It most often causes the combination of:<\/p>\n<ul>\n<li>High <span class=\"term\" data-term-id=\"1176\">fever<\/span>\n<\/li>\n<li><span class=\"term\" data-term-id=\"377\">Morbid<\/span> <span class=\"term\" data-term-id=\"704\">eruption<\/span><\/li>\n<li>\n<span class=\"term\" data-term-id=\"1522\">Hematological<\/span> abnormalities<\/li>\n<li><span class=\"term\" data-term-id=\"503\">Lymphadenopathy<\/span><\/li>\n<li>\n<span class=\"term\" data-term-id=\"148\">Inflammation<\/span> of one or more internal organs<\/li>\n<\/ul>\n<p>Drug hypersensitivity syndrome is also sometimes called a drug reaction with <span class=\"term\" data-term-id=\"83\">eosinophilia<\/span> and <span class=\"term\" data-term-id=\"380\">systemic<\/span> symptoms (DRESS) and drug-induced hypersensitivity syndrome (DIHS).<\/p>\n<p>The syndrome is classified as severe. <span class=\"term\" data-term-id=\"47\">cutaneous<\/span> <span class=\"term\" data-term-id=\"1794\">adverse reaction<\/span> (SCAR). May Have Features Overlapping With Stevens-Johnson Syndrome \/ <span class=\"term\" data-term-id=\"1159\">toxic<\/span> <span class=\"term\" data-term-id=\"846\">epidermal<\/span> necrolysis (SJS \/ TEN) and <span class=\"term\" data-term-id=\"349\">acute<\/span> <span class=\"term\" data-term-id=\"308\">generalized<\/span> <span class=\"term\" data-term-id=\"376\">exanthematous<\/span> <span class=\"term\" data-term-id=\"1130\">pustulosis<\/span> (AGEP)<\/p>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFQuien-contrae-el-sindrome-de-hipersensibilidad-a-medicamentos\"><\/span>Who gets drug hypersensitivity syndrome?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Drug hypersensitivity syndrome is relatively rare. It mainly affects adults and is the same in <span class=\"term\" data-term-id=\"578\">incidence<\/span> in males and females. <span class=\"term\" data-term-id=\"558\">Genetic<\/span> susceptibility and <span class=\"term\" data-term-id=\"1245\">HLA<\/span> associations have been found for several <span class=\"term\" data-term-id=\"1715\">causal drugs<\/span>.<\/p>\n<p>The most common drugs to cause this reaction are a number of antiepileptic drugs (particularly carbamazepine, phenobarbital, and phenytoin), the anti-gout drug, allopurinol, olanzapine, and the sulfonamide group of antibiotics. It has been estimated that at least 1 in 10,000 patients treated with an anticonvulsant will develop drug hypersensitivity syndrome.<\/p>\n<p>The risk of drug hypersensitivity syndrome in patients with allopurinol depends on the dose of allopurinol. It is higher if the patient has kidney disease and is also taking thiazide diuretics.<\/p>\n<p>It has also been rarely reported to be due to other medications. It can be very difficult to determine the exact cause of drug hypersensitivity syndrome if multiple medications have been started in the past weeks. In approximately 10% of cases, the <span class=\"term\" data-term-id=\"1714\">causal drug<\/span> never identifies<\/p>\n<div id=\"square-placement-country-holder\" class=\"country-dependent advert\">\n<div class=\"box-placement square-placement\">\n<div class=\"advert__frame\">\n<div id=\"dermnet-dermnet-mobbox\">\n<\/div><\/div><\/div><\/div>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFQue-causa-el-sindrome-de-hipersensibilidad-a-medicamentos\"><\/span>What causes drug hypersensitivity syndrome?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Drug hypersensitivity syndrome is a delay <span class=\"term\" data-term-id=\"416\">T cell<\/span>-mediated reaction. Tissue damage is due to <span class=\"term\" data-term-id=\"435\">cytotoxic<\/span> <span class=\"term\" data-term-id=\"417\">T cells<\/span> and <span class=\"term\" data-term-id=\"433\">cytokine<\/span> launching.<\/p>\n<ul>\n<li>There&#039;s a <span class=\"term\" data-term-id=\"730\">genetic <span class=\"term\" data-term-id=\"1494\">predisposition<\/span><\/span>  to drug hypersensitivity syndrome.