data-term-id=\"1742\">malignancy<\/span>asociada a la enfermedad de la piel caracterizada por un <span class=\"term\" data-term-id=\"492\">eczematous<\/span> <span class=\"term\" data-term-id=\"255\">eruption<\/span> of the <span class=\"term\" data-term-id=\"825\">anogenital<\/span> regi\u00f3n y <span class=\"term\" data-term-id=\"1004\">vulva<\/span>, donde hay abundancia de <span class=\"term\" data-term-id=\"12\">apocrine glands<\/span>. <span class=\"term\" data-term-id=\"949\">Primary<\/span> <span class=\"term\" data-term-id=\"47\">cutaneous<\/span> existen formas asociadas con subyacente <span class=\"term\" data-term-id=\"1827\">attached<\/span> <span class=\"term\" data-term-id=\"32\">carcinoma<\/span>, adem\u00e1s de las formas asociadas con el adenocarcinoma anal, rectal o vesical subyacente. La enfermedad de Paget extramamaria puede preceder a la neoplasia maligna asociada en hasta 15 a\u00f1os.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"Histologia-de-la-enfermedad-de-Paget-extramamaria\"><\/span>\n<span class=\"term\" data-term-id=\"128\">Histology<\/span> de la enfermedad de Paget extramamaria<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Paget <span class=\"term\" data-term-id=\"289\">tumor<\/span> las c\u00e9lulas se encuentran en el <span class=\"term\" data-term-id=\"85\">epidermis<\/span> (Figura 1-3). En raras ocasiones, las c\u00e9lulas tumorales pueden extenderse al <span class=\"term\" data-term-id=\"61\">dermis<\/span>. En lesiones anteriores, las c\u00e9lulas de Paget se encuentran cerca de la membrana basal y se extienden para llenar toda la epidermis a medida que avanza la afecci\u00f3n. Las c\u00e9lulas tumorales son grandes con clara o <span class=\"term\" data-term-id=\"449\">eosinophilic<\/span> <span class=\"term\" data-term-id=\"838\">cytoplasm<\/span>, grande <span class=\"term\" data-term-id=\"1540\">pleomorphic<\/span> <span class=\"term\" data-term-id=\"729\">nuclei<\/span> and occasional <span class=\"term\" data-term-id=\"1535\">mitosis<\/span> (Figura 3). <span class=\"term\" data-term-id=\"610\">Hyperkeratosis<\/span> and <span class=\"term\" data-term-id=\"218\">parakeratosis<\/span> puede superponerse <span class=\"term\" data-term-id=\"1449\">aggregates<\/span> de c\u00e9lulas tumorales (Figura 2). <span class=\"term\" data-term-id=\"136\">Hyperplasia<\/span> de la epidermis puede ser <span class=\"term\" data-term-id=\"270\">scaly<\/span>, <span class=\"term\" data-term-id=\"216\">papillomatous<\/span> or <span class=\"term\" data-term-id=\"1652\">fibroepithelioma<\/span> de Pinkus-like. <span class=\"term\" data-term-id=\"1278\">Ulceration<\/span> and <span class=\"term\" data-term-id=\"1521\">pigmentation<\/span> may be present. <span class=\"term\" data-term-id=\"52\">Dermal<\/span> los cambios incluyen un <span class=\"term\" data-term-id=\"319\">chronic<\/span> <span class=\"term\" data-term-id=\"490\">inflammatory<\/span> <span class=\"term\" data-term-id=\"541\">infiltrate<\/span> (Figura 1, 2). Inmunohistoqu\u00edmicamente, las c\u00e9lulas de Paget se ti\u00f1en para <span class=\"term\" data-term-id=\"847\">epithelial<\/span> membrane <span class=\"term\" data-term-id=\"410\">antigen<\/span> (EMA), bajo <span class=\"term\" data-term-id=\"1851\">molecular<\/span> peso <span class=\"term\" data-term-id=\"1613\">citoqueratinas<\/span> (CK7 +) y CD23 con positividad variable para el ant\u00edgeno carcinoembrionario (CEA). Las citoqueratinas de alto peso molecular ti\u00f1en la epidermis circundante y resaltan las c\u00e9lulas negativas de Paget.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"Diagnostico-diferencial-de-la-enfermedad-de-Paget-extramamaria\"><\/span>\n<span class=\"term\" data-term-id=\"813\">Differential diagnosis<\/span> de la enfermedad de Paget extramamaria<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Enfermedad mamaria de Paget de la piel: esta afecci\u00f3n es cut\u00e1nea <span class=\"term\" data-term-id=\"164\">injury<\/span> que surge de un ductal subyacente <span class=\"term\" data-term-id=\"1077\">in the place<\/span> carcinoma de mama. La enfermedad de Paget extramamaria carece de la expresi\u00f3n de Her-2 \/ neu observada en la enfermedad de Paget mamaria. Los tumores de mama y los tumores que se originan de las gl\u00e1ndulas apocrinas se ti\u00f1en con GCDFP-15.<\/p>\n<p><span class=\"term\" data-term-id=\"1670\">Melanoma<\/span> <span class=\"term\" data-term-id=\"863\">in the place<\/span> &#8211; en contraste con Paget, el melanoma es negativo para las citoqueratinas, EMA y CEA.
