It is switched in the liver to a related drug, mercaptopurine, and then to <span class=\"term\" data-term-id=\"717\">metabolites<\/span> called thioguanine (thioguanine) <span class=\"term\" data-term-id=\"1863\">nucleotides<\/span>, which <span class=\"term\" data-term-id=\"1022\">inhibit<\/span> cell growth.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFPara-que-trastornos-de-la-piel-se-usan-azatioprina-y-mercaptopurina\"><\/span>What skin disorders are azathioprine and mercaptopurine used for?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>The main <span class=\"term\" data-term-id=\"47\">cutaneous<\/span> The indications for azathioprine and mercaptopurine are:<\/p>\n<ul>\n<li><span class=\"term\" data-term-id=\"380\">Systemic<\/span> lupus erythematosus<\/li>\n<li>Dermatomyositis<\/li>\n<li>Pemphigus vulgaris<\/li>\n<li>\n<p>Cutaneous polyarteritis nodosa.<\/li>\n<\/ul>\n<p>Other skin disorders treated with these medications include:<\/p>\n<ul>\n<li>\n<span class=\"term\" data-term-id=\"922\">Atopic<\/span> <span class=\"term\" data-term-id=\"53\">dermatitis<\/span> (<span class=\"term\" data-term-id=\"491\">eczema<\/span>)<\/li>\n<li><span class=\"term\" data-term-id=\"498\">Bullous<\/span> pemphigoid<\/li>\n<li><span class=\"term\" data-term-id=\"1626\">Pyoderma<\/span> gangrenous<\/li>\n<li>Cutaneous lupus erythematosus<\/li>\n<li>\n<p>Cutaneous <span class=\"term\" data-term-id=\"1091\">vasculitis<\/span>.<\/li>\n<\/ul>\n<div id=\"square-placement-country-holder\" class=\"country-dependent advert\">\n<div class=\"box-placement square-placement\">\n<div class=\"advert__frame\">\n<div id=\"dermnet-dermnet-mobbox\">\n<\/div><\/div><\/div><\/div>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFQue-dosis-se-usan\"><\/span>What doses are used?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>In New Zealand, azathioprine is available as 50 mg tablets or as an injection, brand name Imuran\u00ae, Azamun\u00ae, Imuprine. Mercaptopurine is available in 10mg or 50mg tablets, brand name Puri-nethol\u00ae. The dose is generally one or two tablets once or twice a day (1\u20133 mg \/ kg \/ day). Occasionally, doses of up to 5 mg per kg of body weight per day are needed.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFCuales-son-los-efectos-secundarios-y-los-riesgos-de-la-azatioprina-y-la-mercaptopurina\"><\/span>What are the side effects and risks of azathioprine and mercaptopurine?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Side effects are generally mild, but occasionally serious enough to stop treatment with azathioprine or mercaptopurine.<\/p>\n<ul>\n<li>Loss of appetite, nausea, vomiting, or diarrhea (more common if medications are taken on an empty stomach). Mouth <span class=\"term\" data-term-id=\"1278\">ulceration<\/span> It can also happen.<\/li>\n<li>Liver discomfort<span class=\"term\" data-term-id=\"1711\">cholestatic<\/span> <span class=\"term\" data-term-id=\"1801\">hepatitis<\/span>) <span class=\"term\" data-term-id=\"943\">Hypersensitivity<\/span> The reaction to azathioprine generally affects the liver, most often several weeks after starting treatment.<\/li>\n<li><span class=\"term\" data-term-id=\"886\">Pancreatitis<\/span><\/li>\n<li>Lung disease: <span class=\"term\" data-term-id=\"349\">acute<\/span> <span class=\"term\" data-term-id=\"750\">interstitial<\/span> pneumonia, <span class=\"term\" data-term-id=\"449\">eosinophilic<\/span> pneumonia or <span class=\"term\" data-term-id=\"319\">chronic<\/span> <span class=\"term\" data-term-id=\"1051\">pneumonitis<\/span>\n<\/li>\n<li>Suppression of the bone marrow, particularly <span class=\"term\" data-term-id=\"1813\">leukopenia<\/span> (reduced number of white blood cells); azathioprine should be discontinued if the <span class=\"term\" data-term-id=\"441\">neutrophils<\/span> the count falls below 1.0 \u00d7 10 ^ 9 \/ L, the <span class=\"term\" data-term-id=\"420\">lymphocyte<\/span> the count falls below 0.5 \u00d7 10 ^ 9 \/ L or the <span class=\"term\" data-term-id=\"1089\">platelet<\/span> counts below 50 \u00d7 10 ^ 9 \/ L.<\/li>\n<li>Megaloblastic erythropoiesis or <span class=\"term\" data-term-id=\"1458\">macrocytosis<\/span> (production of large red blood cells).<\/li>\n<li>Prolonged severe drug hypersensitivity <span class=\"term\" data-term-id=\"350\">syndrome<\/span> Resulting in <span class=\"term\" data-term-id=\"309\">extended<\/span> <span class=\"term\" data-term-id=\"255\">eruption<\/span>, <span class=\"term\" data-term-id=\"1176\">fever<\/span>, enlarged lymph nodes and organ failure (heart, lungs, kidneys, liver).<\/li>\n<li><span class=\"term\" data-term-id=\"122\">Hair<\/span> lost<\/li>\n<li>Immunosuppression leads to an increased risk of serious infections.<\/li>\n<li>\n<p>Skin <span class=\"term\" data-term-id=\"30\">Cancer<\/span>, especially cutaneous <span class=\"term\" data-term-id=\"270\">scaly<\/span> cell <span class=\"term\" data-term-id=\"32\">carcinoma<\/span>. This is more likely after several years of azathioprine treatment, especially on skin that has been exposed to the sun. Protect your skin from the sun at all times, using a broad-spectrum sunscreen on exposed areas every day.