<\/span><\/h2>\nLichen aureus is a distinct subtype of pigmented<\/span> purpuric skin disease<\/span>. Clinically, it generally affects children or young adults and presents as a persistent rust color. plates<\/span> in the lower extremities. <\/p>\n<\/span>\nHistology<\/span> lichen aureus <\/span><\/h2>\nIn lichen aureus, sections generally show a different pattern of infiltration<\/span> in all kinds of purpuric pigments dermatitis<\/span>, with infiltration around the small vessels of the superficial plexus and minimal epidermal<\/span> reaction (figure 1). At higher power, the infiltrate<\/span> It mainly consists of lymphocytes<\/span> with a pretty impressive ratio of histiocytes<\/span> (Figure 2). The vessels may have some prominence of the endothelial<\/span> cells but there is no evidence of vascular<\/span> to damage. <\/span>Special studies for lichen aureus<\/span><\/h2>\nPerls' Prussian blue iron stain demonstrates the statement<\/span> of hemosiderin in the superficial dermis (figure 3).<\/p>\n<\/span>\nDifferential diagnosis<\/span> for lichen aureus<\/span><\/h2>\nOther diagnoses to consider include:<\/p>\n
\n- Other forms of pigmented purpuric dermatitis: some authorities no longer make the distinction between clinical forms of pigmented purpuric dermatitis; histopathologically<\/span>, lichen aureus has a high percentage of histiocytes in the infiltrate<\/li>\n
- \nHistiocytic<\/span> disorders (eg, Langerhans cells histiocytosis<\/span>) - these can be excluded by demonstration of dermal<\/span> hemosiderin with Perls iron stain<\/li>\n
- Mycosis fungoides: Pigmented purpuric dermatitis can sometimes mimic cutaneous<\/span> T cell<\/span> lymphoma<\/span> histopathologically; dense histiocytic infiltrators<\/span> and dermal hemosidein are unusual in mycosis fungoides.<\/li>\n
