data-term-id=\"380\">systemic<\/span> mycosis?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Systemic mycoses are fungal infections that affect internal organs. In the right circumstances, fungi enter the body through the lungs, through the intestine, the sinuses, or the skin. Fungi can spread through the bloodstream to multiple organs, including the skin, often causing multiple organ failure and ultimately resulting in the death of the patient.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFQue-causa-las-micosis-sistemicas\"><\/span>What Causes Systemic Mycoses?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Patients who are <span class=\"term\" data-term-id=\"1695\">immunocompromised<\/span> they are more predisposed to systemic mycoses, but can develop in healthy patients. Systemic mycoses can be divided into two main varieties, <span class=\"term\" data-term-id=\"814\">endemic<\/span> respiratory infections and opportunistic infections.<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Infecciones-respiratorias-endemicas\"><\/span>Endemic respiratory infections<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>Fungi that can cause systemic <span class=\"term\" data-term-id=\"145\">infection<\/span> in people with normal immune function, as well as in immunocompromised people, they include:<\/p>\n<ul>\n<li>\n<em>Histoplasma capsulatum<\/em> (causing histoplasmosis)<\/li>\n<li>\n<em>Coccidioides immitis<\/em> (causing coccidioidomycosis)<\/li>\n<li>\n<em>Blastomyces dermatitidis<\/em> (causing blastomycosis)<\/li>\n<li>\n<em>Paracoccidioides brasiliensis<\/em> (causing paracoccidioidomycosis)<\/li>\n<li>\n<em>Penicillium marneffei<\/em> (causing penicilliosis).<\/li>\n<\/ul>\n<p>These fungi are found in soil and wood waste. Histoplasma is <span class=\"term\" data-term-id=\"1676\">predominant<\/span> in the southern United States, Central America, South America, Africa, and Asia. Coccidioides and paracoccidioides are common in the southern United States and Central America. Blastomyces is most commonly found in North America.<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Infecciones-oportunistas\"><\/span>Opportunistic infections<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>Fungi that only result in systemic infection in immunocompromised or diseased people include:<\/p>\n<ul>\n<li>\n<em>Candida<\/em> species including non-albicans candida (which causes yeast infection)<\/li>\n<li>\n<em>Aspergillus<\/em> species (causing aspergillosis)<\/li>\n<li>\n<em>Cryptococcus<\/em> (causing cryptococcosis)<\/li>\n<li>\n<em>Zygomycetes<\/em> (causing zygomycosis).<\/li>\n<\/ul>\n<p>These fungi are found in or on normal skin, decaying plant matter, and bird droppings respectively, but not exclusively. They are present all over the world.<\/p>\n<div id=\"square-placement-country-holder\" class=\"country-dependent advert\">\n<div class=\"box-placement square-placement\">\n<div class=\"advert__frame\">\n<div id=\"dermnet-dermnet-mobbox\">\n<\/div><\/div><\/div><\/div>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFQuien-esta-en-riesgo-de-micosis-sistemicas\"><\/span>Who is at risk for systemic mycoses?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p><span class=\"term\" data-term-id=\"1696\">Compromised immune<\/span> people are at risk for systemic mycoses. Immunodeficiency can result from:<\/p>\n<ul>\n<li>  Human immunodeficiency virus (<span class=\"term\" data-term-id=\"127\">HIV<\/span>) infection <\/li>\n<li>Systemic <span class=\"term\" data-term-id=\"1742\">malignancy<\/span> (<span class=\"term\" data-term-id=\"30\">Cancer<\/span>)<\/li>\n<li>\n<span class=\"term\" data-term-id=\"1814\">Neutropenia<\/span> (low white blood cell count)<\/li>\n<li>Organ <span class=\"term\" data-term-id=\"1792\">transplant<\/span> recipients<\/li>\n<li>After major surgery<\/li>\n<li>Poorly controlled diabetes mellitus<\/li>\n<li>Very old or very young.<\/li>\n<\/ul>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFCuales-son-las-caracteristicas-clinicas-de-las-micosis-sistemicas\"><\/span>What are the clinical features of systemic mycoses?