Cuadro clínico

Es una entidad poco frecuente conformada por múltiples lesiones quísticas de hasta 5 cm. de diámetro que se localizan preferentemente en cara anterior de cuello y tronco, axilas, raíz de miembros superiores, inferiores e ingles.

Los esteatocistomas se consideran hamartomas del folículo pilosebáceo. Existen dos formas clínicas:

• Esteatocistoma solitario o simple, lesión única, sin tendencia hereditaria.
• Esteatocistoma múltiple o sebocistomatosis, caracterizado por la presencia de numerosos esteatocistomas en una única o en diferentes áreas corporales.

El esteatocistoma múltiple o sebocistomatosis afecta a ambos sexos por igual. Puede presentarse en forma esporádica (en la mayoría de los casos) o heredarse con un patrón autosómico dominante. En algunos pacientes las lesiones se tornan evidentes poco después del nacimiento, se incrementan en número y tamaño durante la pubertad y hasta la edad adulta, momento desde el cual permanecen estables. Esto podría sugerir un estímulo androgénico.

El estudio histopatológico de los esteatocistomas muestra cavidades quísticas que se localizan en la dermis media, con una pared gruesa de epitelio escamoso de 2 a 8 capas de células, sin estrato granuloso, que queratiniza de manera similar a como lo hace el conducto sebáceo normal, lo que da lugar a una gruesa cutícula eosinofílica acelular, ondulada y prominente, con finas espículas hacia la luz. La característica histológica más típica, aunque no imprescindible, es el hallazgo de glándulas sebáceas, lobulillos sebáceos, sebocitos aislados o elementos sebáceos atróficos en el seno de las paredes de los quistes.
Los quistes están completamente cerrados, no se comunican con el exterior ni se abren dentro de los folículos pilosos. Su contenido está representado por corneocitos descamados, sebo y tallos de pelos vellosos.

Tratamiento de Esteatocistoma múltiple o sebocistomatosis

El esteatocistoma múltiple, en cualquiera de sus variantes clínicas, representa un verdadero desafío terapéutico. Dado que las lesiones son benignas y asintomáticas, el principal objetivo es mejorar la apariencia de las mismas, dejando una mínima cicatriz y evitando las recidivas. La incisión y drenaje de los quistes, o la extirpación completa de los mismos, suelen ser los tratamientos de elección. La extirpación quirúrgica es curativa, pero pueden quedar numerosas cicatrices.

Clinical picture

It is a rare entity made up of multiple cystic lesions up to 5 cm. in diameter that are preferably located on the anterior aspect of the neck and trunk, armpits, root of upper, lower limbs and English.

Steatocistomas are considered hamartomas of the pilosebaceous follicle. There are two clinical forms:

• Solitary or simple steatocistoma, single lesion, without inherited tendency.
• Multiple steatocistoma or sebocystomatosis, characterized by the presence of numerous steatocistomas in a single or in different body areas.

Multiple steatocistoma or sebocystomatosis affects both sexes equally. It can occur sporadically (in most cases) or be inherited with an autosomal dominant pattern. In some patients the lesions become evident shortly after birth, they increase in number and size during puberty and until adulthood, at which point they remain stable. This could suggest an androgenic stimulus.

The histopathological study of steatocistomas shows cystic cavities that are located in the middle dermis, with a thick wall of squamous epithelium with 2 to 8 layers of cells, without granular stratum, which keratinizes in a similar way as the normal sebaceous duct does, which gives rise to a thick acellular eosinophilic cuticle, wavy and prominent, with fine spicules towards the light. The most typical histological characteristic, although not essential, is the finding of sebaceous glands, sebaceous lobules, isolated sebocytes or atrophic sebaceous elements within the walls of the cysts. The cysts are completely closed, do not communicate with the outside or open inside the hair follicles. Its content is represented by scaly corneocytes, tallow and hairy hair stems.

Treatment of multiple steatocistoma or sebocystomatosis

Multiple steatocistoma, in any of its clinical variants, represents a real therapeutic challenge. Since the lesions are benign and asymptomatic, the main objective is to improve their appearance, leaving a minimal scar and avoiding recurrences. The incision and drainage of the cysts, or the complete removal of them, are usually the treatments of choice. Surgical removal is curative, but many scars may remain.
