It is often shortened to atrophic vaginitis, as it is the vaginal tissue that is often symptomatic. Because of his <span class=\"term\" data-term-id=\"589\">predominance<\/span> in older women, it is also known as senile vulvovaginitis.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFQue-causa-la-vulvovaginitis-atrofica\"><\/span>What Causes Atrophic Vulvovaginitis?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Atrophic vulvovaginitis is associated with <span class=\"term\" data-term-id=\"1844\">estrogen<\/span> deficiency due to:<\/p>\n<ul>\n<li>Menopause<\/li>\n<li>Lactation<\/li>\n<li>Hyperprolactinemia<\/li>\n<li>High dose <span class=\"term\" data-term-id=\"1841\">progesterone<\/span> medicines<\/li>\n<li>Chest <span class=\"term\" data-term-id=\"30\">Cancer<\/span> medications, such as tamoxifen and aromatase inhibitors.<\/li>\n<\/ul>\n<div id=\"square-placement-country-holder\" class=\"country-dependent advert\">\n<div class=\"box-placement square-placement\">\n<div class=\"advert__frame\">\n<div id=\"dermnet-dermnet-mobbox\">\n<\/div><\/div><\/div><\/div>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFCuales-son-los-sintomas-de-la-vulvovaginitis-atrofica\"><\/span>What are the symptoms of atrophic vulvovaginitis?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<ul>\n<li>Vaginal or vulvar dryness<\/li>\n<li>Vaginal or vulvar itching (<span class=\"term\" data-term-id=\"245\">pruritus<\/span> vulvas)<\/li>\n<li>Vaginal burning<\/li>\n<li>Painful <span class=\"term\" data-term-id=\"1167\">sex<\/span> (<span class=\"term\" data-term-id=\"1057\">dyspareunia<\/span>)<\/li>\n<li>Skin division (<span class=\"term\" data-term-id=\"1772\">fissure<\/span>) from the entrance to the vagina (<span class=\"term\" data-term-id=\"239\">later<\/span> fourchette)<\/li>\n<li>Vaginal spotting (bleeding)<\/li>\n<\/ul>\n<p>Estrogen deficiency can also lead to <span class=\"term\" data-term-id=\"506\">dysuria<\/span> (burning sensation when urinating), urinary urgency, frequency and incontinence (the genitourinary <span class=\"term\" data-term-id=\"350\">syndrome<\/span> menopause).<\/p>\n<h2><span class=\"ez-toc-section\" id=\"Las-bacterias-en-la-vagina\"><\/span>\n<span class=\"term\" data-term-id=\"935\">The bacteria<\/span> in the vagina<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Lack of estrogen causes changes in the normal vagina <span class=\"term\" data-term-id=\"883\">organisms<\/span>. Those seen in younger women (especially <span class=\"term\" data-term-id=\"1058\">lactobacilli<\/span>) disappear and are replaced by gram-negative organisms such as Escherichia coli or those associated with <span class=\"term\" data-term-id=\"937\">bacterial<\/span> vaginosis Urinary or bladder infections are more common in postmenopausal women than in younger women.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFQue-hace-un-atrofico-vulva-y-la-vagina-parece\"><\/span>What makes an atrophic <span class=\"term\" data-term-id=\"1004\">vulva<\/span> and the vagina looks like?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Atrophic vulvovaginitis changes the appearance of the female genitalia:<\/p>\n<ul>\n<li>Vulva appears paler<\/li>\n<li>The lips are thinner and smaller.<\/li>\n<li>Clitoral hood is less obvious<\/li>\n<li>A red membrane or polyp may be seen at the opening of the urethra (urethral caruncle)<\/li>\n<li>Vaginal skin appears thin and dry, with small <span class=\"term\" data-term-id=\"953\">blood vessels<\/span> under it resulting in patchy redness<\/li>\n<li>Stretching the vulva can cause splitting of the skin in the lower part of the vagina or elsewhere.<\/li>\n<\/ul>\n<div id=\"square-placement-country-holder\" class=\"country-dependent advert\">\n<div class=\"box-placement square-placement\">\n<div class=\"advert__frame\">\n<div id=\"dermnet-dermnet-mobbox\">\n<\/div><\/div><\/div><\/div>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFSe-deben-realizar-pruebas-para-confirmar-el-diagnostico\"><\/span>Should tests be done to confirm the diagnosis?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Tests for atrophic vulvovaginitis can be done if any symptoms are present. These may include:<\/p>\n<ul>\n<li>Skin swab \/ <span class=\"term\" data-term-id=\"905\">wet mount<\/span> exam: this may reveal <span class=\"term\" data-term-id=\"847\">epithelial<\/span> Typical cells of the postmenopausal vagina can be reported and can help identify <span class=\"term\" data-term-id=\"145\">infection<\/span>\n<\/li>\n<li>\n<span class=\"term\" data-term-id=\"24\">Biopsy<\/span> - This can be done to ensure that there is no <span class=\"term\" data-term-id=\"490\">inflammatory<\/span> skin disease or <span class=\"term\" data-term-id=\"1264\">neoplastic<\/span> condition causing symptoms<\/li>\n<li>Cystoscopy of the bladder and investigations of urinary symptoms.