What are rosettes?

Rosettes are a specific form of a shiny white structure seen with polarized dermatoscopy. They are also known as four-spot clods.

What do rosettes look like through a dermatoscope?

Rosettes consist of fOur white dots are arranged in a square that resembles a four-leaf clover and can only be seen in polarized light. They are always oriented at the same angle.

Rosettes in actinic keratosis

In which lesions are rosettes found?

Although in fIt is first believed to be specific to actinic keratosis and scaly cell carcinoma, rosettes are sometimes seen in otherhis lesions as well as on normal skin. Rosettes are most commonly found in:

  • Actinic keratosis (AK)
  • Thin squamous cell carcinoma (SCC)
  • Sun damaged skin
  • Tumors such as basal cell carcinoma (BCC) and melanoma
  • Molluscum contagiosum
  • Dermatofibroma

The exact histological explanation for the rosettes is unclear, but tHey, it is believed that they are due to an iinteraction between polarized and narrow light curb-filling, or fibrotic attached openings Haspeslagh et al found that sMall rosettes (0.1–0.2 mm) corresponded to concentric horny material in follicular and eccrine ducts in the infundibular rosettes level and larger (0.3–0.5 mm) to peripheral concentric fibrosis [1].
