The severe <span class=\"term\" data-term-id=\"490\">inflammatory<\/span> the reaction resembles a <span class=\"term\" data-term-id=\"349\">acute<\/span> serious <span class=\"term\" data-term-id=\"380\">systemic<\/span> <span class=\"term\" data-term-id=\"145\">infection<\/span> or bone infection. If recognized early, treatment with anakinra, a biological agent, can prevent death from multiple organ failure.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFQuien-recibe-DIRA-y-por-que\"><\/span>Who receives DIRA and why?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>DIRA has been diagnosed only in a very small number of children. It occurs at birth or in the first days of life. Cases of families originating from Puerto Rico, Newfoundland (Canada), the Netherlands and Lebanon have been identified.<\/p>\n<p>DIRA is a <span class=\"term\" data-term-id=\"828\">autosomal<\/span> recessive genetic disorder, that is, each child of two <span class=\"term\" data-term-id=\"1905\">carrier<\/span> Parents have a 25% chance of DIRA.<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Molecular-biologia-y-genetica\"><\/span>\n<span class=\"term\" data-term-id=\"1851\">Molecular<\/span> biology and genetics<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>DIRA is due to <span class=\"term\" data-term-id=\"779\">homozygous<\/span> point <span class=\"term\" data-term-id=\"871\">mutations<\/span> at <span class=\"term\" data-term-id=\"774\">gene<\/span> IL1RN (2q14.2) or a large genomic deletion that affects this gene and others in the IL family in <span class=\"term\" data-term-id=\"1271\">chromosome<\/span> 2. the same <span class=\"term\" data-term-id=\"870\">mutation<\/span> it is detected in all patients originating from a certain area, suggesting a different founder effect in each region. Normal control studies from the same areas found carrier rates for the 0.2% mutation in Newfoundland and 1.3% in Puerto Rica. These mutations, when homozygous, result in the complete absence of functional interleukin-1 receptor antagonist protein. Therefore, interleukin-1 has unopposed effects on the receptor. Interleukin-1 receptor stimulation is a potent activator of inflammatory pathways.<\/p>\n<div id=\"square-placement-country-holder\" class=\"country-dependent advert\">\n<div class=\"box-placement square-placement\">\n<div class=\"advert__frame\">\n<div id=\"dermnet-dermnet-mobbox\">\n<\/div><\/div><\/div><\/div>\n<h2><span class=\"ez-toc-section\" id=\"Caracteristicas-clinicas-de-DIRA\"><\/span>Clinical characteristics of DIRA<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>The distinctive features of this syndrome are present at birth, or within days of birth, and are due to inflammatory changes in the skin and bones in the absence of <span class=\"term\" data-term-id=\"1176\">fever<\/span>.<\/p>\n<p>In the first days of life, common presentation features include:<\/p>\n<ul>\n<li>Fetal suffering<\/li>\n<li>\n<span class=\"term\" data-term-id=\"792\">Pustular<\/span> <span class=\"term\" data-term-id=\"255\">eruption<\/span>\n<\/li>\n<li>Oral lesions<\/li>\n<li>Joint inflammation<\/li>\n<li>Painful movement<\/li>\n<li>Enlarged liver and spleen (<span class=\"term\" data-term-id=\"1047\">hepatosplenomegaly<\/span>)<\/li>\n<\/ul>\n<p>All affected children develop a pustular rash, in which there may be <span class=\"term\" data-term-id=\"66\">discreet<\/span> crops of <span class=\"term\" data-term-id=\"1016\">pustules<\/span> or a severe <span class=\"term\" data-term-id=\"308\">generalized<\/span> pustular <span class=\"term\" data-term-id=\"704\">eruption<\/span> It may resemble generalized pustular psoriasis.<\/p>\n<p>Other skin \/<span class=\"term\" data-term-id=\"1018\">mucous membrane<\/span> reported changes included:<\/p>\n<ul>\n<li>Generalized <span class=\"term\" data-term-id=\"398\">ichthyosis<\/span>-like changes<\/li>\n<li>Nail changes <span class=\"term\" data-term-id=\"233\">bites<\/span>, separation of the nail from the nail bed, such as nail psoriasis\n<\/li>\n<li>\n<span class=\"term\" data-term-id=\"272\">Stomatitis<\/span> - can blister<\/li>\n<li>Mouth <span class=\"term\" data-term-id=\"1279\">ulcers<\/span><\/li>\n<li>\n<span class=\"term\" data-term-id=\"1898\">Conjunctivitis<\/span> (eye pain)<\/li>\n<li>\n<span class=\"term\" data-term-id=\"1626\">Pyoderma<\/span> gangrenous (one case)<\/li>\n<\/ul>\n<p>All children develop <span class=\"term\" data-term-id=\"148\">inflammation<\/span> in the bones, causing painful movements and swelling of the joints. It had recently been made available to treat rheumatoid. <span class=\"term\" data-term-id=\"1578\">arthritis<\/span> when DIRA was first described and characterized in detail.<\/p>\n<p>Diary <span class=\"term\" data-term-id=\"900\">subcutaneous<\/span> Administration of anakinra replaces the missing protein with an excellent response, particularly in those children with point mutations. In the initial reported case series, complete and rapid resolution of symptoms and signs occurred. The skin changes responded in a few days and the bone radiographs normalized in months. Blood markers of inflammation also returned to normal levels. The response in children with large genomic deletion was less dramatic since blood markers did not normalize. The anakinra dose was started at 1 mg \/ kg \/ day, and increased by 0.5 mg \/ kg \/ day until the response was achieved. Resolution of symptoms and signs has been reported to be long-lasting, with rapid deterioration within 36 hours after discontinuation of anakinra treatment. Therefore, the treatment will be for life. Local reactions at the injection site (redness, itching, swelling) are a common adverse effect, but treatment is well tolerated. Long-term monitoring is not yet available.<\/p>\n<p>Note: Anakinra is not registered or subsidized in New Zealand.
