Eye bags

Eye bags

What are dark circles?

Dark circles are a complex condition influenced by various anatomical factors. Below are the main factors that contribute to its appearance:

  1. Fat bags on the lower eyelid.
  2. Malar fat volume loss due to aging or genetic factors.
  3. Lower eyelid lengthening, caused by aging or lack of volume in the malar region.
  4. Groove or tear canal, produced by aging or constitutional predisposition.
  5. Skin factors, related to the texture, color and wrinkles of the skin.

Most patients with dark circles have a lack of volume in the malar region. To effectively correct dark circles, it is essential to restore lost volume in this area.

Aesthetic Impact of Dark Circles

Dark circles make the face less attractive, giving an appearance of tiredness and sadness. They are often believed to be difficult to treat, but each case can be addressed with specialized and personalized treatment.

Treatment with Hyaluronic Acid

Fillers with hyaluronic acid, when applied correctly, offer an immediate improvement, not only in dark circles, but also in the aesthetics of the eyelid and cheekbone. This treatment attenuates eyelid bags and the tear canal, significantly improving facial appearance.

Study cases

Patient 1

screenshot-2024-06-02-at-18-48-20-2-8621402

This patient had bags in her lower eyelid, which were removed through a transconjunctival incision, a procedure that leaves no visible scars. To improve the appearance of the skin, a carbon dioxide laser was used to lighten and tighten the skin. Additionally, volume was added to the malar region with hyaluronic acid. All of this was done in less than 30 minutes under local anesthesia and sedation.

Process:

  • Removal of bags from the lower eyelid through transconjunctival incision.
  • Carbon dioxide laser skin rejuvenation.
  • Volume with hyaluronic acid in the malar region.

Result:

  • Clearer and tighter skin.
  • Restored volume in the malar region.
  • Quick procedure, without visible scars.

Academic works

Patient 2

In this case study, the same procedure that had previously been performed on another patient was performed on Patient 2. This treatment focuses on the correction of certain facial features to improve aesthetic appearance, focusing primarily on the vertical linea nigra that extends from the tarsal edge to the palpebromalar junction.

Preoperative and Postoperative Observations

  • Preoperative:
    • The vertical black line runs from the tarsal edge to the palpebromalar junction.
    • This line is long, which is considered unsightly because it gives the appearance of a long eyelid occupying the malar region.
  • Postoperative:
    • The black line is shorter.
    • The eyelid is shorter, which is a sign of beauty and youth.

  • Preoperative:
    • In the photo on the left, you can see the lack of projection of the cheekbone and a lower eyelid that extends and occupies the malar region.
  • Postoperative:
    • In the photo on the right, you can see a projection of the cheekbone and a shortening of the lower eyelid.
    • The patient acquired a natural beauty.

  • Preoperative:
    • An oval is drawn that extends from the eyebrows to the palpebromalar junction.
    • In the photo on the left, the oval is vertical, which is an unsightly sign.
  • Postoperative:
    • In the photo on the right, you can see a horizontal oval.
    • A natural projection of the cheekbone is observed, absence of bags in the lower eyelid and no visible scars.
    • There is a projection of the cheekbone and a shortening of the lower eyelid.
    • All of this represents natural beauty, youth and a fresh, rested expression.

Patient 3

This patient had dark circles under her eyes due to aging, with eyelid bags and wrinkles, as well as a lack of volume in the malar region. An advanced eyelid surgery technique was performed without visible scars, combined with carbon dioxide laser facial rejuvenation and malar volume augmentation.

Process:

  • Eyelid surgery without visible scars (Hoogstra technique).
  • Facial rejuvenation with carbon dioxide laser.
  • Increase and projection of the malar.

Result:

  • Elimination of bags and eyelid wrinkles.
  • Significant improvement in malar volume.
  • Rejuvenated appearance without visible scars.

This technique was published in the International Society of Plastic Surgeon in 2017, being internationally recognized as an advance in eyelid surgery.

Patient 4

Clinical features:

  1. Fatty bags in the lower eyelid.
  2. Long lower eyelid.
  3. Lack of projection and volume of the cheekbone.
  4. Tear channel.
  5. It does not present wrinkles or skin aging.

Patient Preferences:

  • You do not want to treat the lower eyelid bags at this time.

Treatment Performed

Procedure:

  • Application of two ampoules of hyaluronic acid, one on each cheekbone.

Immediate Results Observed:

  1. Attenuation of the bags of the lower eyelid due to the increase in volume in the cheekbone, which reduces the protrusion of the bags.
  2. Disappearance of the tear canal.
  3. Significant improvement in the aesthetics of the cheekbone.
  4. Shortening of the palpebromalar distance.

Professional Comments:

  • The treatment is considered almost magical because it is painless, fast, outpatient, with natural results and without adverse effects.
  • The result could be improved with one more ampoule of hyaluronic acid, but it is preferred to advance slowly and allow the patient to decide about increasing volume.
  • Avoid treating patients who desire excessive volume that may result in an unnatural appearance.

Duration and Additional Benefits:

  • The filling must be repeated every 20 months, although sometimes it lasts longer.
  • Hyaluronic acid not only provides volume, but also hydrates and stimulates collagen, which can reduce the amount needed over time.

Treatment Reversibility:

  • If the result is not satisfactory, hyaluronidase can be applied to degrade the material.
  • Until now, it has not been necessary to use this enzyme in patients treated at this center.

Patient Presentation

Clinical features:

  • Depression that extends from the inner corner of the eye to the cheek, known as the tear groove or trough.
  • Triangular-shaped lower eyelid whose tip goes into the sulcus.
  • Long eyelid, “rained” and tucked into the cheek.
  • Lack of cheekbone projection, probably hereditary.

Aesthetic Effects:

  • Appearance of sadness, tiredness and old age due to the aforementioned factors.

PHOTO C

Procedure:

  • Application of a filler ampoule.

Immediate Results Observed:

  1. Improved aesthetics of the cheekbone.
  2. Disappearance of the tear canal.
  3. Shortening of the eyelid; the palpebromalar junction is closer to the eye than the cheek.
  4. Attenuation of lower eyelid bags.

Professional Comments:

  • Patients are fascinated by the speed, the immediate result, the naturalness, the absence of bruising and the indolence of the treatment.
  • Sometimes more than one or two syringes are needed to obtain the desired result. You start with a smaller amount and evaluate if more volume is necessary in a subsequent session.

Complementary Treatment:

  • Helios depigmentation laser: It attenuates skin color and treats blemishes, in addition to stimulating collagen and improving the appearance of the skin. A minimum of 6 sessions is recommended, one every 15 days or one per month. For more information on dark spot treatment, Click here.
