swelling, <span class=\"term\" data-term-id=\"366\">papules<\/span> me <span class=\"term\" data-term-id=\"1016\">pustules<\/span>. Ocular rosacea is a form of rosacea that involves the eyelids and the front of the eye. Ocular rosacea includes:<\/p>\n<ul>\n<li>Blepharitis<\/li>\n<li>Conjunctival hyperemia<\/li>\n<li>Associated with rosacea <span class=\"term\" data-term-id=\"1033\">keratitis<\/span><\/li>\n<\/ul>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFQuien-contrae-rosacea-ocular\"><\/span>Who gets ocular rosacea?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Ocular rosacea affects adult men and women equally, with one study reporting a median age of presentation of 56 years. It is rare in children and usually begins after age 30.<\/p>\n<p>It usually occurs in patients with existing rosacea, but may be the first <span class=\"term\" data-term-id=\"899\">sign<\/span> of the disease Ocular rosacea tends to occur in patients with facial redness (1).<\/p>\n<div id=\"square-placement-country-holder\" class=\"country-dependent advert\">\n<div class=\"box-placement square-placement\">\n<div class=\"advert__frame\">\n<div id=\"dermnet-dermnet-mobbox\"><\/div>\n<\/div>\n<\/div>\n<\/div>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFCual-es-la-causa-de-la-rosacea-ocular\"><\/span>What is the cause of ocular rosacea?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>The exact cause of ocular rosacea is unknown. However, <span class=\"term\" data-term-id=\"464\">immune <\/span>micro-factors<span class=\"term\" data-term-id=\"883\">organisms<\/span> on the surface of the skin, and reactive <span class=\"term\" data-term-id=\"953\">blood vessels<\/span> are involved (1).<\/p>\n<ul>\n<li>Demodex <span class=\"term\" data-term-id=\"1312\">Mites<\/span>, normal inhabitants of eyelashes <span class=\"term\" data-term-id=\"1301\">follicles<\/span>, can stimulate <span class=\"term\" data-term-id=\"148\">inflammation<\/span> in ocular rosacea and <span class=\"term\" data-term-id=\"10\">previous<\/span> blepharitis<\/li>\n<li><span class=\"term\" data-term-id=\"935\">The bacteria<\/span> may play a role, as ocular rosacea improves with antibiotics. One theory is that <span class=\"term\" data-term-id=\"937\">bacterial<\/span> lipase release <span class=\"term\" data-term-id=\"1159\">toxic<\/span> free fatty acids and glycerides <span class=\"term\" data-term-id=\"1469\">lipids<\/span> secreted by meibomina glands.<\/li>\n<li>Pro-<span class=\"term\" data-term-id=\"490\">inflammatory<\/span> <span class=\"term\" data-term-id=\"433\">cytokine<\/span> <span class=\"term\" data-term-id=\"915\">interleukin<\/span> 1-alpha (IL-1\u03b1) increases metallomatrix protein-9 (MMP-9) in tear fluid. Upregulation of MMP-9 damages ocular tissues.<\/li>\n<\/ul>\n<p>Blockage of the meibomian glands changes the composition of the tear film and leads to:<\/p>\n<ul>\n<li>Reduced tear film <span class=\"term\" data-term-id=\"1468\">lipid<\/span> cap<\/li>\n<li>Tear film instability<\/li>\n<li>Tear hyperosmolarity<\/li>\n<\/ul>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFCuales-son-los-sintomas-de-la-rosacea-ocular\"><\/span>What are the symptoms of ocular rosacea?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Ocular rosacea mainly affects the eyelids, <span class=\"term\" data-term-id=\"574\">conjunctiva<\/span> and <span class=\"term\" data-term-id=\"1032\">cornea<\/span>. Rarely, it may involve the iris and <span class=\"term\" data-term-id=\"596\">sclerotic<\/span>. Symptoms may include:<\/p>\n<ul>\n<li>itching<\/li>\n<li>Burning and pain<\/li>\n<li>Increased tears<\/li>\n<li><span class=\"term\" data-term-id=\"973\">Photophobia<\/span> (sensitivity to light)<\/li>\n<li>Foreign body sensation.