data-term-id=\"47\">cutaneous<\/span> Cell B <span class=\"term\" data-term-id=\"976\">lymphoma<\/span>?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Lymphomas are tumors of the <span class=\"term\" data-term-id=\"1775\">lymph nodes<\/span> and <span class=\"term\" data-term-id=\"744\">lymphatic<\/span> system.<\/p>\n<ul>\n<li>\n<span class=\"term\" data-term-id=\"1790\">Extranodal<\/span> Lymphomas are tumors that occur in organs or tissues outside the lymphatic system.  <\/li>\n<li>\n<span class=\"term\" data-term-id=\"949\">Primary<\/span> cutaneous lymphomas occur on the skin without evidence of <span class=\"term\" data-term-id=\"995\">extracutaneous<\/span> disease at diagnosis.<\/li>\n<\/ul>\n<p>Primary cutaneous lymphoma can be divided into two categories:<\/p>\n<ul>\n<li>Cutaneous <span class=\"term\" data-term-id=\"631\">T cell<\/span> lymphoma <\/li>\n<li>B cell lymphoma.<\/li>\n<\/ul>\n<p>Primary cutaneous B-cell lymphomas (PCBL) comprise approximately 20% of cutaneous lymphomas and are described on this page. <\/p>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFQue-son-los-linfomas-cutaneos-de-celulas-B\"><\/span>What are cutaneous B-cell lymphomas?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Cutaneous B-cell lymphomas are a <span class=\"term\" data-term-id=\"176\">evil one<\/span> <span class=\"term\" data-term-id=\"893\">proliferation<\/span> of <span class=\"term\" data-term-id=\"421\">lymphocytes<\/span> B cell type. <span class=\"term\" data-term-id=\"870\">Mutation<\/span> what happens at different points of <span class=\"term\" data-term-id=\"429\">Cell B<\/span> Development leads to different forms of lymphoma.<\/p>\n<p>In 2005, the World Health Organization (WHO) and the European Organization for Research and Treatment of <span class=\"term\" data-term-id=\"30\">Cancer<\/span> The classification (EORTC) reached a consensus classification for cutaneous lymphomas. It was reviewed by the WHO in 2008. The three main types of PCBCL are:<\/p>\n<ul>\n<li>Primary dermal <span class=\"term\" data-term-id=\"1300\">follicle<\/span> central lymphoma (PCFCL)<\/li>\n<li>\n<p>Primary dermal <span class=\"term\" data-term-id=\"1187\">marginal<\/span> zone B- cell lymphoma (PCMZL)<\/li>\n<li>Primary dermal <span class=\"term\" data-term-id=\"1769\">diffuse<\/span> Large B-cell lymphoma, leg type (PCDLBCL-LT).<\/li>\n<\/ul>\n<p>Rare cases of PCDLBCL that do not belong to the PCDLBCL-LT or PCFCL groups are classified as PCDLBCL-other. Other entities like <span class=\"term\" data-term-id=\"1327\">anaplastic<\/span> or plasmablastic lymphoma, primary cutaneous T cells Rev.<span class=\"term\" data-term-id=\"857\">histiocyte<\/span>rich in B-cell lymphomas and primary cutaneous <span class=\"term\" data-term-id=\"1162\">intravascular<\/span> Large B-cell lymphomas are very rare.<\/p>\n<p>Due to differences in treatment and <span class=\"term\" data-term-id=\"241\">forecast<\/span>, it is important to discern between PCBCL and <span class=\"term\" data-term-id=\"380\">systemic<\/span> forms of B-cell lymphomas that manifest with secondary skin involvement.  <\/p>\n<section class=\"textBlock imageLinkBlockTitle\">\n<h5 id=\"Linfoma-cutaneo-primario-del-centro-folicular\"><\/span>Primary cutaneous lymphoma of the follicular center<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<ul>\n<li>\n<span>Primary Lymphoma of the Cutaneous Follicular Center (PCFCL)<\/span> It is the second most common lymphoma in the western world.<\/li>\n<li>It is the most common type of PCBCL.<\/li>\n<li>It has a 5-year survival of approximately 95%.<\/li>\n<li>It affects older patients, median age 60 years.<\/li>\n<\/ul>\n<h3><span class=\"ez-toc-section\" id=\"Linfoma-cutaneo-primario-de-celulas-B-de-la-zona-marginal\"><\/span>Primary cutaneous B-cell lymphoma of the marginal zone<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>See B-cell lymphoma of the primary cutaneous marginal zone (PCMZL).<\/p>\n<ul>\n<li>Men are affected twice as much as women.<\/li>\n<li>Some are associated with <em>Borrelia burgdorferi<\/em> <span class=\"term\" data-term-id=\"145\">infection<\/span> (the cause of Lyme disease) in <span class=\"term\" data-term-id=\"814\">endemic<\/span> areas of Europe and the United States.