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>The clinical features of a systemic mycosis depend on the specific infection and the affected organs. Infections in people with normal immune function can cause very mild or no symptoms (this is called a subclinical infection). General symptoms of the disease can include:<\/p>\n<ul>\n<li><span class=\"term\" data-term-id=\"1176\">Fever<\/span><\/li>\n<li>Cough<\/li>\n<li>Loss of appetite<\/li>\n<\/ul>\n<h3><span class=\"ez-toc-section\" id=\"Livianos\"><\/span>Light<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p><span class=\"term\" data-term-id=\"612\">Invader<\/span> Pulmonary mycoses usually lead to a <span class=\"term\" data-term-id=\"1759\">progressive<\/span> dry cough, shortness of breath, pain when taking deep breaths, and fever. These symptoms can progress to the point of being life-threatening. <span class=\"term\" data-term-id=\"349\">acute<\/span> respiratory distress <span class=\"term\" data-term-id=\"350\">syndrome<\/span>. Hemoptysis (coughing up blood) is also seen sometimes, especially if there is <span class=\"term\" data-term-id=\"148\">inflammation<\/span> of the great airways is present.<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Hueso\"><\/span>Bone<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>Bone infection can develop by spread through the blood or rarely by direct spread of <span class=\"term\" data-term-id=\"290\">ulcer<\/span> and infected skin. Fever and pain in the affected bone are cardinal symptoms. This would generally be investigated with X-rays, <span class=\"term\" data-term-id=\"797\">computed tomography<\/span> (<span class=\"term\" data-term-id=\"799\">Connecticut<\/span>) scan and a <span class=\"term\" data-term-id=\"1867\">aspiration<\/span> through a needle to obtain a sample for <span class=\"term\" data-term-id=\"837\">culture<\/span>.<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Cerebro\"><\/span>Brain<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>Brain involvement by systemic mycoses has a particularly high mortality. Symptoms that suggest brain involvement include headaches, seizures, and deficits in the normal brain. <span class=\"term\" data-term-id=\"1854\">control<\/span> about movement or sensation. These are evaluated by CT or <span class=\"term\" data-term-id=\"1410\">magnetic resonance image<\/span> (<span class=\"term\" data-term-id=\"1116\">Magnetic resonance<\/span>) brain scans and, if suggestive, may be followed by <span class=\"term\" data-term-id=\"24\">biopsy<\/span> of the brain<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Meningitis\"><\/span><span class=\"term\" data-term-id=\"1053\">Meningitis<\/span><span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>Inflammation of the tissue surrounding the brain can develop, particularly with candidiasis and cryptococcosis. Symptoms generally include headache, stiff neck, and irritability.<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Ojos\"><\/span>Eyes<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>Almost any of the ocular structures can be infected by mycosis. <em>Candida spp.<\/em> and <em>Cryptococcus spp.<\/em> they are the most common infectious mycoses. Symptoms depend on which part of the eye is infected, but can include blurred images, dark or black images floating across the visual field, pain, and red eyes.<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Piel\"><\/span>Skin<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>A variety of skin changes can be observed in association with systemic mycoses. Blood can also be cultured.<\/p>\n<div id=\"square-placement-country-holder\" class=\"country-dependent advert\">\n<div class=\"box-placement square-placement\">\n<div class=\"advert__frame\">\n<div id=\"dermnet-dermnet-mobbox\">\n<\/div><\/div><\/div><\/div>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFQue-tratamiento-hay-disponible-para-las-micosis-sistemicas\"><\/span>What treatment is available for systemic mycoses?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Once a diagnosis of systemic mycoses has been established, the immune compromise should be reversed if possible, for example, by discontinuing immunosuppressive medications or at least reducing their dose.<\/p>\n<p>Systemic antifungal medications may also be given. Suitable medications include:<\/p>\n<ul>\n<li>Amphotericin B<\/li>\n<li>  Fluconazole <\/li>\n<li>  Itraconazole <\/li>\n<li>  Voriconazole <\/li>\n<li>Caspofungin<\/li>\n<\/ul>\n<p>the <span class=\"term\" data-term-id=\"241\">forecast<\/span> it depends on the immune function of the patient and the degree of infection when treatment is started.
