What are dark circles?

Dark circles are a complex condition influenced by various anatomical factors. Below are the main factors that contribute to its appearance:

  1. Fat bags on the lower eyelid.
  2. Malar fat volume loss due to aging or genetic factors.
  3. Lower eyelid lengthening, caused by aging or lack of volume in the malar region.
  4. Groove or tear canal, produced by aging or constitutional predisposition.
  5. Skin factors, related to the texture, color and wrinkles of the skin.

Most patients with dark circles have a lack of volume in the malar region. To effectively correct dark circles, it is essential to restore lost volume in this area.

Aesthetic Impact of Dark Circles

Dark circles make the face less attractive, giving an appearance of tiredness and sadness. They are often believed to be difficult to treat, but each case can be addressed with specialized and personalized treatment.

Treatment with Hyaluronic Acid

Fillers with hyaluronic acid, when applied correctly, offer an immediate improvement, not only in dark circles, but also in the aesthetics of the eyelid and cheekbone. This treatment attenuates eyelid bags and the tear canal, significantly improving facial appearance.

Study cases

Patient 1

screenshot-2024-06-02-at-18-48-20-2-8621402

This patient had bags in her lower eyelid, which were removed through a transconjunctival incision, a procedure that leaves no visible scars. To improve the appearance of the skin, a carbon dioxide laser was used to lighten and tighten the skin. Additionally, volume was added to the malar region with hyaluronic acid. All of this was done in less than 30 minutes under local anesthesia and sedation.

Process:

  • Removal of bags from the lower eyelid through transconjunctival incision.
  • Carbon dioxide laser skin rejuvenation.
  • Volume with hyaluronic acid in the malar region.

Result:

  • Clearer and tighter skin.
  • Restored volume in the malar region.
  • Quick procedure, without visible scars.

Academic works

Patient 2

In this case study, the same procedure that had previously been performed on another patient was performed on Patient 2. This treatment focuses on the correction of certain facial features to improve aesthetic appearance, focusing primarily on the vertical linea nigra that extends from the tarsal edge to the palpebromalar junction.

Preoperative and Postoperative Observations

PHOTO 2-A

  • Preoperative:
    • The vertical black line runs from the tarsal edge to the palpebromalar junction.
    • This line is long, which is considered unsightly because it gives the appearance of a long eyelid occupying the malar region.
  • Postoperative:
    • The black line is shorter.
    • The eyelid is shorter, which is a sign of beauty and youth.

  • Preoperative:
    • In the photo on the left, you can see the lack of projection of the cheekbone and a lower eyelid that extends and occupies the malar region.
  • Postoperative:
    • In the photo on the right, you can see a projection of the cheekbone and a shortening of the lower eyelid.
    • The patient acquired a natural beauty.

  • Preoperative:
    • An oval is drawn that extends from the eyebrows to the palpebromalar junction.
    • In the photo on the left, the oval is vertical, which is an unsightly sign.
  • Postoperative:
    • In the photo on the right, you can see a horizontal oval.
    • A natural projection of the cheekbone is observed, absence of bags in the lower eyelid and no visible scars.
    • There is a projection of the cheekbone and a shortening of the lower eyelid.
    • All of this represents natural beauty, youth and a fresh, rested expression.

Patient 3

This patient had dark circles under her eyes due to aging, with eyelid bags and wrinkles, as well as a lack of volume in the malar region. An advanced eyelid surgery technique was performed without visible scars, combined with carbon dioxide laser facial rejuvenation and malar volume augmentation.

Process:

  • Eyelid surgery without visible scars (Hoogstra technique).
  • Facial rejuvenation with carbon dioxide laser.
  • Increase and projection of the malar.

Result:

  • Elimination of bags and eyelid wrinkles.
  • Significant improvement in malar volume.
  • Rejuvenated appearance without visible scars.

This technique was published in the International Society of Plastic Surgeon in 2017, being internationally recognized as an advance in eyelid surgery.

Clinical features:

  1. Fatty bags in the lower eyelid.
  2. Long lower eyelid.
  3. Lack of projection and volume of the cheekbone.
  4. Tear channel.
  5. It does not present wrinkles or skin aging.

Patient Preferences:

  • You do not want to treat the lower eyelid bags at this time.

Treatment Performed

Procedure:

  • Application of two ampoules of hyaluronic acid, one on each cheekbone.

Immediate Results Observed:

  1. Attenuation of the bags of the lower eyelid due to the increase in volume in the cheekbone, which reduces the protrusion of the bags.
  2. Disappearance of the tear canal.
  3. Significant improvement in the aesthetics of the cheekbone.
  4. Shortening of the palpebromalar distance.

Professional Comments:

  • The treatment is considered almost magical because it is painless, fast, outpatient, with natural results and without adverse effects.
  • The result could be improved with one more ampoule of hyaluronic acid, but it is preferred to advance slowly and allow the patient to decide about increasing volume.
  • Avoid treating patients who desire excessive volume that may result in an unnatural appearance.

Duration and Additional Benefits:

  • The filling must be repeated every 20 months, although sometimes it lasts longer.
  • Hyaluronic acid not only provides volume, but also hydrates and stimulates collagen, which can reduce the amount needed over time.

Treatment Reversibility:

  • If the result is not satisfactory, hyaluronidase can be applied to degrade the material.
  • Until now, it has not been necessary to use this enzyme in patients treated at this center.

Patient Presentation

Clinical features:

  • Depression that extends from the inner corner of the eye to the cheek, known as the tear groove or trough.
  • Triangular-shaped lower eyelid whose tip goes into the sulcus.
  • Long eyelid, “rained” and tucked into the cheek.
  • Lack of cheekbone projection, probably hereditary.

Aesthetic Effects:

  • Appearance of sadness, tiredness and old age due to the aforementioned factors.

