Aggressive pathology of digital papillary adenocarcinoma

Introduction

Aggressive digital papillary adenocarcinomas are acral tumors, which have a broad morphological spectrum and can cause diagnostic confusion. Previously, tumors were divided into adenoma and adenocarcinoma based on atypia, but this has been revised: even soft tumors can behave in a evil one Fashion.

Histology of aggressive digital papillary adenocarcinoma

In aggressive digital papillary adenocarcinoma, sections show a dermal basis tumor, which can be connected to the top layer epidermis (Figure 1). The tumor may be cystic, papillary, ductal or solid. The example illustrated here has predominantly solid and ductal. morphology with only focal papillary formation areas. The cells are basaloid, show expanded atypical nucleilarge and enlarged nucleoli mitosis (Figures 1-6). Scaly metaplasia it can be seen inside the tumor (figure 5).

Special studies of aggressive digital papillary adenocarcinoma

Generally none is needed. In the cases found in lymph nodes immunohistochemical studies to rule out metastasis Breast, thyroid, or gastrointestinal sites may be required.

Differential diagnosis of aggressive digital papillary adenocarcinoma

Metastatic adenocarcinoma: a clinical history and immunohistochemical studies may be necessary to rule out a dermal or nodal metastasis from another site.

Benign attached Tumor: Some of these tumors can be deceptively smooth. The acral site, infiltration of deep structures and epidermis and the growth pattern helps to reach the correct diagnosis (malignant).