<\/li>\n<li>Dry Eye<\/li>\n<li>inflamed papules<\/li>\n<li>Corneal <span class=\"term\" data-term-id=\"1279\">ulcers<\/span><\/li>\n<li>Red eye<\/li>\n<\/ul>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFCuales-son-los-signos-clinicos-de-la-rosacea-ocular\"><\/span>What are the clinical signs of ocular rosacea?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<h3><span class=\"ez-toc-section\" id=\"Parpado-y-conjuntiva\"><\/span>Eyelid and conjunctiva<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>Anterior blepharitis involves the eyelid margin and eyelash line. Signs include:<\/p>\n<ul>\n<li>Swelling and thickening of the eyelid margin.<\/li>\n<li>Redness and dilated blood vessels.<\/li>\n<li><span class=\"term\" data-term-id=\"385\">Scaly<\/span> remains at the base of the eyelashes<\/li>\n<li>Loss of eyelashes (<span class=\"term\" data-term-id=\"175\">madarosis<\/span>) or misdirected eyelashes.<\/li>\n<\/ul>\n<p><span class=\"term\" data-term-id=\"239\">Later<\/span> Blepharitis involves obstruction of the ducts and loss of the meibomian glands (2). This leads to:<\/p>\n<ul>\n<li>Conjunctival hyperemia (dilated conjunctival vessels)<\/li>\n<li>Cloudy secretions<\/li>\n<li>Papillary and <span class=\"term\" data-term-id=\"107\">follicular<\/span> tarsal plate reactions<\/li>\n<li>Conjunctival scarring<\/li>\n<li>Chalaziones (<span class=\"term\" data-term-id=\"1076\">granulomatous<\/span> inflammatory lesions around the meibomian glands)<\/li>\n<li>Hordeolum externum (sty, i.e. eyelashes <span class=\"term\" data-term-id=\"145\">infection<\/span> with Staphyloccocus aureus)<\/li>\n<\/ul>\n<h3><span class=\"ez-toc-section\" id=\"Cornea\"><\/span>Cornea<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>An inflamed cornea (keratitis) is a rare but serious ocular complication of rosacea and can threaten vision (1).<\/p>\n<ul>\n<li>Keratitis can affect one or both eyes.<\/li>\n<li>The thinned cornea becomes inflamed with superficial stippling. <span class=\"term\" data-term-id=\"536\">erosions<\/span>, increased blood vessels and opacities.<\/li>\n<li>Keratitis due to ocular rosacea tends to start at one edge or bottom of the eye and then spread to affect the lower half of the two-thirds of the cornea.<\/li>\n<li><span class=\"term\" data-term-id=\"805\">Recurrent<\/span> Attacks lead to corneal thinning, increased corneal opacity, and vessel invasion.<\/li>\n<li>Severe eye disease can rarely result in descemetocele (deep <span class=\"term\" data-term-id=\"290\">ulcer<\/span>) and corneal <span class=\"term\" data-term-id=\"1431\">drilling<\/span>.<\/li>\n<li>Corneal disease may include the formation of pannus (a layer of <span class=\"term\" data-term-id=\"753\">vascular<\/span> fibrous tissue) and phlyctenules (allergic cornea <span class=\"term\" data-term-id=\"367\">nodules<\/span>)<\/li>\n<\/ul>\n<h3><span class=\"ez-toc-section\" id=\"Iris-y-esclerotica\"><\/span>Iris and sclera<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<ul>\n<li>Iritis: inflamed iris<\/li>\n<li>Episcleritis: inflammation of the layer between the conjunctiva and the cornea.