related changes in cryopyrin result in the loss of a regulatory step that allows for greater activation of caspase-1 and therefore greater activation of <span class=\"term\" data-term-id=\"915\">interleukin<\/span> (IL) -1. Activated IL-1\u03b2 is a powerful stimulator of the inflammatory cascade. Expression of the <em>NLRP3<\/em> gene in chondrocytes may be relevant to joint pain\/inflammation in FCAS and MWS and bone overgrowth in NOMID\/CINCA. The latter does not seem to respond to IL-1 <span class=\"term\" data-term-id=\"782\">receiver<\/span> blocking. Expression in chondrocytes may also be involved in the development of <span class=\"term\" data-term-id=\"1042\">sensorineural<\/span> deafness in MWS and NOMID\/CINCA.<\/p>\n<p>The mechanism by which cold triggers attacks in FCAS remains unknown.<\/p>\n<p>Not all clinically typical CAPS cases have had a <em>NLRP3<\/em> identified genetic mutation, particularly the NOMID\/CINCA form. <span class=\"term\" data-term-id=\"1422\">Somatic<\/span> Mutations, genetic mutations that occur during fetal development, have been found to explain a small number of apparently mutationally negative cases. the <em>NLRP3<\/em> the mutation is detected in only a percentage of cells in the <span class=\"term\" data-term-id=\"1374\">peripheral<\/span> blood. Such tests are difficult and are currently only performed in a research setting.<\/p>\n<div id=\"square-placement-country-holder\" class=\"country-dependent advert\">\n<div class=\"box-placement square-placement\">\n<div class=\"advert__frame\">\n<div id=\"dermnet-dermnet-mobbox\">\n<\/div><\/div><\/div><\/div>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFCuales-son-las-caracteristicas-clinicas-de-los-sindromes-periodicos-asociados-a-la-criopirina\"><\/span>What are the clinical characteristics of cryopyrin-associated periodic syndromes?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>The clinical features common to these three conditions are intermittent or <span class=\"term\" data-term-id=\"805\">recurrent<\/span> attacks that include:<\/p>\n<ul>\n<li><span class=\"term\" data-term-id=\"1176\">Fever<\/span><\/li>\n<li>\n<span class=\"term\" data-term-id=\"1646\">Urticaria<\/span>-I like it <span class=\"term\" data-term-id=\"255\">eruption<\/span> (see below)<\/li>\n<li>Joint pain (<span class=\"term\" data-term-id=\"601\">arthralgia<\/span>)<\/li>\n<li>\n<span class=\"term\" data-term-id=\"1898\">Conjunctivitis<\/span>.<\/li>\n<\/ul>\n<p>In addition, each syndrome has its own distinctive clinical features (see the description of a specific syndrome).<\/p>\n<h3><span class=\"ez-toc-section\" id=\"%C2%BFCuales-son-las-caracteristicas-de-la-erupcion-cutanea\"><\/span>What are the characteristics of the skin rash?<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>The urticaria-like rash is the same clinically and <span class=\"term\" data-term-id=\"1669\">histologically<\/span> in all forms of cryopyrin-associated periodic syndromes. Of trait:<\/p>\n<ul>\n<li>It&#039;s usually the first <span class=\"term\" data-term-id=\"899\">sign<\/span> of the syndrome<\/li>\n<li>It develops at or shortly after birth, or in early childhood.<\/li>\n<li>It is migratory, meaning that the skin lesions move.<\/li>\n<li>\n<span class=\"term\" data-term-id=\"938\">Maculopapular<\/span> or hives may appear.<\/li>\n<li>The symptoms tend to be <span class=\"term\" data-term-id=\"1395\">atypical<\/span> for hives, with itching, burning, warmth and\/or tightness, rather than itching.<\/li>\n<li>Lesions last less than 24 hours.<\/li>\n<li>They often show a diurnal pattern, worsening at night.<\/li>\n<li>The intensity varies between patients and between episodes.