<\/li>\n<\/ul>\n<p>Azathioprine and mercaptopurine are FDA category D pregnancy. When possible, these medications should not be taken during pregnancy, except when the benefits outweigh the risks. Those taking these medications should not breastfeed. In some circumstances, it may also be advisable for men to ensure that their partners do not become pregnant, but it is generally believed that azathioprine does not affect fertility in men. Discuss your own situation with your doctor.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"Prevencion-de-infeccion-cuando-toma-azatioprina-o-mercaptopurina\"><\/span>Prevention of <span class=\"term\" data-term-id=\"145\">infection<\/span> when you take azathioprine or mercaptopurine<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Evidence to exclude <span class=\"term\" data-term-id=\"1040\">latent<\/span> Tuberculosis and Hepatitis B \/ C are wise before starting azathioprine and for determining immunity against measles and chicken pox. It is also important to consider the need to update immunization in <span class=\"term\" data-term-id=\"1699\">immunosuppressed<\/span> <span class=\"term\" data-term-id=\"55\">dermatology<\/span> patients<\/p>\n<div id=\"square-placement-country-holder\" class=\"country-dependent advert\">\n<div class=\"box-placement square-placement\">\n<div class=\"advert__frame\">\n<div id=\"dermnet-dermnet-mobbox\">\n<\/div><\/div><\/div><\/div>\n<h2><span class=\"ez-toc-section\" id=\"Monitoreo-mientras-toma-azatioprina-o-mercaptopurina\"><\/span>Monitoring while taking azathioprine or mercaptopurine<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Patients taking azathioprine or mercaptopurine should be closely monitored and regular blood tests should be performed to monitor the safety of the treatment, first biweekly and then monthly long term.<\/p>\n<ul>\n<li>Complete blood count (CBC or FBC)<\/li>\n<li>Liver function tests<\/li>\n<\/ul>\n<p>Azathioprine and mercaptopurine are not properly metabolized by some people, who have a failure <span class=\"term\" data-term-id=\"1525\">enzyme<\/span> called thiopurine methyltransferase. They can have one <span class=\"term\" data-term-id=\"774\">gene<\/span> for the bad enzyme (found in the 11% of the population) or two bad <span class=\"term\" data-term-id=\"775\">genes<\/span> (at 0.3%). Those with both genes are at severe risk of dangerous marrow suppression, that is, reduction of white blood cells (leukopenia) or of all blood cells (pancytopenia). Some other people have high levels of enzyme activity and may require a higher dose than normal to <span class=\"term\" data-term-id=\"1150\">therapeutic<\/span> effect.<\/p>\n<p>Thiopurine methyltransferase (TPMT) levels can be measured to determine the patient&#039;s level of risk before starting treatment. Low levels are &lt;5 U \/ mL, intermediate levels are 5\u201313.7 U \/ mL, and high levels are&gt; 13.8 U \/ ml. A typical therapeutic dose of azathioprine is 1 mg \/ kg \/ day when the TPMT level is 10 U \/ ml and 3.5 mg \/ kg \/ day when it is 20 U \/ ml.<\/p>\n<p>6-thioguanine <span class=\"term\" data-term-id=\"1862\">nucleotide<\/span> levels are sometimes used to check if the dose is correct.<\/p>\n<p>Patients taking azathioprine and mercaptopurine should also have regular skin checks and self-exams.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"Interacciones-farmacologicas-con-azatioprina-y-mercaptopurina\"><\/span>Pharmacological interactions with azathioprine and mercaptopurine<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>If you take azathioprine, you should inform any doctor or pharmacist. It is particularly important if you are taking allopurinol for gout since the azathioprine dose should be reduced from a quarter to a third of the normal amount to avoid <span class=\"term\" data-term-id=\"1160\">toxicity<\/span>.<\/p>\n<p>It is best to avoid the following medications, but if it is unavoidable, it is important to monitor your blood count very carefully:<\/p>\n<ul>\n<li style=\"list-style-type: none;\">\n<ul>\n<li>Neuromuscular blocking agents (such as rocuronium, mivacurium, vecuronium, atracurium, tubocurarine)<\/li>\n<li>Warfarin (warfarin dose requirement may increase 3 to 4 times during azathioprine treatment)<\/li>\n<li>D-penicillamine<\/li>\n<li><span>Trimethoprim + sulfamethoxazole<\/span><\/li>\n<li>Captopril, enalapril, lisinopril and other angiotensin converters <span class=\"term\" data-term-id=\"1526\">enzymes<\/span> Inhibitors (ACE)<\/li>\n<li>Cimetidine<\/li>\n<li>Indomethacin<\/li>\n<\/ul>\n<\/li>\n<li>\n<p>Sulfasalazine and other derivatives of benzoic acid.<\/li>\n<li>Live vaccines like yellow fever.<\/li>\n<\/ul>\n<section class=\"textBlock\">\n<div class=\"grey-box\"><em><span style=\"font-size: 0.9em;\">New Zealand approved data sheets are the official source of information for these prescription drugs, including approved uses and risk information.
