<span class=\"term\" data-term-id=\"623\">thrombocytopenia<\/span>?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>There are two forms of heparin-induced thrombocytopenia, type 1 and type II. <span>Type I heparin-induced thrombocytopenia is harmless, causing a mild and transient reduction in <span class=\"term\" data-term-id=\"1089\">platelet<\/span> count.<\/span><\/p>\n<p>Heparin-induced thrombocytopenia type II (HIT) is a more serious disease <span class=\"term\" data-term-id=\"691\">autoimmune<\/span> disease that causes a low platelet count and paradoxical blood clotting.<\/p>\n<ul>\n<li>\n<span>Heparin-induced thrombocytopenia<\/span> type II occurs in 1 to 5% of patients started on heparin.<\/li>\n<li>It usually occurs 5 to 10 days after starting the medication.<\/li>\n<li>It is characterized by a platelet count of <150 x 10<sup>9 9<\/sup>\/ l (or a reduction \u2265 50% in <span class=\"term\" data-term-id=\"616\">platelets<\/span>), which occurs in &gt;90% of HIT patients.<\/li>\n<\/ul>\n<p>Heparin can also lead to other <span class=\"term\" data-term-id=\"47\">cutaneous<\/span> reactions, including:<\/p>\n<ul>\n<li>\n<span class=\"term\" data-term-id=\"1278\">Ulceration<\/span> due to other causes of heparin induced <span class=\"term\" data-term-id=\"403\">necrosis<\/span>.<\/li>\n<li>Injection site <span class=\"term\" data-term-id=\"90\">erythema<\/span>and <span class=\"term\" data-term-id=\"492\">eczematous<\/span>painful or itchy <span class=\"term\" data-term-id=\"368\">plates<\/span>\n<\/li>\n<li>\n<span class=\"term\" data-term-id=\"308\">Generalized<\/span> <span class=\"term\" data-term-id=\"943\">hypersensitivity<\/span> reactions like <span class=\"term\" data-term-id=\"440\">anaphylaxis<\/span><span>,<\/span> <span class=\"term\" data-term-id=\"349\">acute<\/span> <span class=\"term\" data-term-id=\"1646\">urticaria<\/span>and cutaneous <span class=\"term\" data-term-id=\"1091\">vasculitis<\/span>.<\/li>\n<\/ul>\n<h2><span class=\"ez-toc-section\" id=\"%C2%BFCuales-son-las-caracteristicas-clinicas-de-la-trombocitopenia-inducida-por-heparina\"><\/span>What are the clinical characteristics of heparin-induced thrombocytopenia?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Two thirds of people with <span>heparin-induced thrombocytopenia<\/span> develop a reduction in platelet count without symptoms.<\/p>\n<p>However, approximately 30% of patients with heparin-induced thrombocytopenia develop <span class=\"term\" data-term-id=\"769\">thrombosis<\/span> (heparin-induced thrombocytopenia with thrombosis or HITT). Thrombosis can occur in veins, arteries or small vessels in the skin, in the brain (causing stroke), in the legs (causing deep vein thrombosis), in the lungs (causing<span class=\"term\" data-term-id=\"1009\">pulmonary<\/span> <span class=\"term\" data-term-id=\"82\">plunger<\/span>), in the heart (myocardium <span class=\"term\" data-term-id=\"382\">heart attack<\/span>), or in fingers and toes (digital necrosis). Blood clots block the affected vessel causing necrosis.  <\/p>\n<p>Skin necrosis manifests itself as acutely <span class=\"term\" data-term-id=\"811\">demarcated<\/span> tender red or purple patches (<span class=\"term\" data-term-id=\"250\">purple<\/span>) that develop black centers and can cause dryness <span class=\"term\" data-term-id=\"1286\">gangrene<\/span>. A retiform, network-like or branching pattern of purple may also be seen. The Fc-binding protein in the antibody complex matches the Fc receptors on platelets, activating them. Platelet <span class=\"term\" data-term-id=\"1450\">aggregation<\/span> and consumption continues. Platelet levels are reduced from pre-treatment levels. If platelet aggregation is severe enough, it causes the formation of a \u201cwhite clot\u201d that results in a complete blockage of the affected artery or vein. The specific symptoms depend on the organ to which the blood supply is blocked.<\/p>\n<p><span class=\"term\" data-term-id=\"426\">Antibodies<\/span> Take the time to develop into autoimmune diseases. h<span>eparin-induced thrombocytopenia<\/span> It usually occurs 5 to 10 days after starting treatment. Patients who develop heparin-induced thrombocytopenia within hours of starting treatment are likely to have been exposed and therefore sensitized to heparin previously.<\/p>\n<p>Unfractionated heparin is more likely to cause heparin-induced thrombocytopenia than fractionated heparin (also called low <span class=\"term\" data-term-id=\"1851\">molecular<\/span> heparin weight).<\/p>\n<h2><span class=\"ez-toc-section\" id=\"Investigaciones-si-sospecha-de-trombocitopenia-inducida-por-heparina\"><\/span>Investigations if heparin-induced thrombocytopenia is suspected<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Patients receiving heparin should have their blood count measured regularly to check platelet levels. Heparin-induced thrombocytopenia can be diagnosed if there is a reduction compared to pretreatment levels, even when the absolute platelet level is still within the normal range.<\/p>\n<p>Functional tests to detect heparin-induced thrombocytopenia include:<\/p>\n<ul>\n<li>Heparin-induced platelet aggregation (HIPA) test<\/li>\n<li>Serotonin release assay (SRA).<\/li>\n<\/ul>\n<p>Immunoassays to detect antibodies against heparin-platelet factor 4 are more sensitive but less specific compared to functional tests.<\/p>\n<p>Skin <span class=\"term\" data-term-id=\"24\">biopsy<\/span> of skin necrosis can help in the diagnosis. In heparin-induced thrombocytopenia, there are blocked arteries, veins, and small <span class=\"term\" data-term-id=\"953\">blood vessels<\/span> without <span class=\"term\" data-term-id=\"148\">inflammation<\/span>. <span class=\"term\" data-term-id=\"128\">Histology<\/span> cannot distinguish white clots formed by platelet plugs from red <span class=\"term\" data-term-id=\"850\">fibrin<\/span> clots<\/p>\n<h2><span class=\"ez-toc-section\" id=\"Tratamiento-para-la-trombocitopenia-inducida-por-heparina\"><\/span>Treatment for heparin-induced thrombocytopenia<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Heparin injections or infusions should be discontinued.<\/p>\n<p>Alternative anticoagulation may be required to prevent life-threatening thrombosis. This usually occurs with factor Xa inhibitors, such as danaparoid, or direct thrombin inhibitors, such as bivalirudin or dabigatran.<\/p>\n<p>There have been several cases of warfarin-induced skin necrosis when warfarin monotherapy has been used in patients with heparin-induced thrombocytopenia. This is thought to be because warfarin reduces the activity of protein C. Therefore, warfarin alone is not recommended in patients with active heparin-induced thrombocytopenia.<\/p>\n<p>Reexposure to heparin is not recommended during the acute phase of heparin-induced thrombocytopenia and the post-acute period while circulating antibodies are still present.
