\nCutaneous<\/span> tuberculosis is caused by infection<\/span> for Mycobacterium tuberculosis. There have been several methods to classify the infection. Traditionally, the infection has been divided into primary<\/span> infection, where there has been no previous exposure to organism<\/span>and secondary tuberculosis, resulting from reinfection.<\/p>\n<\/span>\nHistology<\/span> of cutaneous tuberculosis<\/span><\/h2>\nPrimary inoculation<\/span> with tubercle bacilli it can start with the surface ulceration<\/span>, epidermal<\/span> hyperplasia<\/span> and a dense dermal<\/span> infiltrate<\/span> rich in neutrophils<\/span><\/span> (Figure 1). Disseminated<\/span> cutaneous tuberculosis and orificial tuberculosis will typically show a similar neutrophilic<\/span> reaction in early injuries. Over time, neutrophils are replaced by chronic<\/span> inflammatory<\/span> cells and necrotizers granulomatous<\/span> inflammation<\/span> surrounded by numerous lymphocytes<\/span> whether the patient has a competent immune system (as seen with the clinical subtype vurrucosa tuberculosis). Figures 2,3,4 show the same image of the characteristic necrotizing granulomatous inflammation surrounded by a lymphocytic<\/span> response to progressively higher increases. Tuber organisms<\/span> are generally easy to find at first suppurative<\/span> lesions and difficult to identify when granulomatous inflammation has developed well.<\/p>\nLupus vulgaris is the most common clinical type of cutaneous tuberculosis reinfection and shows good formation. granulomas<\/span> surrounded by chronic inflammatory cells, and central necrosis<\/span> (Figures 2-4). <\/span>Special studies for cutaneous tuberculosis.<\/span><\/h2>\nOrganisms can be demonstrated in cutaneous tuberculosis with Ziehl-Neelen \/ rapid acid staining, but they can be extremely difficult to find and absent in many cases, particularly when the granulomatous response is well established. Culture<\/span> Studies are sometimes helpful, but can take months to complete. Polymerase chain reaction<\/span> (PCR<\/span>) may allow rapid analysis, but may not be very sensitive in paucibacillary forms of cutaneous tuberculosis.<\/p>\n\n

<\/span>\nDifferential diagnosis<\/span> of cutaneous tuberculosis pathology<\/span>
\n<\/span><\/h2>\nGranulomatous syphilis, deep fungal infection, leishmaniasis - PCR and culture studies may be necessary for the distinction in difficult cases.<\/p>\n
Tuberculoid Leprosy - Neural infiltration<\/span> and less necrosis are clues to this diagnosis. Plasma<\/span> Cells are said to be more common in leprosy. PCR or culture studies may be necessary for this distinction.<\/p>\nSarcoidosis: generally lacks caseante necrosis<\/span> but it can show something fibrinoid<\/span> Degeneration within the granulomas that can cause confusion. The sarcoid generally lacks the lymphocytic response that is often seen in tuberculosis (Figures 1, 2).