<\/li>\n<li>It has the best 5-year survival of the 99%.<\/li>\n<li>Younger patients are affected, with a median age of 55 years, but it can also be found in children.<\/li>\n<\/ul>\n<h3><span class=\"ez-toc-section\" id=\"Linfoma-cutaneo-primario-difuso-de-celulas-B-grandes-tipo-pierna-PCDLBCL-LT\"><\/span>Diffuse primary cutaneous large B-cell lymphoma, leg type (PCDLBCL-LT)<span class=\"term\" data-term-id=\"1190\">violaceous<\/span>.<\/li>\n<li>They are usually found on the scalp, forehead, or trunk.<\/li>\n<li>They are very rarely found on the leg.<\/li>\n<\/ul>\n<p>Especially when it affects the head or face, PCFCL can simulate rosacea, folliculitis, acne, lupus miliaris (TB), and insect bites. The list of differential diagnoses should also include <span class=\"term\" data-term-id=\"831\">basal<\/span> cell <span class=\"term\" data-term-id=\"32\">carcinoma<\/span>, Merkel cell carcinoma, cutaneous <span class=\"term\" data-term-id=\"1266\">lymphoid<\/span> <span class=\"term\" data-term-id=\"136\">hyperplasia<\/span> and other non-B-cell skin tumors (eg, cutaneous T-cell lymphomas).<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Linfoma-cutaneo-primario-de-celulas-B-de-la-zona-marginal-2\"><\/span>Primary cutaneous B-cell lymphoma of the marginal zone<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>PCMZL is a low-grade malignant B-cell lymphoma of MALT (<span class=\"term\" data-term-id=\"189\">mucous membrane<\/span>associated lymphoid tissue).  <\/p>\n<ul>\n<li>PCMZL presents with solitary or multiple papules, plaques, or nodules.<\/li>\n<li>They are red to purplish in color.<\/li>\n<li>They are most often found on the trunk and upper extremities.<\/li>\n<\/ul>\n<p>Often the lesions appear clinically minor and, in some case reports, can mimic the appearance of basal cell carcinoma. Other differential diagnoses include <span class=\"term\" data-term-id=\"390\">arthropod<\/span> bites <span class=\"term\" data-term-id=\"1646\">urticaria<\/span>, <span class=\"term\" data-term-id=\"613\">leukemia<\/span> <span class=\"term\" data-term-id=\"48\">skin<\/span> and drug-induced pseudolymphoma.<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Linfoma-cutaneo-primario-difuso-de-celulas-B-grandes-tipo-pierna\"><\/span>Diffuse primary cutaneous large B-cell lymphoma, leg type<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<ul>\n<li>\n<span>PCDLBCL-LTT presents with s<\/span>multiple or olitary nodules, infiltrated plaques and tumors.<\/li>\n<li>Rarely, it occurs as <span class=\"term\" data-term-id=\"299\">warty<\/span> plates o <span class=\"term\" data-term-id=\"309\">extended<\/span> garland-like lesions.<\/li>\n<li>They are typically red to bluish in color.<\/li>\n<li>They are found mainly on one or both legs, but affects other sites at 10-15%.<\/li>\n<li>Represents an aggressive subtype of PCBCL.<\/li>\n<li>Multiple skin lesions on the leg have a poor prognosis.<\/li>\n<\/ul>\n<h3><span class=\"ez-toc-section\" id=\"Linfoma-cutaneo-primario-difuso-de-celulas-B-grandes-otro\"><\/span>Diffuse primary cutaneous lymphoma of large B cells, other<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<ul>\n<li>\n<span>PCDLBCL-other <\/span>includes large B-cell lymphoma, which presents with skin lesions on the head, trunk, or extremities.<\/li>\n<li>Blastic plasmacytoid dendritic cell <span class=\"term\" data-term-id=\"195\">neoplasm<\/span> it presents with plaques similar to hematomas, nodules and tumors. Later it progresses to involve the central nervous system. It represents a type of leukemia cutis.<\/li>\n<li>\n<p>Intravascular large B-cell lymphoma proliferates within the <span class=\"term\" data-term-id=\"1265\">lumen<\/span> when I was little <span class=\"term\" data-term-id=\"953\">blood vessels<\/span>, mainly in the skin and the central nervous system and later in other organs. Lesions appear as <span class=\"term\" data-term-id=\"967\">erythematous<\/span>, sensitive nodules, tumors and <span class=\"term\" data-term-id=\"700\">telangiectasias<\/span> mainly on the trunk and lower legs. Initial blood tests should include:<\/p>\n<ul>\n<li>Complete and differential blood count<\/li>\n<li>Metabolic profile and lactate dehydrogenase.