<\/li>\n<\/ul>\n<section class=\"textBlock imageLinkBlockTitle\">\n<h5 class=\"colour--primary\"><span class=\"ez-toc-section\" id=\"Erupcion-periodica-asociada-al-sindrome-de-criopirina\"><\/span>Periodic rash associated with cryopyrin syndrome<span class=\"ez-toc-section-end\"><\/span><\/h5>\n<\/section>\n<section class=\"imageLinkBlock\">\n<div class=\"flex\">\n<div class=\"imageLinkBlock__item__image\">\n<p>                        <img alt='caps-1__protectwyjqcm90zwn0il0_focusfillwzi5ncwymjisingildiwxq-8736313-7579697' src='https:\/\/doctorhoogstra.com\/wp-content\/uploads\/2020\/04\/caps-1__ProtectWyJQcm90ZWN0Il0_FocusFillWzI5NCwyMjIsIngiLDIwXQ-8736313.jpg'>\n<\/div>\n<p class=\"p-small\">Periodic rash associated with cryopyrin syndrome<\/p>\n<div class=\"imageLinkBlock__item__image\">\n<p>                        <img alt='caps-2__protectwyjqcm90zwn0il0_focusfillwzi5ncwymjisingildiwxq-5086516-3400386' src='https:\/\/doctorhoogstra.com\/wp-content\/uploads\/2020\/04\/caps-2__ProtectWyJQcm90ZWN0Il0_FocusFillWzI5NCwyMjIsIngiLDIwXQ-5086516.jpg'>\n<\/div>\n<p class=\"p-small\">Periodic rash associated with cryopyrin syndrome<\/p>\n<\/p><\/div>\n<\/section>\n<div class=\"clearfix imageLinkBlock\"><\/div>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFComo-se-diagnostican-los-sindromes-periodicos-asociados-a-la-criopirina\"><\/span>How are cryopyrin-associated periodic syndromes diagnosed?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>The diagnosis of a cryopyrin-associated periodic syndrome should be considered in patients who present with recurrent episodes of fever, rash, joint pain, and <span class=\"term\" data-term-id=\"148\">inflammation<\/span> of the eyes, without evidence of <span class=\"term\" data-term-id=\"145\">infection<\/span> or <span class=\"term\" data-term-id=\"691\">autoimmune<\/span> disease. Although these are all <span class=\"term\" data-term-id=\"829\">autosomal dominant<\/span> conditions, a positive family history cannot always be obtained.<\/p>\n<p>A delay in the diagnosis of a cryopyrin-associated periodic syndrome is common since all forms are rare. Initial diagnoses of viral infections and <span class=\"term\" data-term-id=\"1137\">allergies<\/span> are generally considered This delay in diagnosis, and therefore treatment, can lead to interleukin-1-induced organ damage, such as deafness and amyloidosis.<\/p>\n<p><span class=\"term\" data-term-id=\"349\">Acute<\/span> phase reactants in blood, such as <span class=\"term\" data-term-id=\"692\">erythrocytes<\/span> sedimentation rate (<span class=\"term\" data-term-id=\"1114\">ESR<\/span>), <span class=\"term\" data-term-id=\"946\">C-reactive protein<\/span> (<span class=\"term\" data-term-id=\"947\">CRP<\/span>) and <span class=\"term\" data-term-id=\"1430\">serum<\/span> <span class=\"term\" data-term-id=\"1340\">amyloid<\/span> A (SAA), increase markedly, even when the patient is well between episodes. <span class=\"term\" data-term-id=\"598\">Anemia<\/span> of chronic diseases and a greater number of white blood cells, especially neutrophils, can be detected.<\/p>\n<p>Skin <span class=\"term\" data-term-id=\"24\">biopsy<\/span> of the urticaria-like rash may suggest the diagnosis of a cryopyrin-associated periodic syndrome, as it shows a <span class=\"term\" data-term-id=\"228\">perivascular<\/span> and sometimes peri-<span class=\"term\" data-term-id=\"844\">eccrine<\/span> (around sweat glands) <span class=\"term\" data-term-id=\"443\">neutrophilic<\/span> <span class=\"term\" data-term-id=\"541\">infiltrate<\/span> of the <span class=\"term\" data-term-id=\"1400\">lattice<\/span> <span class=\"term\" data-term-id=\"61\">dermis<\/span>, without <span class=\"term\" data-term-id=\"582\">mast cells<\/span> or <span class=\"term\" data-term-id=\"1091\">vasculitis<\/span>. This is quite different from normal hives (hives).