<\/li>\n<li><span class=\"term\" data-term-id=\"977\">Scleritis<\/span>: inflammation of the white of the eye<\/li>\n<\/ul>\n<p>Iritis, episcleritis, and scleritis are rare in ocular rosacea. Symptoms and signs are nonspecific, so diagnosis is more difficult in the absence of cutaneous rosacea.<\/p>\n<p>Blepharitis can also be caused by <span class=\"term\" data-term-id=\"378\">seborrheic<\/span> <span class=\"term\" data-term-id=\"53\">dermatitis<\/span>, a scaly skin condition and inflammatory papules on and around the eyelids may be due to <span class=\"term\" data-term-id=\"1559\">periorificial<\/span> dermatitis.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFCuales-son-las-opciones-de-tratamiento-para-la-rosacea-ocular\"><\/span>What are the treatment options for ocular rosacea?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<h3><span class=\"ez-toc-section\" id=\"Tratamiento-conservador\"><\/span>Conservative treatment<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<ul>\n<li>Lid hygiene: diluted baby shampoo, diluted baking soda <span class=\"term\" data-term-id=\"1119\">solution<\/span> or the proprietary preparation can be applied with a cotton swab to clean the eyelid margins and remove <span class=\"term\" data-term-id=\"320\">scale<\/span> along the lash line.<\/li>\n<li>Warm compresses on closed eyelids for five to ten minutes daily can improve the flow of secretions from the meibomian gland, chalazion, and stye.<\/li>\n<li>Artificial tears (eye lubricants) reduce symptoms due to dry eye.<\/li>\n<li>Avoid wearing contact lenses if they irritate the inflamed eye.<\/li>\n<\/ul>\n<h3><span class=\"ez-toc-section\" id=\"Actual-antimicrobianos\"><\/span><span class=\"term\" data-term-id=\"1330\">Current<\/span> <span class=\"term\" data-term-id=\"1712\">antimicrobials<\/span><span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>Anterior blepharitis can be successfully treated with various topical antiseptics and antibiotics including:<\/p>\n<ul>\n<li>Erythromycin<\/li>\n<li>Metronidazole<\/li>\n<li>Azithromycin<\/li>\n<li>Bacitracin<\/li>\n<li>Polymyxin B<\/li>\n<li>Fusidic acid<\/li>\n<\/ul>\n<h3><span class=\"ez-toc-section\" id=\"Actual-antiinflamatorio-agentes\"><\/span>Current <span class=\"term\" data-term-id=\"1413\">anti-inflammatory<\/span> agents<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<ul>\n<li>Topical nonsteroidal anti-inflammatory agents such as ibuprofen may be helpful.<\/li>\n<li>Topical steroids are used short-term to treat marked inflammation of the eyelid or rosacea keratitis. Long-term use of topical steroids should be avoided as they can lead to <a href=\"https:\/\/unovision.com.ar\/cirugia-de-glaucoma\/\"><span class=\"term\" data-term-id=\"975\">glaucoma<\/span><\/a> and <span class=\"term\" data-term-id=\"927\">waterfalls<\/span>.<\/li>\n<li>Topical cyclosporine is typically used in ocular rosacea that has not responded to topical steroids.<\/li>\n<\/ul>\n<h3><span class=\"ez-toc-section\" id=\"Sistemico-antibioticos\"><\/span><span class=\"term\" data-term-id=\"380\">Systemic<\/span> antibiotics<span class=\"ez-toc-section-end\"><\/span><\/h3>\n<p>The following oral antibiotics are used for ocular rosacea:<\/p>\n<ul>\n<li>Tetracyclines such as doxycycline.<\/li>\n<li>Macrolides such as erythromycin<\/li>\n<\/ul>\n<p>They reduce bacteria, improve the stability of the tear film and normalize the secretions of the meibonic glands.<\/p>\n<p>Oral antibiotics are usually continued for 6 to 12 weeks, and then slowly tapered over the course of one to two months.