<\/li>\n<li>A defect in the way the liver metabolizes drugs may be responsible.<\/li>\n<li>Reactivation of human herpes virus 6 (HHV6, the cause of roseola) or Epstein Barr virus (EBV) may also be important.<\/li>\n<\/ul>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFCuales-son-las-caracteristicas-clinicas-del-sindrome-de-hipersensibilidad-a-medicamentos\"><\/span>What are the clinical features of drug hypersensitivity syndrome?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Drug hypersensitivity syndrome usually develops over several days, with onset between 2 and 8 weeks after starting the responsible drug.<\/p>\n<p>Usually a high fever of 38\u201340 C is first noted. This is quickly followed by a <span class=\"term\" data-term-id=\"309\">extended<\/span> skin <span class=\"term\" data-term-id=\"255\">eruption<\/span>. The severity of the rash does not necessarily correlate with the degree of involvement of the internal organs. Later symptoms depend on the internal organs affected. They may include:<\/p>\n<ul>\n<li>Enlarged <span class=\"term\" data-term-id=\"1775\">lymph nodes<\/span> multi-site (75%)<\/li>\n<li>Hematological disorders: elevated white count (or less commonly, reduced white count), eosinophilia (in 30% this is&gt; 2.0 x 109 \/ L), <span class=\"term\" data-term-id=\"1395\">atypical<\/span> <span class=\"term\" data-term-id=\"421\">lymphocytes<\/span>, <span class=\"term\" data-term-id=\"623\">thrombocytopenia<\/span>, <span class=\"term\" data-term-id=\"598\">anemia<\/span>, hemophagocytic syndrome<\/li>\n<li>Enlargement of the liver, <span class=\"term\" data-term-id=\"1801\">hepatitis<\/span> and rarely <span class=\"term\" data-term-id=\"1105\">hepatic<\/span> <span class=\"term\" data-term-id=\"403\">necrosis<\/span> with liver failure Abnormal liver function tests are found in 70 to 90%<\/li>\n<li>Kidney disease: affects approximately 10% and is usually mild (<span class=\"term\" data-term-id=\"750\">interstitial<\/span> <span class=\"term\" data-term-id=\"994\">nephritis<\/span>) <span class=\"term\" data-term-id=\"896\">Renal<\/span> failure is rare<\/li>\n<li>Inflammation of the heart (<span class=\"term\" data-term-id=\"1049\">myocarditis<\/span>) or cardiac sac (<span class=\"term\" data-term-id=\"1055\">pericarditis<\/span>), which causes chest pain, dyspnea, and low blood pressure<\/li>\n<li>Lung disease, causing shortness of breath and coughing (interstitial <span class=\"term\" data-term-id=\"1051\">pneumonitis<\/span>, pleurisy, pneumonia and acute respiratory distress syndrome)<\/li>\n<li>\n<span class=\"term\" data-term-id=\"402\">Neurological <\/span>participation, which can lead to <span class=\"term\" data-term-id=\"1053\">meningitis<\/span> Y, <span class=\"term\" data-term-id=\"716\">encephalitis<\/span>, polyneuritis, which causes headache, seizures, coma, and paralysis<\/li>\n<li>Gastrointestinal symptoms: <span class=\"term\" data-term-id=\"1052\">stomach flu<\/span>, <span class=\"term\" data-term-id=\"886\">pancreatitis<\/span>, bleeding and dehydration. In severe, acute cases <span class=\"term\" data-term-id=\"993\">colitis<\/span> and pancreatitis may occur, and <span class=\"term\" data-term-id=\"319\">chronic<\/span> enteropathy may occur<\/li>\n<li>\n<span class=\"term\" data-term-id=\"1196\">Endocrine<\/span> abnormalities: thyroiditis and diabetes<\/li>\n<li>Muscle disease<span class=\"term\" data-term-id=\"1802\">myositis<\/span>)<\/li>\n<li>Eye disease (<span class=\"term\" data-term-id=\"505\">uveitis<\/span>)<\/li>\n<\/ul>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFCuales-son-las-complicaciones-del-sindrome-de-hipersensibilidad-a-medicamentos\"><\/span>What are the complications of drug hypersensitivity syndrome?