<\/p>\n<p>A commercial test is available to sequence exon 3 of <em>NLRP3<\/em> gene. This will detect most, but not all, known mutations associated with cryopyrin-associated periodic syndromes.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFCual-es-el-tratamiento-de-los-sindromes-periodicos-asociados-a-la-criopirina\"><\/span>What is the treatment of periodic syndromes associated with cryopyrin?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>All three clinical syndromes respond very well to therapy with interleukin-1 antagonists, such as anakinra, rilonacept, and canakinumab. All patients with CAPS respond, provided a sufficient dose is administered.<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Anakinra\"><\/span>Anakinra<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>The dramatic <span class=\"term\" data-term-id=\"914\">effectiveness<\/span> of the biologic agent anakinra, an IL-1 receptor antagonist, has been demonstrated in a series of large clinical trials involving many patients with cryopyrin-associated periodic syndromes. It is administered daily. <span class=\"term\" data-term-id=\"900\">subcutaneous<\/span> injection, and local injection reactions (itching, swelling, redness) are the most common adverse effects. Anakinra is the standard of care for NOMID\/CINCA and was approved by the FDA for this indication in 2013. The dose range varies: 0.5\u20131.5mg\/kg\/d for FCAS, up to 3.5 mg\/kg\/d for MWS and up to at 10 mg\/kg\/day for severe NOMID\/CINCA in infants.<\/p>\n<p>Note: Anakinra is not registered or subsidized in New Zealand (March 2011). In other countries, such as the US and Europe, its registered indication is rheumatoid <span class=\"term\" data-term-id=\"1578\">arthritis<\/span>.<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Rilonacept\"><\/span>Rilonacept<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>Rilonacept was the first FDA-approved therapy for CAPS. It was approved in 2008 for FCAS and MWS in adults and children aged 12 years and older. Rilonacept is a dimeric fusion protein that binds to the interleukin-1 receptor accessory protein and the IL-1 type 1 receptor. It is administered weekly as a subcutaneous injection. <span class=\"term\" data-term-id=\"909\">Adverse events<\/span> Mild to moderate severity, including local injection site reactions, upper respiratory tract infections, headache, joint pain, and diarrhea, has been reported in clinical trials. Two deaths have been reported during treatment with rilonacept for CAPS: one from pneumococcus <span class=\"term\" data-term-id=\"1053\">meningitis<\/span> and the other from coronary heart disease.<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Canakinumab\"><\/span>Canakinumab<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>Canakinumab has also been approved by the FDA (2009) for FCAS and MWS and has been approved for ages 4 years and older. In Europe, it has been approved for all forms of CAPS. Canakinumab is a completely humanized product. <span class=\"term\" data-term-id=\"466\">monoclonal<\/span> <span class=\"term\" data-term-id=\"425\">antibody<\/span> specifically directed against IL-1\u03b2. It is administered as a subcutaneous injection every 8 weeks. In a clinical trial with 35 patients with CAPS, the commonly reported side effects were <span class=\"term\" data-term-id=\"1222\">nasopharyngitis<\/span>, rhinitis, nausea, diarrhea and <span class=\"term\" data-term-id=\"974\">Vertigo<\/span>. No local injection site reactions were reported. Two serious adverse events reported during the trials were acute angle vertigo <span class=\"term\" data-term-id=\"975\">glaucoma<\/span> and recurrent lower urinary tract infection with <span class=\"term\" data-term-id=\"263\"><span class=\"term\" data-term-id=\"1165\">septicemia<\/span><\/span>.