PHOTO C

screenshot-2024-06-02-at-19-13-34-6492058

Procedure:

  • Application of a filler ampoule.

Immediate Results Observed:

  1. Improved aesthetics of the cheekbone.
  2. Disappearance of the tear canal.
  3. Shortening of the eyelid; the palpebromalar junction is closer to the eye than the cheek.
  4. Attenuation of lower eyelid bags.

Professional Comments:

  • Patients are fascinated by the speed, the immediate result, the naturalness, the absence of bruising and the indolence of the treatment.
  • Sometimes more than one or two syringes are needed to obtain the desired result. You start with a smaller amount and evaluate if more volume is necessary in a subsequent session.

Complementary Treatment:

  • Helios depigmentation laser: It attenuates skin color and treats blemishes, in addition to stimulating collagen and improving the appearance of the skin. A minimum of 6 sessions is recommended, one every 15 days or one per month. For more information on dark spot treatment, Click here.

Frequent questions

  • Is the application of hyaluronic acid painful? Not at all.
  • Should I rest after the procedure? There's no need.

[:in]One of the most effective, immediate and lasting results for the treatment of dark circles is the filling of the cheekbones and tear canal.

Dark circles removal solution

The Dr. Ricardo Hoogstra development this treatment after proving that many of the patients who had dark circles consulted by lack of volume cheekbone or between the cheekbone and nose, known as the tear channel area.

The lack of volume of the cheekbone and / or the tear canal allows the skin of the eyelids to extend to the cheeks, this increase in the length of the eyelid and its dark color increases the dark circles.

The time of completion of the procedure 5 minutes, painless, great patient satisfaction, which immediately continues its normal life.

Causes

According to the histopathological studies, this alteration of the coloration of the skin of the eyes is due in most cases to an excessive production of melanin and to the dilation of the capillaries near the surface of the skin.

These alterations usually follow the anatomy of the orbicularis muscle of the eyes surrounding the orbit. We receive many young patients who report a great aesthetic concern about the dark color of their eyes. The color of the eyelids and the anatomical characteristics of the orbit gives these patients an aspect of tiredness, sadness and old age, despite the fact that most of them are very young.

Triggers

  • Hereditary dark circles, family history: dark circles have a genetic predisposition.
  • Dark circles produced by repeated episodes of irritation.
  • Dark circles in patients with diseases such as hyperthyroidism, heart, kidney, etc.
  • Dark circles due to skin aging: excessive exposure to the sun increases the melanin of the eyelids.
  • Dark circles produced by light effect in patients with prominent malar bones.
  • Hormonal changes: they also influence the color of the skin of the eyelids, this is how dark circles are accentuated days before or during the menstrual period and during pregnancy.

Dark circles characteristics

  • The color of the dark circles can be brown, violet or black.
  • The brown and black color occurs mainly in patients with dark skin, while the violet color in patients with light skin.
  • Dark circles can be presented as a single entity in very young patients or associated with fine wrinkles, deep grooves or skin festoons. At the same time it can occur without excess of eyelid bags or with protrusion of them.

Treatments available for dark circles

  • Moisturizers and Antioxidants.
  • Decongestants
  • Lymphatic drainage
  • Peeling with different acids.
  • Laser depigmenting Helios II.
  • Radio frequency with micro needles (Scarlet).
  • Erbium laser (Lotus II).
  • Carbon Dioxide Laser ( Firexel ).
  • Carbon dioxide laser assisted surgery.
  • Stuffed with vololuma.

How to know which is the most appropriate treatment for you?

We have verified that the majority of patients suffering from dark circles have a lack of development of the malar bone, especially in its part closest to the nose. Until a while ago the correction of this anatomical situation was difficult given that we had to resort to surgery with prosthesis placement.

Melanin Deposit

Today, thanks to the development of heavy chain hyaluronic acids such as Voluma, we can provide a very fast, very effective and satisfactory solution for the patient. The cheekbone filling procedure with Voluma takes 5 minutes , is painless and the patient can immediately check the result. The interesting thing about this treatment is that it not only improves the color of the eyecup, it also improves the overall aesthetic appearance of the patient.

Other treatments that we perform to improve the dark color of dark circles are with the Helios II depigmenting laser. This is done monthly or biweekly, does not cause irritation of the treated skin and you can continue immediately with your activity. From the third session you can see an improvement in skin color, texture and tonism.

The Laser Helios is completely painless, has no adverse effects, lightens skin color and removes dark spots. For more information, we invite you to visit  dark spots on the skin .

If you are looking for a quick and effective result, the recommended treatment is peeling with the Fire-Xel carbon dioxide laser.

The Carbon Dioxide Laser is used in patients who have dark circles and wrinkles. For more information on this procedure we invite you to visit  Peeling with Carbon Dioxide Laser .

As we see in this histopathological study, the melanocytes responsible for the dark color of the eyelid skin are located in the dermal epidermal junction.

How does the Carbon Dioxide Laser work?

The Fire-Xel carbon dioxide laser in addition to having a skin analyzer allows us to choose the depth of the ablation. For young patients, without wrinkles and in which there is only dark pigmentation of the eyelid skin, we use a 600 micron piece that allows us to exfoliate to the basement membrane.

The most compact and strongest tone tissue below the basement membrane is the thermal collagenizing effect at the dermal level .

This biopsy clearly shows how ablation occurs only in the most superficial layer of the skin, more specifically at the basement membrane level. These patients will have a very fast recovery because the thermal effect is superficial and the result will be excellent and lasting.

For those patients who in addition to the dark color have excess skin of the eyelids, the laser must be used with the 300 micron piece with which we can reach the deep collagen and contract the skin.
The tensor effect on the skin increases as the months go by and lasts for many years.
