<\/span><\/h2>\nErythrodermic psoriasis usually occurs in the setting of known worsening or unstable psoriasis, but it can be the first presentation of psoriasis. Onset can occur acutely over a few days or weeks, or progress gradually over several months from pre-existing psoriasis.<\/p>\n
The characteristics are those of any form of <\/span>erythroderma (red and dry skin all over the body). <\/span><\/p>\nComplications include:<\/p>\n
\n- Dehydration<\/li>\n
- Heart failure<\/li>\n
- Infection<\/span><\/li>\n
- Hypothermia<\/li>\n
- Protein loss and malnutrition.\n<\/li>\n
- \nEdema<\/span> (swelling), particularly of the legs<\/li>\n
- Death<\/li>\n<\/ul>\n
<\/span>What is the treatment for erythrodermic psoriasis?<\/span><\/h2>\nTreating erythrodermic psoriasis can be difficult. Management includes:<\/p>\n
\n- Hospitalization for supportive care, including IV fluids and temperature regulation.<\/li>\n
- Nice emollients<\/span> and cool wet dressings<\/li>\n
- Bed rest<\/li>\n
- Treatment of complications (eg, antibiotics, diuretics (water tablets), nutritional support)<\/li>\n
- Low-dose methotrexate, cyclosporine, or acitretin\n<\/li>\n
- Individual cases have been reported to have successful results with biological agents, including the TNF-alpha inhibitors adalimumab, etanercept, and infliximab, and the IL12 \/ 23 inhibitor, ustekinumab.<\/li>\n<\/ul>\n
Oral corticosteroids should be avoided if possible because withdrawal runs the risk of worsening erythrodermic status and may cause generalization pustular<\/span> psoriasis. However, sometimes they are the only treatment that helps.<\/p>\nTopical tar preparations and phototherapy should also be avoided in the early treatment of erythrodermic psoriasis, as they can make the condition worse.<\/p>\n<\/section>","protected":false},"excerpt":{"rendered":"