<\/li>\n<\/ul>\n<p>If the complete blood count shows lymphocytosis, <span class=\"term\" data-term-id=\"1374\">peripheral<\/span> Blood flow cytometry may be needed.<\/p>\n<p>Adequate skin <span class=\"term\" data-term-id=\"24\">biopsy<\/span> It is also important for evaluation and staging. 4\u20136 mm punch, incisional or <span class=\"term\" data-term-id=\"723\">excisional <span class=\"term\" data-term-id=\"1111\">biopsies<\/span><\/span>  must be obtained and must include the <span class=\"term\" data-term-id=\"1400\">lattice<\/span> <span class=\"term\" data-term-id=\"61\">dermis<\/span> and <span class=\"term\" data-term-id=\"900\">subcutaneous<\/span> grease. Surface biopsy samples may not differentiate PCBCLs from reagents or <span class=\"term\" data-term-id=\"490\">inflammatory<\/span> processes.<\/p>\n<p>Extracutaneous involvement should be evaluated, especially in patients with <span class=\"term\" data-term-id=\"1171\">palpable<\/span> <span class=\"term\" data-term-id=\"503\">lymphadenopathy<\/span>.<\/p>\n<ul>\n<li>Contrast <span class=\"term\" data-term-id=\"799\">Connecticut<\/span> scan<\/li>\n<li>\n<span class=\"term\" data-term-id=\"796\">PET<\/span> scan<\/li>\n<\/ul>\n<p>None <span class=\"term\" data-term-id=\"1774\">lymph node<\/span> identified on images greater than 1.5 cm in length or showing high PET activity must undergo a biopsy; a <span class=\"term\" data-term-id=\"722\">excision biopsy<\/span> preferred wherever possible.<\/p>\n<p><span class=\"term\" data-term-id=\"1669\">Histologically<\/span>, PCBCL shows a monotonous diffuse population of centroblasts and immunoblasts. Histochemical tests are essential to classify the exact type of lymphoma.<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Linfoma-cutaneo-primario-del-centro-folicular-3\"><\/span>Primary cutaneous lymphoma of the follicular center <span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>PCFCL needs to be <span class=\"term\" data-term-id=\"1308\">differentiated<\/span> of a secondary cutaneous lymphoma, which is where a <span class=\"term\" data-term-id=\"1001\">nodal<\/span> <span class=\"term\" data-term-id=\"107\">follicular<\/span> The lymphoma has spread to affect the skin. <span class=\"term\" data-term-id=\"956\">Cellular<\/span> <span class=\"term\" data-term-id=\"187\">morphology<\/span> may vary with age and size of <span class=\"term\" data-term-id=\"164\">injury<\/span>.<\/p>\n<p>Histologically, PCFCL shows:<\/p>\n<ul>\n<li>\n<span class=\"term\" data-term-id=\"52\">Dermal<\/span> and subcutaneous proliferation of centrocytes (excised follicle core cells) and centroblasts (large transformed cells) in a pattern of follicular and \/ or diffuse growth.<\/li>\n<\/ul>\n<p>Immunostaining shows:<\/p>\n<ul>\n<li>Positive B cell <span class=\"term\" data-term-id=\"411\">antigens<\/span>i.e. CD20 and CD79a<\/li>\n<li>Bcl-2 negative protein, y, t (14; 18) <span class=\"term\" data-term-id=\"1849\">translocation<\/span> (which if present may suggest systemic follicular lymphoma)<\/li>\n<li>Negative <span class=\"term\" data-term-id=\"427\">immunoglobulin<\/span> M expression.<\/li>\n<\/ul>\n<p>A <span class=\"term\" data-term-id=\"1237\">FISH<\/span> Analysis is not useful.<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Linfoma-cutaneo-primario-de-celulas-B-de-la-zona-marginal-3\"><\/span>Primary cutaneous B-cell lymphoma of the marginal zone<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p> the <span class=\"term\" data-term-id=\"128\">histology<\/span> PCMZL consists of:<\/p>\n<ul>\n<li>\n<span class=\"term\" data-term-id=\"733\">Nodular<\/span> to spread dermal <span class=\"term\" data-term-id=\"542\">infiltrators<\/span> marginal zone B cells, lymphocytes, lymphoplasmocyte cells, <span class=\"term\" data-term-id=\"1513\">plasma<\/span> cells mixed with reagent <span class=\"term\" data-term-id=\"417\">T cells<\/span> and centroblastic or immunoblastic cells<\/li>\n<li>Marginal zone of B cells, which tend to be small to medium in size with irregular <span class=\"term\" data-term-id=\"729\">nuclei<\/span>discreet nucleoli and abundant pale <span class=\"term\" data-term-id=\"838\">cytoplasm<\/span>. They proliferate in the dermal and subcutaneous regions of the skin.