Emboli can be composed of:<\/p>\n<ul>\n<li>\n<span class=\"term\" data-term-id=\"1507\">Cholesterol<\/span> Dislodged from the heart (80%) or a large artery (aorta, iliac vessel)<\/li>\n<li><span class=\"term\" data-term-id=\"289\">Tumor<\/span><\/li>\n<li>Infectious particles<\/li>\n<li>Fat shed during an operation<\/li>\n<li>Strange objects.<\/li>\n<\/ul>\n<p>Large emboli lodge in the bifurcation of the femoral artery (43%), the iliac arteries (18%), the aorta (15%), and the <span class=\"term\" data-term-id=\"944\">popliteal<\/span> arteries (15%). The little ones are deposited on the fingers and toes, palms and soles of the feet.<\/p>\n<p>Vascular obstruction may also be due to <span class=\"term\" data-term-id=\"1383\">hyperviscosity<\/span> syndromes or abnormal circulating proteins such as cryoglobulins or macroglobulins.<\/p>\n<h3><span class=\"ez-toc-section\" id=\"Trombosis\"><\/span>Thrombosis<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>Thrombosis is a blood clot that arises within a <span class=\"term\" data-term-id=\"1068\">blood vessel<\/span>. Predisposing factors include:<\/p>\n<ul>\n<li><span class=\"term\" data-term-id=\"263\"><span class=\"term\" data-term-id=\"1165\">Septicemia<\/span><\/span><\/li>\n<li>\n<span class=\"term\" data-term-id=\"626\">Hypotension<\/span> (low blood pressure)<\/li>\n<li>Low cardiac output<\/li>\n<li>An aneurysm (a bulge in the wall of a weakened artery)<\/li>\n<li>\n<span class=\"term\" data-term-id=\"1569\">Aortic<\/span> dissection (a division in the wall of the aorta)<\/li>\n<li>Bypass graft (a surgical procedure)<\/li>\n<li>\n<span class=\"term\" data-term-id=\"1385\">Atherosclerotic<\/span> narrowing of the artery.<\/li>\n<\/ul>\n<p>Whatever the <span class=\"term\" data-term-id=\"949\">primary<\/span> Cause of ischemia, thrombosis due to slow blood flow can make it worse.<\/p>\n<p>Chronic ischemia can be due to:<\/p>\n<ul>\n<li>Peripheral arterial disease<\/li>\n<li>\n<span class=\"term\" data-term-id=\"1178\">Venous<\/span> insufficiency.<\/li>\n<\/ul>\n<h3><span class=\"ez-toc-section\" id=\"Enfermedad-arterial-periferica\"><\/span>Peripheral arterial disease<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>Chronic peripheral arterial disease is most often due to <span class=\"term\" data-term-id=\"1384\">atherosclerosis<\/span>. This is a buildup of cholesterol, <span class=\"term\" data-term-id=\"850\">fibrin<\/span> and other proteins within the arterial wall, causing the vessel to narrow and eventually become completely blocked. This process is also called obliterative arteriosclerosis.<\/p>\n<p>Risk factors for peripheral arterial disease include:<\/p>\n<ul>\n<li>\n<span class=\"term\" data-term-id=\"561\">Hypertension<\/span> (hypertension)<\/li>\n<li>Hyperlipidemia (high fats in the blood)<\/li>\n<li>Diabetes<\/li>\n<li>Cardiovascular disease including myocardial infarction, atrial fibrillation, valve disease<\/li>\n<li>\n<span class=\"term\" data-term-id=\"1848\">Cerebrovascular<\/span> disease including stroke, transient <span class=\"term\" data-term-id=\"765\">ischemic<\/span> attack<\/li>\n<li>Of smoking<\/li>\n<li>\n<span class=\"term\" data-term-id=\"691\">Autoimmune<\/span> diseases: <span class=\"term\" data-term-id=\"1091\">vasculitis<\/span>, <span class=\"term\" data-term-id=\"1578\">arthritis<\/span>coagulopathy<\/li>\n<li>\n<span class=\"term\" data-term-id=\"1742\">Malignancy<\/span> (<span class=\"term\" data-term-id=\"30\">Cancer<\/span>)<\/li>\n<\/ul>\n<p>Peripheral arterial disease can also be caused by:<\/p>\n<ul>\n<li>\n<span class=\"term\" data-term-id=\"43\">Connective tissue<\/span> disease, including <span class=\"term\" data-term-id=\"380\">systemic<\/span> <span class=\"term\" data-term-id=\"260\">sclerosis<\/span>\n<\/li>\n<li>\n<p>Buerger&#039;s disease (obliterative thromboangitis).<\/li>\n<\/ul>\n<h3><span class=\"ez-toc-section\" id=\"Insuficiencia-venosa\"><\/span>Venous insufficiency<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>Veins may fail to remove blood from tissues due to valve <span class=\"term\" data-term-id=\"1253\">dysfunction<\/span> (varicose veins) and obstruction by deep vein thrombosis (DVT) or thrombophlebitis. High pressure within the veins (venous hypertension) results in the movement of proteins and fibrin into the <span class=\"term\" data-term-id=\"1183\">Soft fabric<\/span>. This leads to <span class=\"term\" data-term-id=\"102\">fibrosis<\/span>, <span class=\"term\" data-term-id=\"1254\">dysfunctional<\/span> <span class=\"term\" data-term-id=\"756\">capillary<\/span> fat formation and necrosis (lipodermatosclerosis). When severe, these processes can cause ischemia and gangrene.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"Caracteristicas-clinicas-de-la-obstruccion-vascular\"><\/span>Clinical characteristics of vascular obstruction.<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Signs of peripheral vascular disease depend on which tissues are ischemic and their severity. A patient may present with:<\/p>\n<ul>\n<li>\n<span class=\"term\" data-term-id=\"1283\">Claudication<\/span> (intermittent pain in the leg)<\/li>\n<li>Pain at rest<\/li>\n<li>Shiny, thin, dry, pale and cold skin.<\/li>\n<li>Fragile <span class=\"term\" data-term-id=\"193\">nail<\/span>\n<\/li>\n<li>The most advanced disease can be revealed by <span class=\"term\" data-term-id=\"1349\">livedo<\/span> reticularis (network appearance)<\/li>\n<li>Bad healing and <span class=\"term\" data-term-id=\"1278\">ulceration<\/span> (most often leg ulcers)<\/li>\n<li>Gangrene: an area of gray or black <span class=\"term\" data-term-id=\"770\">necrotic<\/span> (dead) tissue.<\/li>\n<\/ul>\n<h2><span class=\"ez-toc-section\" id=\"Investigaciones-en-un-paciente-con-isquemia-o-gangrena-seca\"><\/span>Investigations in a patient with ischemia or dry gangrene.<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>When a patient has peripheral ischemia or gangrene of unknown cause, a complete physical examination is performed to evaluate the vascular system, including the heart and peripheral pulses. An electrocardiogram (ECG) assesses heart function.