<\/li>\n<li>\n<span class=\"term\" data-term-id=\"834\">Colposcopy<\/span> of the vagina and cervix and investigations for gynecological symptoms.<\/li>\n<\/ul>\n<h2><span class=\"ez-toc-section\" id=\"Medidas-generales-para-mejorar-la-vulvovaginitis-atrofica\"><\/span>General measures to improve atrophic vulvovaginitis.<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>The following measures are recommended.<\/p>\n<ul>\n<li>Use a soap-free cleanser or wash gently with plain warm water, no more than once or twice a day<\/li>\n<li>Apply <span class=\"term\" data-term-id=\"1887\">emollient<\/span> <span class=\"term\" data-term-id=\"1121\">cream<\/span> Suitable for sensitive skin or Vaseline if the vulva feels dry or itchy.<\/li>\n<li>Use a lubricant for sexual intercourse; if it stings, try another or use oil or petroleum jelly.<\/li>\n<li>A test of a vaginal wetness<\/li>\n<li>Anticholinergic, antihistamine, decongestant, or antidepressant medications may be contributing to the dryness; if you take these consider stopping<\/li>\n<li>Short-term use of <span class=\"term\" data-term-id=\"1330\">current<\/span> steroids may be needed to <span class=\"term\" data-term-id=\"53\">dermatitis<\/span> caused by <span class=\"term\" data-term-id=\"1443\">irritants<\/span> such as urine, pantyhose, or tight clothing<\/li>\n<\/ul>\n<h2><span class=\"ez-toc-section\" id=\"Tratamiento-con-estrogenos\"><\/span>Estrogen treatment<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Atrophic vulvovaginitis is treated with topical estrogens, a prescription drug. This can be provided as a vaginal cream, pessaries, or vaginal ring. In New Zealand, Ovestin \u2122, which contains estriol, is used at a dose of 0.5 mg \/ day for 1 to 2 weeks and then once or twice a week. It can be inserted with an applicator or with the tip of a finger. Vagifem \u2122 pessaries (containing oestrodiol) can also be used, but are currently not funded by PHARMAC in New Zealand.<\/p>\n<p>Estrogen treatment produces:<\/p>\n<ul>\n<li>Normalization of the cells of the vaginal wall.<\/li>\n<li>Improved vaginal blood flow<\/li>\n<li>Vaginal decrease <span class=\"term\" data-term-id=\"890\">pH<\/span>\n<\/li>\n<li>Recolonization by lactobacilli<\/li>\n<li>Improves vaginal thickness and elasticity.<\/li>\n<li>Reduction in <span class=\"term\" data-term-id=\"1003\">vulvovaginal<\/span> symptoms<\/li>\n<li>Improved sexual function<\/li>\n<li>Reduction of urinary infections.<\/li>\n<\/ul>\n<p>Topical estrogen is considered safe because very little is absorbed systemically. However, estrogen is generally not prescribed to women with severe liver disease.<span class=\"term\" data-term-id=\"1179\">dependent<\/span> cancers or thromboembolic disease if the risk of these conditions is increased.<\/p>\n<p>Sometimes other forms of estrogen are recommended, such as tablets, transdermal patches, gels, sprays, and emulsions. <span class=\"term\" data-term-id=\"380\">Systemic<\/span> estrogen is generally mixed with progestogens. There are significant risks and side effects, so they are generally not used if atrophic vaginitis is the only problem.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"Efectos-secundarios-y-riesgos-de-la-terapia-vaginal-con-estrogenos\"><\/span>Side effects and risks of vaginal estrogen therapy.<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Topical estrogen can cause side effects, including:<\/p>\n<ul>\n<li>Vaginal itching and burning<\/li>\n<li>Increased risk of vaginal Candida albicans infection (vaginal yeast infection)<\/li>\n<li>Breast discomfort (rare)<\/li>\n<li>Vaginal bleeding (rare).<\/li>\n<\/ul>\n<p>When used only once or twice a week, there are no other side effects described with higher doses or modes of estrogen administration.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"Otros-tratamientos-para-la-vulvovaginitis-atrofica\"><\/span>Other treatments for atrophic vulvovaginitis<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Experimental options for vulvovaginal rejuvenation in symptomatic women who are unfit or intolerant of local or systemic estrogen therapy include: <\/p>\n<ul>\n<li><span class=\"term\" data-term-id=\"1089\">Platelet<\/span>-Rich <span class=\"term\" data-term-id=\"1513\">plasma<\/span> injections<\/li>\n<li>\n<p>Hyaluronic acids or fat implants (lipofilling of the labia majora)<\/li>\n<li>Fractional carbon dioxide <span class=\"term\" data-term-id=\"1617\">To be<\/span>, non-ablative erbium: YAG lasers, diode lasers, and monopolar radio frequency devices <\/li>\n<li>Vaginal surgery (vaginoplasty).<\/li>\n<\/ul>\n<p>The optimal regimens, effectiveness, and safety of these procedures have yet to be determined.<\/p>\n<\/section>","protected":false},"excerpt":{"rendered":"<p>Que es atr\u00f3fica vulvovaginitis?