<\/li>\n<\/ul>\n<p>Immunostaining shows<\/p>\n<ul>\n<li>CD20 positive, CD79a and Bcl-2<\/li>\n<li>Negative CD10 and Bcl-6.<\/li>\n<li>Less than 25% from PCMZL harbors the translocation t (14; 18).<\/li>\n<li>It expresses 150 and 155 microRNAs that, if present, can be predictive of longer progression-free survival.<\/li>\n<\/ul>\n<h3><span class=\"ez-toc-section\" id=\"Linfoma-cutaneo-primario-de-celulas-B-grandes-tipo-pierna\"><\/span>Primary cutaneous large B-cell lymphoma, leg type<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>PCFCL and PCDLBCL-LT have diffuse large B-cell infiltrates. PCDLBCL-LT shows:<\/p>\n<ul>\n<li>Diffuse dermal proliferation of centroblasts and immunoblasts in monotonous or <span class=\"term\" data-term-id=\"544\">confluent<\/span> leaves<\/li>\n<li>Larrge round cores with open <span class=\"term\" data-term-id=\"1528\">chromatin<\/span>prominent and identifiable nucleoli <span class=\"term\" data-term-id=\"1536\">mitotic<\/span> figures<\/li>\n<li>Few T cells confined to <span class=\"term\" data-term-id=\"228\">perivascular<\/span> zones<\/li>\n<\/ul>\n<p><span class=\"term\" data-term-id=\"1419\">Histopathological<\/span> Differential diagnoses should include:<\/p>\n<ul>\n<li>Diffuse large B-cell lymphoma not otherwise specified (DLBCL NOS)<\/li>\n<li>\n<p>DLBCL positive for elderly Epstein-Barr virus<\/li>\n<li>\n<span class=\"term\" data-term-id=\"1728\">Lymphomatoid<\/span> granulomatosis<\/li>\n<\/ul>\n<p>Immunostaining shows:<\/p>\n<ul>\n<li>CD20 and CD79a positive with strong expression of cytoplasmic and surface immunoglobulin M.<\/li>\n<li>Strong expression of Bcl-2, MUM1 \/ IRF4 and FOX-P1<\/li>\n<li>Any PCBCL showing expression of Bcl-2 and MUM1 should be classified as a paw type, regardless of anatomical location.<\/li>\n<\/ul>\n<p>FISH analysis shows <span class=\"term\" data-term-id=\"1850\">translocations<\/span> of myc, Bcl-6 and immunoglobulin H <span class=\"term\" data-term-id=\"775\">genes<\/span>. <\/p>\n<div id=\"square-placement-country-holder\" class=\"country-dependent advert\">\n<div class=\"box-placement square-placement\">\n<div class=\"advert__frame\">\n<div id=\"dermnet-dermnet-mobbox\">\n<\/div><\/div><\/div><\/div>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFCual-es-el-tratamiento-para-el-linfoma-cutaneo-de-celulas-B\"><\/span>What is the treatment for cutaneous B-cell lymphoma?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>The rarity of PCBCLs and the lack of prospective randomized comparative studies limit the choice of therapy, as most treatments are based on data from small retrospective studies. The European Organization for Cutaneous Lymphoma Cancer Research and Treatment Group (EORTC-CLG) and the International Society for Cutaneous Lymphoma (ISCL) have uniform recommendations on the treatment of the three main types of PCBCL. Treatment is also influenced by whether the lesion is unique or limited to a single site.<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Linfoma-cutaneo-primario-del-centro-folicular-4\"><\/span>Primary cutaneous lymphoma of the follicular center<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>Without any treatment, PCFCL lesions may be stable, gradually enlarge, or, rarely, return. the <span class=\"term\" data-term-id=\"474\">histological<\/span> Growth pattern does not influence survival or treatment choice.<\/p>\n<ul>\n<li>If PCFCL presents with a solitary lesion or is confined to a single site, treatment involves <span class=\"term\" data-term-id=\"1326\">radiotherapy<\/span> or surgery. Recurrences of the skin are common and often outside the treated site, but do not affect the prognosis.<\/li>\n<li>Radiation can be observed or administered to patients with multiple injuries, <span class=\"term\" data-term-id=\"1330\">current<\/span> agents, <span class=\"term\" data-term-id=\"1145\">cryotherapy<\/span>, intralesional steroids or systemic therapy.
