There are several types of urticaria. The name urticaria is derived from the common European nettle &#039;Urtica dioica&#039;.<\/p>\n<p>A <span class=\"term\" data-term-id=\"302\">bump<\/span> (or <span class=\"term\" data-term-id=\"1010\">bump<\/span>) is a superficial swelling of pale or skin color, usually surrounded by <span class=\"term\" data-term-id=\"90\">erythema<\/span> (redness) that lasts from a few minutes to 24 hours. Usually itchy, may have a burning sensation.<\/p>\n<p>Angioedema is a deeper swelling inside the skin or <span class=\"term\" data-term-id=\"188\">mucous<\/span> membranes and can be skin colored or red. It resolves in 72 hours. It often goes away in a matter of hours or days.<\/p>\n<div id=\"square-placement-country-holder\" class=\"country-dependent advert\">\n<div class=\"box-placement square-placement\">\n<div class=\"advert__frame\">\n<div id=\"dermnet-dermnet-mobbox\">\n<\/div><\/div><\/div><\/div>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFQuien-contrae-urticaria-aguda\"><\/span>Who gets acute urticaria?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>One in five children or adults has an episode of acute urticaria during their lifetime. It is more common in <span class=\"term\" data-term-id=\"922\">atopic<\/span> individuals. It affects all races and both sexes.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFQue-causa-la-urticaria-aguda\"><\/span>What causes acute urticaria?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>The scars are due to the release of chemical mediators from the tissue. <span class=\"term\" data-term-id=\"582\">mast cells<\/span> and circulating <span class=\"term\" data-term-id=\"445\">basophils<\/span>. These chemical mediators include <span class=\"term\" data-term-id=\"856\">histamine<\/span>, <span class=\"term\" data-term-id=\"1089\">platelet<\/span>activation factor and <span class=\"term\" data-term-id=\"434\">cytokines<\/span>. Mediators activate the sensory <span class=\"term\" data-term-id=\"952\">nerves<\/span> and cause dilation of <span class=\"term\" data-term-id=\"953\">blood vessels<\/span> and fluid leakage into the surrounding tissues. The release of bradykinin causes angioedema.<\/p>\n<p>Several hypotheses have been proposed to explain urticaria. The immune, arachidonic acid and <span class=\"term\" data-term-id=\"604\">coagulation<\/span> systems are involved, and <span class=\"term\" data-term-id=\"558\">genetic<\/span> <span class=\"term\" data-term-id=\"871\">mutations<\/span> are under investigation<\/p>\n<p><span class=\"term\" data-term-id=\"1430\">Serum<\/span> The disease and disease-like serum reactions are due to the immune complex. <span class=\"term\" data-term-id=\"370\">statement<\/span> in affected tissues.<\/p>\n<p>Acute urticaria can be induced by the following factors, but the cause is not always identified.<\/p>\n<ul>\n<li>Acute viral <span class=\"term\" data-term-id=\"145\">infection<\/span> - an upper respiratory infection, viral <span class=\"term\" data-term-id=\"1801\">hepatitis<\/span>infectious mononucleosis\n<\/li>\n<li>Acute <span class=\"term\" data-term-id=\"937\">bacterial<\/span> infection - a dental <span class=\"term\" data-term-id=\"1\">abscess<\/span>sinusitis<span>, <\/span>mycoplasma\n<\/li>\n<li>\n<p>Food <span class=\"term\" data-term-id=\"1136\">allergy<\/span> (IgE mediated) - usually milk, egg, peanuts, shellfish<\/li>\n<li>\n<p>Drug-induced urticaria (IgE-mediated): often an antibiotic\n<\/li>\n<li>\n<p>Drug-induced urticaria due to pseudoallergy - aspirin, non-selective non-steroidal<span class=\"term\" data-term-id=\"490\">inflammatory<\/span> drugs, opiates, radio contrast media; these cause hives without immune activation<\/li>\n<li>Vaccination<\/li>\n<li>Bee or wasp stings<\/li>\n<li>\n<span class=\"term\" data-term-id=\"309\">Extended<\/span> next reaction <span class=\"term\" data-term-id=\"324\">located<\/span> contact urticaria - rubber latex\n<\/li>\n<\/ul>\n<p>Severe allergic urticaria can lead to <span class=\"term\" data-term-id=\"1014\">anaphylactic<\/span> shock (<span class=\"term\" data-term-id=\"1679\">bronchospasm<\/span>, collapse).<\/p>\n<p>A single episode or <span class=\"term\" data-term-id=\"805\">recurrent<\/span> Episodes of angioedema without urticaria may be due to angiotensin conversion <span class=\"term\" data-term-id=\"1525\">enzyme<\/span> (ACE) inhibitor drug.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFCuales-son-las-caracteristicas-clinicas-de-la-urticaria-aguda\"><\/span>What are the clinical characteristics of acute urticaria?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p><span class=\"term\" data-term-id=\"293\">Urticaria<\/span> Scars can be a few millimeters or several centimeters in diameter, white or red in color, with or without a red flare. Each cure can last a few minutes or several hours and can change shape. The marks can be round, or form rings, a map-like pattern, <span class=\"term\" data-term-id=\"1623\">targettoid<\/span> giant lesions or patches.