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Mortality from drug hypersensitivity syndrome is estimated to be around 8%. Causes of death include:<\/p>\n<ul>\n<li>Acute liver failure causes <span class=\"term\" data-term-id=\"604\">coagulation<\/span> problems, <span class=\"term\" data-term-id=\"732\">jaundice<\/span> and altered consciousness<\/li>\n<li>Multi-organic failure<\/li>\n<li>Fulminant myocarditis<\/li>\n<li>Hemophagocytosis<\/li>\n<\/ul>\n<div id=\"square-placement-country-holder\" class=\"country-dependent advert\">\n<div class=\"box-placement square-placement\">\n<div class=\"advert__frame\">\n<div id=\"dermnet-dermnet-mobbox\">\n<\/div><\/div><\/div><\/div>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFComo-se-diagnostica-el-sindrome-de-hipersensibilidad-a-medicamentos\"><\/span>How is drug hypersensitivity syndrome diagnosed?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>The diagnosis of drug hypersensitivity syndrome is based on the clinical presentation of the triad of:<\/p>\n<ul>\n<li>High fever<\/li>\n<li>Extensive skin rash<\/li>\n<li>Organ compromise<\/li>\n<\/ul>\n<p>It is supported by eosinophilia and abnormal liver function tests.<\/p>\n<p>As drug hypersensitivity syndrome can occur up to eight weeks after the first exposure to the responsible drug, a great deal of care is required in determining the responsible drug. A <span class=\"term\" data-term-id=\"1317\">temporary<\/span> The association between the use of medications and the onset of the syndrome is the strongest evidence.<\/p>\n<p>The European Severe Skin Registry <span class=\"term\" data-term-id=\"1333\">Adverse reactions<\/span> Drug and Biological Sample Collection (RegiSCAR) has produced diagnostic criteria to aid in the diagnosis of drug hypersensitivity syndrome. RegiSCAR&#039;s inclusion criteria for potential cases require at least 3 of the following:<\/p>\n<ul>\n<li>Hospitalization<\/li>\n<li>Reaction suspected of being related to drugs<\/li>\n<li>Acute skin rash<\/li>\n<li>Fever above 38 C<\/li>\n<li>Enlarged lymph nodes at two sites<\/li>\n<li>Involvement of at least one internal organ<\/li>\n<li>Blood count abnormalities such as low <span class=\"term\" data-term-id=\"616\">platelets<\/span>raised <span class=\"term\" data-term-id=\"448\">eosinophils<\/span> or abnormal <span class=\"term\" data-term-id=\"420\">lymphocyte<\/span> tell<\/li>\n<\/ul>\n<p>Attempts to confirm which drug has caused the drug hypersensitivity syndrome may include <span class=\"term\" data-term-id=\"224\">patch<\/span> testing Patch testing has been reported to be most successful for antiepileptic drugs, with a 50% of positive reactions. Not useful for allopurinol, with 0% of positive reactions. It is very important that patients with high fever and rash, where it is considered a diagnosis of drug hypersensitivity syndrome, have blood tests as soon as possible.<\/p>\n<p>Systemic steroids (eg, Prednisone) are generally used in the most severe cases of drug hypersensitivity syndrome involving significant exfoliative dermatitis, pneumonitis, and \/ or hepatitis. However, the benefits of corticosteroids are unknown as controlled clinical trials are lacking. Once effective, they must be withdrawn very slowly, as the syndrome may recur as the dose is reduced.<\/p>\n<p>Cyclosporine has been reported to be an effective alternative treatment for drug hypersensitivity syndrome.<\/p>\n<p>Additional treatment may include intravenous <span class=\"term\" data-term-id=\"427\">immunoglobulin<\/span>, plasmapheresis and <span class=\"term\" data-term-id=\"680\">immunomodulator<\/span> medications such as cyclophosphamide, mycophenolate, and rituximab.<\/p>\n<p>Supportive treatment for the rash may include:<\/p>\n<ul>\n<li>Dressings<\/li>\n<li><span class=\"term\" data-term-id=\"1330\">Current<\/span> corticosteroids<\/li>\n<li><span class=\"term\" data-term-id=\"1888\">Emollients<\/span><\/li>\n<li>Oral antihistamines.<\/li>\n<\/ul>\n<p>Fluid, electrolyte, and calorie intake may need attention. A warm environment and skilled nursing care are required.