<\/p>\n<p>Acute urticaria can affect any part of the body and tends to spread widely.<\/p>\n<p>Angioedema is located more frequently. It commonly affects the face (especially the eyelids and <span class=\"term\" data-term-id=\"227\">perioral<\/span> sites), hands, feet and genitals. May involve tongue, uvula, soft palate, <span class=\"term\" data-term-id=\"864\">larynx<\/span>.<\/p>\n<p>Serum sickness due to blood transfusion and serum sickness-like reactions due to certain medications cause acute urticaria that leaves bruises, <span class=\"term\" data-term-id=\"1176\">fever<\/span>, swollen lymph nodes, joint pain and swelling.<\/p>\n<div id=\"square-placement-country-holder\" class=\"country-dependent advert\">\n<div class=\"box-placement square-placement\">\n<div class=\"advert__frame\">\n<div id=\"dermnet-dermnet-mobbox\">\n<\/div><\/div><\/div><\/div>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFComo-se-diagnostica-la-urticaria-aguda\"><\/span>How is acute urticaria diagnosed?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Acute urticaria is diagnosed in people with a short history of illness lasting less than 24 hours, with or without angioedema. A complete physical examination should be performed to search for the underlying causes.<\/p>\n<p>Radioallergosorbent skin tests (RAST) or CAP fluoroimmunoassay may be ordered if a food or drug allergy is suspected in acute urticaria.<\/p>\n<p><span class=\"term\" data-term-id=\"24\">Biopsy<\/span> Hives can be nonspecific and difficult to interpret. the <span class=\"term\" data-term-id=\"223\">pathology<\/span> shows <span class=\"term\" data-term-id=\"201\">edema<\/span> at <span class=\"term\" data-term-id=\"61\">dermis<\/span> and dilated blood vessels, with a mixed inflammatory variable <span class=\"term\" data-term-id=\"541\">infiltrate<\/span>. Damage to the vessel wall indicates hives <span class=\"term\" data-term-id=\"1091\">vasculitis<\/span>.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFCual-es-el-tratamiento-para-la-urticaria-aguda\"><\/span>What is the treatment for acute urticaria?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>The main treatment for acute urticaria in adults and children is with a second-generation oral antihistamine chosen from the list below. If the standard dose (eg, 10 mg of cetirizine) is not effective, the dose may be increased four times (eg, 40 mg of cetirizine daily). They are best taken on an ongoing basis rather than on demand. They stop when the acute urticaria has subsided. There is not believed to be any benefit in adding a second antihistamine.<\/p>\n<ul>\n<li>Cetirizine<\/li>\n<li>Loratadine<\/li>\n<li>Fexofenadine<\/li>\n<li>Desloratadine<\/li>\n<li>Levocetirizine<\/li>\n<li>Rupatadine<\/li>\n<li>Bilastine<\/li>\n<\/ul>\n<p>Terfenadine and astemizole should not be used as they are cardiotoxic in combination with ketoconazole or erythromycin. They are no longer available in New Zealand.<\/p>\n<p>Although <span class=\"term\" data-term-id=\"380\">systemic<\/span> it is better to avoid treatment during pregnancy and lactation, there have been no reports that second generation antihistamines cause birth defects. If treatment is required, loratadine and cetirizine are currently preferred.<\/p>\n<p>Conventional first-generation antihistamines like Promethazine or Chlorpheniramine are no longer recommended for urticaria.<\/p>\n<p>Psychotherapy reportedly benefits some patients with <span class=\"term\" data-term-id=\"319\">chronic<\/span> urticaria.<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Evitar-los-factores-desencadenantes\"><\/span>Avoid triggers<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>In addition to antihistamines, the cause of hives should be eliminated if known (for example, food or drug allergies). Avoid relevant type 1 (IgE mediated) <span class=\"term\" data-term-id=\"409\">Allergens<\/span> clears hives within 48 hours.<\/p>\n<p>In addition to antihistamines, urticaria triggers should be avoided whenever possible. For example:<\/p>\n<ul>\n<li>Avoid aspirin, opiates, and non-steroids. <span class=\"term\" data-term-id=\"1413\">anti-inflammatory<\/span> medications (acetaminophen is generally safe).<\/li>\n<li>Avoid known <span class=\"term\" data-term-id=\"1137\">allergies<\/span> that have been confirmed by specific positive IgE \/ skin prick tests if they have clinical relevance for urticaria.<\/li>\n<li>Cool the affected area with a fan, cold flannel, an ice pack, or a soothing moisturizer. <span class=\"term\" data-term-id=\"1120\">lotion<\/span>.<\/li>\n<\/ul>\n<h3><span class=\"ez-toc-section\" id=\"Tratamiento-de-la-urticaria-aguda-refractaria\"><\/span>Treatment of acute refractory urticaria<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>If non-sedating antihistamines are not effective, a 4-5 day course of oral prednisone (prednisolone) may be warranted in severe acute urticaria, particularly if angioedema is present.
